Wellwork Duri. – IP condition : Concious, burn injury grade 2 and 3, 1 %
on right and left hands; grade 1 and 2, 1 % on left knee. Cardiography OK;
– He is still being observed in Medical. – Description– The IP was working together with crane operator to unload the cement
from cargo truck which was parked below 112.5 kVA power line. After the crane boom was erected, he was attempting to put lifting sling that hang on at the crane to the material pallet, he fell down on the cargo due to electrical shock transmitted from the lifting sling. Then, on concious condition he stood up, run to rig office and reported the incident to Wellsite Manager (company man) that was on rig office. Injured person (IP) was brought to Duri medical for further treatment. The IP got skin burnt on his right and left hands also on left knee. He was still under observation when this report is made.

– On Friday 4 August 2006 at 14:50, FOCO Truck 4X1297 loaded with a unit of agitator which the height is higher than the normal position of the boom of the FOCO Truck. The swamper assumed that the FOCO Truck can get out from Mill Wright Shop safely without making sure the abnormal condition of the boom versus the height of the rail door beam. So, the swamper together with the operator get in the FOCO and give no guidance to the operator when suddenly they heard the boom of the FOCO Truck hitting the beam of rail door caused the beam bent. No damage at the FOCO Truck and no injury

othersDescriptionOn Tuesday, 08 August 2006 at 23:15, on Rig SZP#02, a foco truck was about to transport equipments (tool box and access control post). After loaded both equipments on the foco deck, the foco truck's operator forgot that the boom was still lengthen 75% of full length and the sling was still linked to access control's post hanger. He then continued to drive the truck towards PH-41, and after passed intersection (about 400 meters from access control's previous position) the boom cut off one of three over pass power line cable. The personnel managed to escape from the truck with no injury. Small spark of fire came out from cut-off power line cable which then burned grass (naturally extinguished in short time).
