Have you noticed? PANAC now has online registration, a Facebook page, an enhanced website (www.panac.org) with links to seminar details, an updated educational calendar, seminar photos, links to the Pulseline, scholarship information, and much more. As we continue to enhance our professional offerings, we

now accept research or best practice posters for presentation at PANAC seminars. I know that many nurses have not had formal training in research and are intimidated

by the concept of conducting and documenting a study. So, here’s a suggestion. In your place of work, find a new or current area of interest, practice change or an issue, the outcome of which you want to influence and conduct a survey or an audit. Write up your findings in an acceptable study format, then present your results as a poster. This is a good starting point and will help to develop your confidence. The American Nurses’ Association Code of Ethics states, “The nurse owes the same duties to self as to others, including the responsibility to preserve integrity and safety, to maintain competence, and to continue personal and professional growth.”

As a nursing student almost thirty years ago, the need to continue learning was impressed upon me by my instructors and mentors. Every day, new studies are reported and best practice information is published. As a nurse, I feel I am not giving my patients the best possible care if I don’t make an effort to learn everything I can about my area of practice.

Preparing for the CPAN and CAPA exams enriched my practice and motivates me to maintain my certification through continuing education for years to come.

I became certified for my patients. Each one is a child, parent, grandparent, sibling or spouse. If someone I love needs surgery, I want them to receive care from knowledgeable professionals. I believe we owe the same to the patients entrusted to our care.

New Chapters Key to PANAC Success

Last year, PANAC boasted nine Chapters; now there are twelve. The newest are the East Bay Chapter, Contra Costa County, with Chapter Coordinator, Patti Crosby, and the North Coast Chapter with Heidi O’Neil in Eureka, Humbolt County.

This March, the Central Valley’s Visalia Chapter celebrates its first anniversary. From their first 2013 lunch meeting at Café 225, the nine Visalia Chapter founders have grown to 30 active members. It takes only one or two nurses to start a Chapter. Local nurses enthusiastically embrace regular meetings and educational programs. Like Visalia, East Bay first met at a restaurant for lunch, while the North Coast held an educational program attracting 21 founding members.

At new Chapter meetings, PANAC District Directors offer support. District 3 Director Shidah Farahmand attended East Bay’s founding lunch and Helen Fong, District 4 Director, flew to Eureka for North Coast’s first meeting. In District 5, Director Kathy Nichols, former Fresno Chapter Coordinator, regularly attends Visalia meetings.

Build it and they will come. Each chapter decides meeting format, preferred meeting times, and goals. The Visalia Chapter decided on monthly lunch meetings “to gain momentum.” It worked. Soon, more members joined to network and plan educational programs.

Host hospital’s support and use of their BRN provider number is essential to chapter educational offerings. For their first program last July, Adventist Medical Center-Hanford provided free CEs and PANAC provided a Sunday lunch attracting 30 nurses to hear Anesthetic Agents by Hanford CRNA, Richard McGrory. When Visalia’s Kaweah Delta Healthcare District hosts programs, Porterville’s Sierra District Hospital provides the contact hours, thanks to Surgical Services Director, Dee Piazza.

A primary Visalia Chapter goal, developing professionalism, had Certification Coach, Susan Morris, and Dual Certified, Ronda Dyer, leading Visalia and Hanford CPAN/CAPA study groups. In November, the Chapter met for lunch to celebrate our six new CPAN/CAPAs and Dual Certified nurses.

The key to any Chapter’s success is regular meetings (monthly, bimonthly, or quarterly) that involve local nurses and provide venues for networking, education and professional development. With the ongoing support, attendance, and positive encouragement of the Board of Directors, PANAC is one of the ASPAN Components developing upcoming leaders. These Chapter member leaders are the future of PANAC and ASPAN. As last year’s president Shay Glevy pointed out, we are growing our own.

It takes only one or two nurses to start a Chapter.

ASPAN Region 1 Spring Update –See You in Vegas

Here it comes. The Las Vegas National Conference is not only just around the corner—it is in our corner of the world, Region 1!

From all of us in Region 1, we send a special thank you with appreciation to the volunteers, the National Conference Strategic Work Team (NCSWT), Kevin Dill and the National Office

staff, and the NevPANA Board of Directors for an outstanding conference.

Congratulations to the NCSWT, headed by NevPANA and PANAC member, Susan O’Day, MSEd, RN, CPAN from Reno, NV, you all make Region 1 very proud. Deb Ebert, BSN, RN, CCRN, CNOR, CPAN, CAPA is planning something special for Component Night and Lori Silva, RN, CCRN, CPAN is VERY busy scheduling hostesses to make sure no one gets lost or has unanswered questions. Thank you just doesn’t seem to be enough!

Region 1 will be there in force! If anyone who is unable to attend the whole conference, please consider coming for a few days; you won’t regret it. If this is your first National Conference, plan to attend the First Timer’s Orientation on Sunday afternoon or Monday morning for a great overview to make the most of the conference so you don’t miss anything. the Certification Luncheon for CPAN and CAPA nurses. After the afternoon sessions, head for the Exhibit Hall to meet vendors, check out the poster presentations, and see the Silent Auction. Monday evening, plan to attend the ASPAN Development Reception at the Poolside, the premier fundraising event for member scholarships. Tuesday and Wednesday• take time between the educational sessions to view and listen to the poster presentations and Tuesday evening, don’t miss the President’s Reception to greet President Twilla Shrout and enjoy a classy, fun affair with food, drink, and lots of dancing. Thursday• , it’s rise and shine for an early breakfast and the Closing Ceremonies as our next ASPAN President, Jacque Crosson, Arizona, Region 1, shares her vision for the coming year.

Finally, each morning pick up the daily conference bulletin, PArtiCUlars to keep up with highlights and happenings; and check out the Conference Bulletin Board for announcements and postings for news and Component Get-togethers.

Germ Warfare: The Battle Against SSI

(ASHP) published the 89 page Clinical Practice Guidelines for Antimicrobial Prophylaxis in Surgery. This report is a collaboration of ASHP, the Infectious Diseases Society of America (IDSA), the Surgical Infection Society (SIS), and the Society for Healthcare Epidemiology of America (SHEA). These experts reviewed over one-thousand published studies focused on identifying the strongest evidence-based pharmaceutical measures for preventing surgical site infections (SSI). They made the following recommendations for the timing of antibiotic administration:

The recommended administration of the first dose of an antimicrobial is to begin within 60 minutes before the surgical incision. Administration of vancomycin and fluoroquinolones should begin within 120 minutes before surgical incision because of the prolonged infusion times required for these drugs. Intraoperative redosing is needed to ensure adequate serum and tissue concentrations of the antimicrobial if the duration of the procedure exceeds two half-lives of the antimicrobial or there is excessive blood loss (i.e., >1,500 mL). The redosing interval should be measured from the administration of the preoperative dose, not from the beginning of the procedure.

Quality-improvement initiatives and mandated performance reporting are subject to change, so readers of these guidelines are advised to consult their local or institutional quality-improvement departments for new developments in requirements for SSI measures and data reporting that apply to their practice. PANAC Announces Scholarship Recipients

Each year, through the Alice Wise Memorial Fund, PANAC offers scholarships for perianesthesia nurses pursuing certification, advanced degrees, perianesthesia research or conference attendance. This year, eleven PANAC members will benefit.

With the ASPAN National Conference in nearby Las Vegas, there were many who applied for $500 scholarships to assist with the cost of registration. The following ten members hit the jackpot. Congratulations Ernestine Nunes, Visalia; Denise DeFendis, Fresno; Tita Invina, Cerritos; Sheryl Michelson, Cupertino; Monica Laurent, Santa Barbara; Gina Dias, Hanford; Kathy Sim, Rosamond; Esther Sang, Sunnyvale; John Kuykendall, Sunnyvale; and Shay Glevy, San Diego. Member Gloria Luu, San Bruno, received a $250 scholarship for CAPA certification.

The annual application period for the AWMF is October 1 to December 15. Information, applications, and point criteria are available on the PANAC website, www.panac.org.

While celebrating their success, PANAC members Kathy O’Byran, Susie Nunes, Gina Dias and Dora Faulk shared their thoughts about certification and offered advice to those preparing for CPAN and CAPA exams.

“Becoming certified as CAPA justifies what I do. Studying for CAPA really enhanced my practice and empowered me.” Susie Nunes went on to say, “I would recommend a study group for anyone planning to take the test.” All these nurses participated in small study groups and expressed their appreciation for the support, encouragement, motivation and accountability. All thought meeting at a consistent time and at a distraction-free site was critical. At the meetings, Gina Dias said, “we shared personal experiences which helped us to better apply and remember the material.”

Kathy O’Bryan suggested that each member “commit to studying one hour each day and write questions to generate group discussion.” Gina Dias found that creating flash cards was an effective study tool. Dora Faulk gathered abnormal rhythm strips and ABG results for group review. Others found it helpful to search YouTube for additional information on

Pediatric Pearls: The Perils of Phenergan

Dr. Julie Williamson, DO, FAAP, Clinical Assistant Professor of Anesthesia and Pediatics, Stanford, cautioned about the dangers of Phenergan.

Promethazine (PHENERGAN) possesses antihistamine, sedative, anti-motion sickness, and antiemetic effects. It causes sedation and is a known vesicant.

Respiratory Depression—Pediatrics: Promethazine injection should not be used in patients less than two years of age because of the potential for fatal respiratory depression. Severe Tissue Injury, Inc luding Gangrene: Promethazine inject ion c a n c au s e d a m age to tissues regardless of the route of administration. Th i s c a n r e s u l t f rom perivascular extravasation or unintent ional intra-arterial injection. Adverse reactions include burning, pain, phlebitis, necrosis, and gangrene. In some cases, surgical intervention, including fasciotomy, skin graft, and/or amputation have been required.Safe Practice Recommendations: The following strategies should

be considered to prevent or minimize tissue damage when giving IV promethazine:

Limit the dose.• ISMP recommends a starting IV dose of 6.25 to 12.5 mg.Dilute the drug• in 10-20 ml of normal saline to reduce vesicant effects and enable slow administration.Use large patent veins and • check patency of the access site before administration.Administer slowly • over 10-15 minutes.Educate patients• to let you know immediately if burning or pain occurs during or after the injection.

schedule, adding one or two meetings at the end for final review. A list of suggested resources and a study plan is available at www.cpancapa.org, the website for ABPANC, the American Board of Perianesthesia Nursing Certification. Most of the nurses found the new ABPANC practice exams, with references for each question to be very helpful.

PANAC Celebrates PANAW By Ernestine Nunes, RN, CNOR

Per iAnesthesia Nurse Awareness Week (PANAW) was celebrated across the nation February 3-9. Nurses at the PANAC's Winter Conference shared with Pulseline some of the celebrations enjoyed during the week set aside by ASPAN for recognition of our specialty.

Nurses at John Muir Medical Center in •Concord were treated to lunch by their director and Ernestine Hughes shared they enjoyed cake cake and special treats throughout the week.Susie Nunes• and pre-op and PACU nurses at Visalia’s Kaweah Delta District Hospital enjoyed PANAW cookies from their Visalia Chapter. Ernie Nunes, Chapter Coordinator delivered the cookie trays to perianesthesia units in Visalia, Hanford, Selma, Tulare, and Porterville.Arrowhead Regional Medical Center’s • Angelique Weathersby reports having enjoyed lunch provided by management to nurses on each shift.Denise DeFendis• shared that Children’s Hospital Central California celebrated throughout the week with snacks and drawings for prizes on two days and lunch provided by anesthesiologists on an additional two days.At Rady Children’s Hospital –San Diego, the staff got into the spirit •when the OR, anesthesia and SPD departments sponsored a pancake breakfast for PACU staff. Linda Sparks shared that the PACU leadership team sponsored a Panini luncheon as well.Denise Oxford• proudly shared that staff at Adventist Medical Center, Hanford formed a committee to use PANAW funds from management to celebrate each day with a new treat: breakfast sandwiches, salads, nacho bar, cake, and ice cream sundaes. At Porterville Sierra View District Hospital, Surgical Services Director •Dee Piazza and Manager Carie Carillo say they celebrate PANAW every year. This year they did pastries, pizza, an ice cream social, special gifts and on Friday, lunch from their anesthesiologists.

On January 15, 2014, the growing Inland Empire Chapter offered a Celine Peters’ presentation, From Idea to Product: The Role of Nursing in Industry. Various nursing roles in business as well as implications for patient

safety and product development were discussed over a dinner sponsored by CRISI Medical Systems, Inc.

The January 18 Orange County Chapter meeting was a huge success. Peter Fotinakes, MD lead a fully engaged audience in a lively conversation on the hot topic of Sleep Apnea in the Hospitalized Patient. President, Tess Mazloomian reported on PANAC activities and encouraged members to be involved. Coordinators Kathy Dureault and Marlene Jamieson did a fantastic job.

I'm so impressed with the San Diego Chapter. Barbara Demetros reports, “On January 4, 2014, our distinguished guest was ENT surgeon Dr. Brian Weeks who presented a fabulous and informative lecture reviewing surgical techniques from the 1800s through the latest techniques including balloon sinuplasty. Highlights included post-operative complications and key assessments to make.” This was followed by discussion and analysis of an article from a professional nursing journal. Thank you Shay Glevy and Linda Sparks for supporting Barbara in her new role.

District 1 will hold a Mid-Year Seminar on June 28, 2014 at Redlands Community Hospital. Planned topics are Caring for the Perplexing Pregnant Patient: ACLS Adaptations in Pregnancy and The Hospitalized Patient with Pain. Steven Potjer is doing an awesome job coordinating this event. You won't want to miss it!

Remember, “He who hesitates is lost.” I encourage each of you to get involved. Together, we can make a difference. See you in Vegas!

Well color me red and call me proud! On January 25, forty-five nurses heard the call and joined us for our first 2014 mini-seminar, Down to the Bones. Our speakers covered topics from top to bottom: teeth, shoulders, backs, and all

the way down to the toes. They were a hit! This event could not have happened without the Valley Chapter’s awesome team of Anna Jackson, Linda Macdonald-Dominguez, and Amber Trudgeon. Their hard work produced incredible results.

We also talked about expanding our chapters and our venues. Low and behold we had two different hospitals express interest in hosting future seminars. Ready? Get Set. GO! SAVE THE DATE-- April 12, 2014-- more information to follow!

I would also like to celebrate our new CAPA's and CPAN's. Knowledge is power. It is power to give the best care and power to advocate. Congratulations on increasing your power and striving to be the best nurse you can be!

Obamacare: The Impact on Your Practice

Part one of a three-part series

What is “Obamacare?” The Patient Protection and Affordable Care Act (ACA), Obamacare, is a set of health care reforms signed by President Obama in March, 2010. Though the efforts to insure an additional 30 million Americans have dominated headlines, the ACA is a larger set of reforms. Examples of other elements include: elimination of annual or lifetime limits, inclusion of children on their parent’s policy to age 26, mandated preventative health and wellness coverage, and the expansion of Food and Drug Administration (FDA) power to more rapidly approve inexpensive generic drugs.

Nurses are asking, “How will the ACA affect my insurance?” “How will it affect my practice?” “What are the projections for nursing jobs?” “Will it bring an influx of patients to our facilities?” “Can we accommodate them?” “How will people learn the rules of accessing care when insured?” “Are we headed for socialized medicine?” The complications with the opening of the enrollment websites generated additional questions, such as, “If the enrollment went poorly, should we expect trouble with the rest of the program?” “Is this good, or bad for healthcare?”

In this issue, let’s answer where nursing fits in the early stages of implementation. Nurses will play a valuable role on the front lines of the ACA rollout as advocates for patients and their families. For some individuals, this will be the first time in their lives they will have health care coverage. They will need help understanding the choices available when selecting policies and help learning how to access care once they have coverage. Nurses will do what we do best; we will educate.

My term as District 3 Director just started in October and already I have exciting news! On January 18, 2014, the East Bay Chapter had a successful inaugural meeting. I had the

pleasure of meeting nine inspired members who have already planned the next meeting for March 15, 2014 to include a possible talk on the electronic health record (EHR) and its impact on the PACU. More information will be shared as it develops. If you or your co-workers are interested in connecting with the East Bay Chapter, please let me know. I am looking forward to supporting Chapter Coordinator Patti Crosby as we spread the word among hospitals in East Bay area.

The downpour of rain did not deter more than twenty nurses who came for the historic launching of the North Coast Chapter in Eureka. Dr. Eric McGoldrick’s education about shoulder surgeries and tips for preventing shoulder injuries generated much discussion.

I encourage all members in the area to join us May 10, 2014 for the next quarterly meeting and stay tuned for more chapter news. A big shout-out to Chapter Coordinator Heidi O’Neil whose hard work made the new chapter a reality.

After two years of excellent service, Marcy Wolfe is laying down the mantel of Sacramento Chapter Coordinator. Thank you, Marcie, for your dedicated service. Kate Mooney has been very active in planning for the chapter and has secured speakers for the May meeting. Watch for a flyer outlining more details.

The Modesto Chapter hosted education about oral anticoagulants by speaker Pam Lenhert, PharmD on February 20, 2014. Star Yager is planning more educational offerings in the near future, please watch for flyers and on-line postings.

At the March 8, 2014 seminar. Dr. Axelrod, Mary Frances Mullins, Peggy Holbrook, and Sonja Severso were the speakers for Exploring the PeriAnesthesia Practice: Novice to Expert. It was great to see so many of you there!

District 5Kathy Nichols, BSN, RN, [email protected] The counties of Kern, Tulare, Kings, Fresno, Madera, and Mariposa District 5 opened the year collaborating

with AORN to present A Breath of Fresh Air which was attended by more than fifty surgical and perianesthesia nurses. The speakers and their topics were all exceptional and very well

received. Congratulations to Denise DeFendis and Kris Scaffidi for organizing this event.

The Fresno Chapter began its monthly meetings Saturday, February 8, 2014 at Fresno's Phoenician Gardens Restaurant with new coordinator Fran Buchnoff. We celebrated PANAW week and planned events for the year.

The Visalia Chapter, led by Ernie Nunes, continues to have bi-monthly meetings, with every other meeting being an educational offering with free CEU's. In January, when the planned physician presenter was called into surgery, Susan Morris stepped up to present an excellent program on pediatric assessment. Dr. Varshney’s program, Caring for the Spinal Surgery Patient is rescheduled for May 18. Please watch for details.
