Diabetes MellitusObjetivos:

1. Comprobar si el programa ISIS sirve para obtener los datos de calidad que utiliza el Sistema Nacional de Salud del Reino Unido (Quality and Outcomes Framework guidance for GMS contract 2008/09 - National Health Service - NHS)

2. Comprobar el resultado de dichos indicadores en cada consulta de los 13 médicos de familia del Centro de Salud El Greco de Getafe, Madrid

3. Comprobar si cumplimos los estándares de calidad que establece el NHS

4. Implantar sistemas de medida rápidos y reproducibles que faciliten la comparación entre profesionales y el intercambio de información sin esperar a estudios exhaustivos

Objectives:

1. To check whether the ISIS software from Instituto @pCOM® is a useful tool to obtain quality data, comparable to those used in the U.K. N.H.S. (Quality and Outcomes

Framework guidance for GMS contract 2008/09)

2. To obtain the results of such indicators for each one of the thirteen family doctors working at El Greco health centre, Getafe, Madrid

3. To check if we meet the quality standards set by the N.H.S.

4. To implement fast and reproducible measuring systems that would make easier the comparisons between professionals and the information exchange, without having to wait for exhaustive studies

Aclaraciones:

1. Los médicos de familia trabajan en 2 turnos: turno de mañana, de 8 a 15 h y turno de tarde, de 14 a 21h

2. Hay 5 médicos de familia en turno de mañana (en los gráficos: M1 a M6) y 8 en turno de tarde (en los gráficos: T1 a T8)

3. La población de los médicos de familia es mayor de 15 años, los menores de dicha edad acuden a pediatras de atención primaria

4. En el turno de mañana hay 1 enfermera por cada médico de familia, formando 5 equipos. En el turno de tarde hay 6 enfermeras; 2 cupos de médicos de familia comparten varias enfermeras

Explanation:

1. Family practitioners work in 2 shifts: morning, from 8 to 15 h, and afternoon, from 14 to 21 h

2. There are 5 family practitioners in the morning (in the charts: M1 to M6) and 8 family practitioners in the afternoon (in the charts: T1 to T8)

3. The patients seen by family practitioners are over 15 years old, younger patients are seen by primary care pediatricians

4. The morning team has one nurse for each family practitioner, caring for the same patient list. In the afternoon there are 6 nurses, several nurses are shared by the patients of 2 family practitioners

Conclusiones:

1. ISIS es un programa que permite la obtención de los datos de control clínico de nuestros pacientes diabéticos según los criterios de calidad del Framework.

2. Framework no hace referencia al número absoluto de diabéticos ni a la prevalencia, lo que impide comparar las poblaciones.

3. De los 15 criterios para el pago de incentivos en el control de la diabetes mellitus, en 11, las 13 consultas del centro de salud cumplen el criterio para el pago, 3 criterios lo cumplen menos de la mitad de las consultas y 1 criterio ninguna consulta lo cumple.

4. Por lo menos, en el Reino Unido saben hacía dónde dirigir sus esfuerzos clínicos y, gracias a ello, podemos mejorar, incluso sin incentivos económicos.

Conclusions:

1. ISIS is a software that permits obtaining the clinical control data of our diabetic patients according to the quality criteria of the Framework.

2. Framework does not make any reference to absolute numbers of diabetics nor to prevalence, which prevents us from comparing both populations.

3. Out of the 15 criteria for the payment of incentives on the control of diabetic patients, 11 are met by all the thirteen family doctors of our health centre, 3 by less than half of them, and 1 by no one at all.

4. At least, in the United Kingdom there´s a clear idea of where to direct their clinical efforts. Thanks to this we can improve, even without economical incentives.
