• To develop collaborative relationships with in-network practitioners resulting in increased satisfactionand adherence to evidence-based guidelines and standards of care.

• To identify and arrange for services not typically covered under member’s health insurancebenefits such as: medically tailored meals when appropriate; community health workers to assistmembers to appointments; and potential approval of health careservices that are not currently covered benefits.

• To improve member health outcomes and quality of life.

What expectations does Neighborhood Health Plan have of me?

We encourage our member’s providers to be a support in helping the patient reach their care plan goals. You may hear from one of the Care Circle team members to help with coordination of care and/or services. We believe that with an “all hands on deck” approach, we can help these members reach their individual goals and, in turn, improve their quality of life.
