11/5/2009 F/O 645 Main St Reporter stated she observed a male and female walking down the street with the male repeatedly attempting to strike the female. Reporter notified PSD, but upon arrival victim and subject had left the scene. Search of area yielded negative results.

Victim advised concierge that subject would turn off lights in the hallway. Search of area yielded negative results.

HAZARDOUS CONDITION11/11/2009 F/O 200 Main St Reporter stated she saw a syringe needle on the ground in front of

location. Upon arrival search of area yielded negative results for syringe and reporter was not on scene. Hospital police notified.

INVESTIGATIONS (NON-CRIM.)11/3/2009 550 Main St Reporter was contacted from a previous arrest where subject was in

possession of reporters license. Reporter stated license was missing from 2008. Reporter filed a report with the 115 pct. Previously.

11/3/2009 20 River Rd Three males reported leaving the group home without authorization. They went to the F train. Social worker stated they have done this before and came back shortly after. Just wanted a report to be filed.

11/4/2009 400 Main St Victim stated bus pulled up on the curb almost hitting her. Bus supervisor notified.

11/6/2009 546 Main St Victim entered PSD to report she had given RISAR money for uniforms to become a volunteer, but did not receive a receipt for the $60 she paid after she requested one several times during her training. Victim will file a complaint with civil court.

11/8/2009 580 Main St NYPD notified PSD of possible water rescue. EMS on scene who stated large debris had floated away from a dock in the Bronx. Nothing was observed in the water.

11/9/2009 510 Main St Victim stated on two separate occasions someone was trying to enter her apartment with a key banging on the door. Victim was advised to contact PSD while the incident was occurring. Area will be monitored at times of occurrences.

11/9/2009 546 Main St NYPD Detective came into PSD to obtain information on victim who filed a previous report of an attempted robbery and sustained injuries from being stabbed. Detective was unable to contact victim via house visit and will attempt to contact victim again in the future. Incident reported five hours later in lower Manhattan.

11/12/2009 580/560 Main St Tenant reported unknown youths ringing her doorbell in the middle of the night. Search of area for subjects yielded negative results.

11/12/2009 625 Main St Unknown female tenant complained of smell of illegal substance. Search of area for evidence of debris or smell yielded negative results.

11/13/2009 Adj. 888 Main St Victim complains of subject harassing her regarding her son's broken leg from a soccer game wanting victim to pay her doctor's bills.

11/13/2009 645 Main St Victim (youth #1) and Subject (youth #2) involved in altercation where victim was running away from subject at a school function. Subject was intimidating and asked to leave premises. Victim was escorted home by security and was followed by subject. Subject was advised he was not allowed at any more school functions.

11/15/2009 540 Main St Victim stated while delivering her papers unknown male jumped out and starting fondling himself with his pants down. Search of area for subject yielded negative results. Victim later called PSD again to report same unknown male at another location. Victim was advised to go to PSD before delivering her papers in the morning for an escort.

11/18/2009 580 Main St Victim stated subject comes to her house at all hours of the night asking for money. Parties were advised to stay away from each other.

11/19/2009 F/O 560 Main St Two male youths had left their foster Mother and youths taken into PSD for precautionary measures. Foster Mother knew the location of youths.

11/21/2009 645 Main St Victim reports he asked subject to hold his watch while playing a sport and never returned it to him. Subject states he misplaced it and will look for it. Victim advised to go to civil court if watch is not found.

11/23/2009 625 Main St Reporter stated paper lady complained of unknown male revealing his private parts to her at two separate locations. PSD has escorted delivery person in search of subject.

11/23/2009 F/O 546 Main St Reporter stated Red Bus did not pick her up at stop. Reporter stated she followed bus to next stop and again bus did not pick her up. Bus Supervisor notified of situation.

11/24/2009 30 River Rd Reporter stated package was delivered to wrong address by doorman and concerned the incident will happen again. Building management notified.

11/24/2009 546 Main St Reporter stated unknown male knocked on her door asking for food donation. Reporter was fearful and called PSD for a report.

11/26/2009 540 Main St Reporter stated her husband took out the lock and chain from front door. Reporter did not know why. This is an ongoing issue between them. Otherwise all is in order.

11/26/2009 Opp 475 Main St Reporter stated male and female videotaping Con Edison Power Plant. Information was gather from subjects and determined they were film students filing a student movie.

JUVENILE RELATED (INCIDENTS)11/13/2009 506 Main St Youth and babysitter were separated at location. Youth entered PSD,

babysitter called PSD and babysitter was then escorted to PSD to pick up youth. Mother of youth advised.

11/14/2009 580 Main St Reporter (foster mother) advised PSD youth left dwelling and went to visit his grandmother then returned knocking on the door in an irate manner and then left. Search of area for youth yielded negative results. Grandmother of youth later called to state youth was at her house. NYPD on scene.

LOCK-OUT (VEH / PREMISE)

LOST/MISSING PROPERTY11/9/2009 546 Main St NYPD on scene for victim reporting her laptop computer missing from

apartment while she went shopping. Victim states her apartment door is easily broken into and she uses a slam lock to secure her door. NYPD deemed case as lost property.

11/9/2009 Unknown Victim reported he lost his wallet containing various credit cards and $120 in cash at unknown location. Victim was advised to cancel his credit cards. Search of area yielded negative results.

11/10/2009 100 Main St Victim stated she lost her note pad with all of her personal information from her pocketbook. Search of area yielded negative results. Victim will check with lost and found at location.

11/28/2009 575 Main St Victim left wallet on table at location. Youth found wallet and brought into PSD. Victim contacted and collected wallet later that day. All contents in wallet except for $5

To be referred to Civil Court.11/15/2009 465 Main St Parked vehicle damaged to another vehicle parking at location and

damaging rear bumper. NYPD refused.11/18/2009 527 Main St Reporter stated three male youths running around location damaging a

portrait hanging on the wall. Youths brought into PSD and released to their parents. NYPD refused.

11/24/2009 688 Main St Reporter stated damage to her vehicle. Search of area for similar damage to other vehicles yielded negative results. NYPD report filed.

RECOVERED PROPERTY11/4/2009 510 Main St Black tote belonging to victim was returned.

SMOKE ALARMS / CONDITION / FIRE11/2/2009 625 Main St FDNY on scene for fire in oven caused by burnt food. Fire was contained

to the oven in the kitchen. Minimal damage and no injuries to report. FDNY extinguished fire.

11/10/2009 405 Main St FDNY on scene for fire in apartment cause by burnt food on stove. Fire was put out by FDNY and damage to apartment caused by water damage. No injuries, but tenant was relocated to a new apartment until condition can be corrected.

11/14/2009 540 Main St Smoke condition caused by food left to burn on the stove. No injuries or property damage. Windows opened to allow smoke to clear.

11/26/2009 560 Main St Smoke condition caused by spill from cooking turkey in oven. No damage or fire to report.

11/30/2009 551 Main St Fire at location. FDNY on scene. Fire extinguished by units. All apartments evacuated. Adjacent apartments allowed to reoccupy the premises. Apartment involved in fire could not be re-entered until Fire Marshalls investigated the scene. Fire Marshalls arrived on scene the next day for investigation.

SHOPLIFTING

UNSECURED (VEH / PREMISE)11/3/2009 580 Main St Reporter stated vacant apartment door left open. Super secured lock.

Construction workers did not secure the door after they left.11/5/2009 504 Main St Reporter stated unknown male trying to gain entry to location. Upon

arrival door was unsecured and search of premises found all to be in order. Door was then secured by R/O.

11/5/2009 625 Main St While on patrol R/O observed several items outside of apartment in hallway. R/O notified building management.

11/17/2009 250 Main St Employee of location had obstructed the access control panel enabling members to get into location. Condition corrected.

VEHICLE ACCIDENT11/10/2009 688 Main St Reporter stated as she was parking her vehicle at location she mistakenly

stepped on the gas, the car jerked forward and she hit the guard rail. Guard rail was snapped apart. Vehicle sustained damage to front bumper. No other property damage or injuries. NYPD report refused.

11/18/2009 540 Main St Victim stated she was struck by the drivers side mirror of vehicle. No injuries were sustained from vehicle but victim was thrown to the ground and twisted her ankle. Victim transported to Mt. Sinai. Accident report filed with NYPD.

WATER LEAK11/4/2009 510 Main St Water from unknown source was found on floor and furniture. Super

contacted and matter will be inspected further.11/5/2009 605 Main St Reporter stated water leak coming from the ceiling. Source of leak could

not be found. Building Super will come by again in the morning.

11/9/2009 595 Main St Water leak coming from apartment due to washing machine attached to the sink. Damage to rugs in the hallway on lower floors. Super on scene.

11/9/2009 550 Main St Water leaking from ceiling at location. Source of leak could not be located at the time. Source of leak was later identified and condition was corrected.

11/13/2009 546 Main St Water leaking at location in hallway between the bathroom and kitchen. Source of leak could not be identified. Super on scene. Water may have come from toilet to apartment above.

11/15/2009 546 Main St Water leak at location due to broken toilet. Super on scene and condition being corrected.

11/17/2009 550 Main St Water leak at location from ceiling. Building management corrected the condition.

11/22/2009 540 Main St Water leak at location coming from washing machine. Super corrected condition.

NOISE11/15/2009 200 Main St ARREST R/O observed vehicle with a modified exhaust to increase noise

level. Subject purposely allowed vehicle to create unreasonable noise. Summons issued on scene.

11/4/2009 595 Main St Victim received numerous calls from unknown caller located in California. R/O called the number in question and spoke to a woman who stated she was seeking counseling for her problems and would stop calling.

11/6/2009 2 River Rd Victim stated she was receiving numerous calls from unknown caller. Also previously had someone knocking on her door four times.

11/9/2009 556 Main St Victim stated next door neighbor has been threatening her since she moved in and accusing her of having an affair with her sons father. Victim tried getting an order of protection but did not have enough evidence. Victim was advised to contact PSD or NYPD immediately if an incident occurs.

11/11/2009 580 Main St Victim stated female tenant came out yelling obscenities and threats at him. Building super notified. No injuries. This incident is from a previous incident where female subject assaulted victim and subject was taken to hospital as an EDP.

11/12/2009 Opp 688 Main St Victim stated subject got out of his vehicle, shouted profanity and threats at him and drove off. Victim stated he will call PSD if another incident occurs. Vehicle information was taken.

11/20/2009 579 Main St Victim stated female subject entered location, made a scene demanding him to leave as she has an order of protection against him. Subject left location.

11/23/2009 688 Main St Victim reports subject accused him of hitting his car. PSD did a complete analysis of the situation and informed the subject that it was not possible. Subject was satisfied and left the scene.

11/28/2009 688 Main St Reporter stated Motorgate attendant has been tampering with her vehicle. No witnesses.

11/30/2009 348 W 48th St Victim stated subject approached her in an irate manner. Victim has an order of protection against subject. Reporter refused NYPD currently but will go to precinct on her own.

DISORDERLY CONDUCT11/1/2009 609 Main St ARREST Door attendant stated she observed two subjects, one breaking a

glass door with his fist. PSD arrived at his residence placed both in custody, one subject released and the other who broke the window was brought to 114 pct for processing.

11/7/2009 F/O 579 Main St ARREST Subject was acting in a disorderly manner and when instructed to stop subject refused and continued. Subject was transported to PSD issued summons and released.

11/18/2009 580 Main St Two male youths being disorderly at location. Youths stated they were locked out of apartment. No answer on door. Youths would not comply with lawful order and brought into PSD and released to guardian. Guardian later reported youths were being disorderly and were transported to hospital for evaluation.

11/21/2009 F/O 563 Main St ARREST Subject panhandling at location. Subject was prohibiting customers from entering and exiting the store. Subject issued summons.

11/23/2009 500 Main St ARREST After vehicle accident driver became irate and disorderly. Was issued summons for disorderly conduct and failure to yield to pedestrian.

DOMESTIC DISPUTE11/1/2009 540 Main St Husband and wife involved in dispute. Wife stated husband grabbed her

and pushed her to the ground. Wife is 5 months pregnant, but stated she was fine. EMS and NYPD refused.

11/2/2009 560 Main St Mother reported her son does not reside with her and took the key to her apartment. Super advised and changed the locks.

11/3/2009 4 River Rd Reporter stated her two daughters drink from morning until night constantly fighting and wants them removed. Her other daughter is the lease holder and R/O informed the mother that unless the lease holder wants them out they cannot be thrown out. Daughter said her two sisters are not being thrown out.

11/4/2009 540 Main St Husband and wife involved in a dispute regarding the top lock of their apartment door. Husband does not have a copy of the key and wife had agreed to make one for him.

11/6/2009 580 Main St NYPD ARREST Wife is 5 months pregnant and stated husband smacked her earlier in the day and then later in the evening proceeded to pull her hair, smack her and throw her to the ground. Domestic report filed with NYPD.

11/8/2009 I/O 560 Main St Mother stated her son is not obeying her rules. When she confronts him for discipline he becomes belligerent, cursing and screaming at her. R/O spoke to son for him to comply, but son refused. Mother was instructed to contact PSD if another incident occurs.

11/13/2009 595 Main St Mother and son arguing over going to the doctor. Son stated parents are hearing impaired and argument escalated. All in order upon departure.

11/15/2009 555 Main St Male and female involved in dispute. NYPD on scene. Female stated she was filing a report with NYPD. No injuries to report.

11/15/2009 555 Main St Mother and son involved in a dispute as son hit the daughter earlier in the day. Parties refused NYPD and EMS. No visible injuries observed.

11/17/2009 546 Main St Husband and wife involved in a dispute. Husband stated wife was yelling at him, but also that she was unable to walk and talk. EMS notified, however no need for medical attention. Door secured by victim.

11/19/2009 Opp 750 Main St Husband sought out PSD to speak to another man spending time with his wife. R/O spoke to gentleman and wife with the husband. Husband became irate and threatening. R/O suggested couple seek counseling. Wife stated she is seeking a divorce. Husband left without incident.

11/29/2009 580 Main St Foster mother in dispute with two foster sons. Youths were being disrespectful. Victim notified 911. NYPD on scene. Both youths left the apartment. Victim stated she would call agency in the morning.

OTHER VIOLATIONS11/22/2009 Opp 688 Main St Subject was observed disobeying stop sign. Upon stop driver could not

provide valid license. Subject taken into custody and subjects girlfriend was contacted to bring in identification. Subject issued summonses and released.

11/29/2009 415 Main St Vehicle parked on sidewalk. Towing company called, but owner of vehicle arrived and paid towing fee on scene. Vehicle was released to owner.

AGG. HARASSMENT

PETIT LARCENY11/3/2009 688 Main St Bicycle stolen from location after being locked and secured. Search of

area yielded negative results.11/14/2009 Adj. 30 River Rd Bicycle stolen from location valued at apx $600. Search of area for bicycle

yielded negative results. Victim will file a report with NYPD.

11/28/2009 686 Main St Victim stated GPS system valued at $500 missing from vehicle. NYPD refused. Victim will report incident to 114 pct on his own.

CRIMINAL MISCHIEF11/3/2009 F/O 510 Main St Reporter stated unknown subject threw bottle of water onto the street. No

injuries. Search of area yielded negative results. 11/5/2009 580 Main St Report of broken glass door. Building management notified. Condition

corrected by building maintenance.11/6/2009 560 Main St R/O observed shattered patio door at location. Tenant stated it happened

in the middle of the night. NYPD previously on scene and filed a report of incident. Building management will fix the condition in the morning.

11/6/2009 546 Main St R/O observed subject throw a metal object from the window hitting a moving vehicle. Vehicle did not stop. R/O went to location and found subject was an Autistic male who was not aware of his actions. R/O informed family member.

11/7/2009 560 Main St Victim stated unknown subject threw a brick threw her patio door shattering the glass for the second time in two days. Building management notified to correct the condition. NYPD report previously filed. Search of area for subject yielded negative results.

11/10/2009 R/O 500 Main St Reporter stated she witnessed three female youths about 10 years old squeeze through a hole in the fence. Search of area for youths yielded negative results. Caution tape was placed over the area and Grounds Supervisor notified to correct the condition.

11/21/2009 510 Main St Victim stated her mailbox was opened and lock damaged. Building management notified. Search of area for subjects yielded negative results.

11/21/2009 527 Main St Victim stated four unknown youths wrote obscenities in the sign in book at location. Detectives notified.

11/23/2009 510 Main St R/O observed a hole in the wall at location. Super notified to correct the condition. Search of area yielded negative results for subjects.

11/29/2009 455 Main St Reporter notified PSD of fire in garbage can. Fire put out with extinguisher by officer. Reporter stated a group of youths started the fire. No description of youths available.

investigation subject possessed two ziplock bags of marijuana. Subject taken into custody. Marijuana vouchered at 114 pct. Subject issued summons and released.

11/7/2009 F/O 888 Main St ARREST Subject observed smoking a marijuana cigarette. Subject placed into custody and brought into PSD. Female subject is a youth and her father was contacted. Subject issued summons and released to her parent.

11/19/2009 R/O 555 Main St ARREST Subject observed at location with open container. Subject issued summons on scene.

11/21/2009 Opp 625 Main St ARREST Subject observed at location with open container. Subject issued summons on scene.

11/22/2009 200 Main St ARREST Subjects observed at location with open container. Subjects issued summonses and released.

11/22/2009 900 Main St ARREST Subjects observed with open containers at location. One male subject also had ziplock bag of marijuana in his possession. All subjects issued summonses and one subject issued summons for unlawful possession.

11/23/2009 F/O 571 Main St ARREST R/O observed female purchase a cigarette from subject for $1. Subject was issued a summons on scene.

11/28/2009 Opp 688 Main St ARREST Upon car stop unlicensed motorist stated his license was suspended due to not making child support payments. Subject was placed under arrest and transported to Manhattan Central Booking for processing.

GRAND LARCENY AUTO11/2/2009 F/O 200 Main St Reporter stated vehicle was missing from location. Towing companies

and NYPD contacted to see if vehicle was towed or summonsed with negative results. NYPD reported to scene for report of stolen vehicle.

ASSAULT11/29/2009 Manhattan NYC NYPD Investigation - Four subjects involved in a stabbing incident in
