TUMOR MARKERSWhat are they?
Are substances usually proteins, that are produced by the body in response to cancer growth or by the cancer tissue itself and certain benign (noncancerous) conditionsDetected in higher than normal amounts in the bloodDetected in higher than normal amounts in the blood, urine, or body tissuesSome tumor markers are specific for one type of cancer, while others are seen in several cancer types
Measurements can be useful – when used along with x-rays, or other tests in the detection and diagnosis of some types of cancer
TUMOR MARKERSMeasurements of tumor marker levels alone Measurements of tumor marker levels alone are not sufficient to diagnose cancer for the following reasons:following reasons:
Tumor marker levels can be elevated in people with benign conditionsTumor marker levels are not elevated in every person with cancer – especially in early stages of the diseaseM k ifi i l f Many tumor markers are not specific to a particular type of cancer

quality requirements relevant to all tumor markerquality requirements relevant to all tumor marker measurements
Pre-analytical requirements :choice of tumor marker, specimen type, specimen timing, sample handling. Analytical requirements :assay standardisation, internal and external quality control, interferences. Post-analytical requirements :reference intervals, interpretation and reporting of tumor marker results
Pre‐analytical quality requirements Reporting of erroneous tumor marker results is more likely to cause undue alarm to patients than is the case for many other laboratory testsother laboratory tests. The laboratory must therefore exercise extra vigilance in ensuring that
correct results are reported.correct results are reported. Errors in the pre-analytical phase reportedly occur ten
times as often as in the analytical phasethe majority of pre-analytical errors for tumor markers will be simple specimen handling errors – e.g. inappropriate sampling handling hemolyzed specimens insufficientsampling handling, hemolyzed specimens, insufficient specimens, and incorrect specimens – and their occurrence should be minimized by adherence to good laboratory practice and assessment in an effective audit cycle.

Specimen stability
Tumor markers are generally stable, but serum or plasma should be separated from the clot and stored at 4°C (short‐term) or ‐30°C (long‐term) as

NACB recommendations for use of tumor markers in routine clinical practice are should be according to locally
agreed protocols, based on established national and international guidelines. l h h
markers in routine clinical practice are summarized in Table 1, PSA should never be measured routinely in females. CA125 should never be measured routinely
Although in certain circumstances tumor markers may aid in diagnosis, speculative measurement of panels of tumor markers (“fishing”) should be discouraged
in males. CA15.3 should only be measured routinely in males with an established diagnosis of breast cancer. Interpretation of subsequent results is aided by discouraged Interpretation of subsequent results is aided by knowledge of the pre‐treatment “baseline” level

Analytical quality requirements The almost universal use of automated immunoassay yanalysers for many commonly requested tests means that responsibility for analytical quality now rests largely with the diagnostic industrylargely with the diagnostic industry, which must meet quality requirements defined by national or international regulatory authorities [e.g. US Food and Drug Administration (FDA) regulations, European Commission In Vitro Diagnostics Directive (IVDD)]Diagnostics Directive (IVDD)].
It is nevertheless crucial for satisfactory measurement of any analyte that laboratories independently monitor their
f f ll b th t th t lown performance carefully, both to ensure that analyzers are being used appropriately and to confirm that individual methods are performing according to specification. p g g pThis is best achieved by implementation of rigorous Internal Quality Control (IQC) and participation in well-designed Proficiency Testing [External Quality Assessment (EQA)] programs (1).
Analytical quality requirements Long-term monitoring presents major challenges as patients may change hospital and laboratories may change the tumor marker methods during the relevant time period. While ideally results obtained in different methods would be fully interchangeable, data from PT schemes confirm that this is not the case with between methodschemes confirm that this is not the case, with between-method coefficients of variation in excess of 20% still observed for some tumor markers .Major causes of observed between-method variation for these complex analytes include poor calibration differences in the specificitycomplex analytes include poor calibration, differences in the specificity of antibodies used, and differences in method design. It should be possible to achieve reasonably standardized and accurate calibration but only for those analytes for which a recognizedcalibration, but only for those analytes for which a recognized International Standard (IS) or Reference Reagent (IRR) is both available and universally adopted by diagnostic manufacturers for primary calibration of their methods. Unfortunately, as yet there are no IS for y, yany of the important CA series of tumor markers, a major gap that should be addressed urgently. Long‐term PT scheme data can also confirm the effect of successful introduction of a new IS. Data from the UK National External Quality Assessment Scheme (UK NEQAS) for PSA, for example, confirm that mean geometric coefficients of variation (which reflect scatter) decreased from 21.9% in 1995, before the 1st IRR was introduced, to 9.5% in 2004

Principle ofPrinciple ofiimmunoassayy
labeled materialAll immunoassays require the use of labeled material in order to measure the amount of antigen or antibody present. A label is a molecule that will react as partof the assay, so a change in signal can be measured in the blood:reagent solution.
Examples of a labelExamples of a label include a radioactive compound, an enzyme that causes a change of color in a solution, or a s bstance that prod ces lightsubstance that produces light. The label can be applied during the manufacture of the reagent to either the antibody (Ab* see Figure 1-5) or antigen (Ag*to either the antibody (Ab , see Figure 1 5) or antigen (Ag , see Figure 1-6).Immunoassay technologies utilize different formats to y gdistinguish the bound antigen-antibody complex from the free unbound label.
Competitive and NoncompetitiveCompetitive and Noncompetitive ImmunoassaysThe measurement of analyte in an immunoassay is achieved by using either
a competitiveor
a noncompetitive format.
Competitive FormatCompetitive Format
In competitive formats,In competitive formats, unlabelled analyte (usually antigen) in the test sample is measured by its ability to compete with labeled antigen in the immunoassay. The unlabeled antigen blocks the ability of the labeled
ti t bi d b th t bi di it th tib d iantigen to bind because that binding site on the antibody is already occupied.
Con’tedh i i i i l l b l d i hThus, in a competitive immunoassay, less label measured in the
assay means more of the unlabeled (test sample) antigen is present The amount of antigen in the test sample is inverselypresent. The amount of antigen in the test sample is inversely related to the amount of label measured in thecompetitive format (Figure 1-7).g
One step competitive formatp pIn the one step competitive format (see Figure 1-8), both the labeled antigen reagent (Ag*) and the unlabeled specimen (or testlabeled antigen reagent (Ag ) and the unlabeled specimen (or test sample analyte) compete for a limited amount of antibody.
Two step competitive format
In the two step competitive format, the antibody concentrationp p , yof the reaction solution is present in excess in comparison to the concentration of antigen. Antibody reagent is first incubated with specimen containing antigens of interest; thenincubated with specimen containing antigens of interest; then in the second step, labeled antigen is added. Remember that in the competitive format, less bound labeled antigen indicates more antigen present in the test sample T o step competiti emore antigen present in the test sample. Two step competitiveassay formats provide several fold improved assay sensitivity compared to one step assay formats.
Noncompetitive (Sandwich) MethodNoncompetitive assay formats generally provide the highest level of assay sensitivity and specificity and are applied to the y ymeasurement of critical analytes such as cardiac and hepatitis markers. This format is referred to as a “sandwich” assay because analyte is bound (sandwiched) between two highly specificanalyte is bound (sandwiched) between two highly specific antibody reagents (Figure 1-10).
N titiNoncompetitive assayNoncompetitive assay formats can also utilize either one step or p y ptwo step methods, as with the competitive assay. The two step assay format employs wash steps in which the sandwich binding complex is isolated andsandwich binding complex is isolated and
washed to remove excess unbound labeled reagent and any other interfering substances.gThe two step noncompetitive format usually offers the highest specificity and sensitivity of all the assay formats di d hdiscussed here.
