What is the Neighborhood Care Circle program?

Neighborhood Care Circle is a free member-centered community based care management program for NHP members with the highest utilization. We aim to increase engagement with these historically hard-to-reach members through the use of novel approaches and meet a member’s unique bio-psycho-social healthcare needs to improve overall quality of life. We accomplish this through a dedicated interdisciplinary team. The t eam is mobile and is poised to leverage many “feet on the street” care management supports to meet the needs of these members in nearly any location

How does the Neighborhood Care Circle program work?

team will develop an appropriate care plan and work with your patient to address their needs and achieve goals that include enhancing access to appropriate care and improving his or her quality of life. team consists of a Medical Care Manager, a Behavioral Health Care Manager, Social Care Manager, Community Health Worker, and a Clinical Support Coordinator. The team ha consultation services available to them from a Nutritionist, a Medical Director, Rehab Specialist and Addictions/Recovery Coach.

My patient is enrolled in the NCC program, how will this benefit me?

Our program objectives include addressing the unique needs of individual members through a comprehensive team approach—and doing so with important input from providers.

Goals of the NCC program:

• To reduce emergency room and acute hospital admissions/readmissions through enhanced communicationbetween behavioral, social, and medical providers resulting in increased coordination/transitions ofcare for members.

• To improve adherence with Primary Care treatment and encourage the continued use of preventiveand screening programs which decrease the occurrence, emergence, or worsening of medical andbehavioral health disorders.

• To develop collaborative relationships with in-network practitioners resulting in increased satisfactionand adherence to evidence-based guidelines and standards of care.

• To identify and arrange for services not typically covered under member’s health insurancebenefits such as: medically tailored meals when appropriate; community health workers to assistmembers to appointments; and potential approval of health careservices that are not currently covered benefits.

• To improve member health outcomes and quality of life.

What expectations does Neighborhood Health Plan have of me?

We encourage our member’s providers to be a support in helping the patient reach their care plan goals. You may hear from one of the Care Circle team members to help with coordination of care and/or services. We believe that with an “all hands on deck” approach, we can help these members reach their individual goals and, in turn, improve their quality of life.
