Baylor Medical Center at Waxahachie

Baylor Medical Center at Waxahachie (BMCW) demonstrates exceptional performance with several best practices identified. BMCW utilizes a Shared Leadership System that is well integrated throughout the organization. The system empowers participants from all workforce levels and stakeholder groups to affect governance and makes them responsible for the impact of their decisions. The Shared Leadership System is integrated with key work processes; used to deploy and implement Strategic Goals and Action Plans; utilized in the performance improvement process; and used in fostering open two way communication both vertically and horizontally. BMCW has a process to identify and create service offerings to meet the requirements of its customer groups, market segments, enter new markets, and attract potential customers that ties into its strategic planning process. The implementation of the Patient Advisory Council and the work of the Auxiliary are examples of the organization responding to the needs of its stakeholders. BMCW was also noted for their partnerships with the local nursing homes that allows for continuum of care by sharing evidence based medicine and ensuring standardized physician to physician communications regarding patient transition from hospital to nursing home. Baylor Medical Center at Waxahachie is a fully accredited, acute care, 57-bed, not-for-profit, community hospital deeply rooted in the community of Waxahachie, Texas. In 2011, BMCW was recognized as one of the 100 Top Hospitals in America by Thomson Reuters, a leading healthcare industry data analytics firm.

2012 Achievement Level Recognitions

Baylor All Saints Medical Center

Baylor All Saints Medical Center (BASMC) is a subsidiary of the Baylor Health Care System (BHCS). BASMC provides a continuum of care through primary care, specialty care, rehabilitation, senior health and affiliated ambulatory surgery centers. Since opening its doors in 1906, BASMC has continuously served communities located in Tarrant County growing to over 500-bed capacity at its campus in Fort Worth, TX. Consistent with the BHCS vision, BASMC seeks to be distinguished as the place to give and receive care through providing industry-leading Quality and patient centered Service while maintaining Finance operating margins that provide the capability to reinvest in resources to serve the needs of our communities. A focus on the finance pillar has strengthened the net operating margin in 2011 despite increased demands placed on BASMC by uncompensated care due to Medicare and Medicaid. The Senior Administrative Team has been able to create a climate of financial accountability and productivity within all levels of the workforce. BASMC has created a strong philanthropic climate within its community as evidenced by its ability to secure development funds for Andrews Women’s Hospital and the Faxel Funds for Nursing. An ongoing commitment to fund development allows BASMC to achieve and exceed their strategic goals in this challenging economic climate.

Baylor Heart and Vascular Center, LLP Licensed as a 64-bed acute care hospital, Baylor Jack and Jane Hamilton Heart and Vascular Hospital (BHVH) serves the primarily adult population of Dallas patients with cardiovascular disease and related conditions. BHVH provides inpatient and outpatient services, focused cardiovascular disease prevention and preventative health care. BHVH opened in 2002 as the first joint venture hospital in Baylor Health Care System (BHCS). BHCS exists to serve all people through exemplary health care, education, research and community service and holds a vision to be trusted as the best place to give and receive safe, quality, compassionate health care. Research that informs clinical practices and improvements in patient care and services is a core competency of BHVH. The Clinical Research Council consists of nurses who develop and conduct research studies. Examples of studies include sneeze study related to cardiac rehabilitation patients, depression among cardiac patients, and nerve blocks for pain. Physicians participate and lead cardiovascular clinical trials and studies on a variety of topics, including: atrial fibrillation and arrhythmias, coronary artery disease, congestive heart failure, hypertrophic cardiomyopathy, valve disease, kidney disease and the heart, peripheral vascular disease, and pulmonary hypertension. Research is high quality and disseminated through peer reviewed journals. The research conducted by BHVH is translated into clinical practices that improve patient care, save lives, and reduce health care costs.

Fort Bend ISD Welcome to our World—FBISD—A Global Tomorrow guides the district forward to prepare our children and community for success in a global society. Embedded in the Fort Bend Independent School District (FBISD) vision, mission and values are the ideals which support the district’s goals and objectives. The district includes three municipalities and several unincorporated rural communities. Formed in 1959 from the consolidation of two smaller districts, FBISD has become a blend of urban, suburban, and rural communities appealing to families world-wide. As such, the district has evolved into an ethnically and culturally diverse community serving a minority-majority student body. As a majority minority district, seventy-five percent of the students represent countries from all over the world, and more than 100 dialects and languages are spoken. Capitalizing on their core competency of data-informed decision making, FBISD uses Data Teams, which is a systemic process for learning with a dynamic feedback loop. On the campus level, data team members identify students’ curriculum needs through pre-assessment and post assessment analysis, adjusting plans in response to the data. The reviews are used to align instructional decision making with the result of enhanced student achievement. The model includes timely reviews of performance by teachers, campus administrative leadership, Campus Based Leadership Teams, and instructional staff.

Hill Country Memorial

Hill Country Memorial (HCM) is a non-profit, nontax-supported community hospital in Fredericksburg, Texas. When the hospital opened in 1971, HCM leaders recognized the community as the ultimate stakeholder/partner and patients as the ultimate customer. The principle value of others first always is imbedded in the organizational culture. HCM’s main health care service offerings are inpatient, outpatient, and emergency services and are delivered through a coordinated effort of the various organizational departments. Outpatient represents the largest volume but inpatient is still the heart of the organization. HCM is comprised of an 86-bed acute care hospital and a wide spectrum of support services. The Hill Country Memorial Hospital Foundation is a strategic partner which raises philanthropic funds and facilitates support from the HCM key stakeholders—donors and the community. HCM’s senior leaders share a visionary passion of continuous improvement and organizational learning within the entire organization. By using quality tools in all areas, they have inspired an understanding of the urgency of improvement to all stakeholders. Implementing innovative ideas such as an organization-wide presentation of Chasing Zero has inspired workforce buy-in of necessary improvement to challenges of HCM and provided opportunities for rapid creation and implementation of action plans. HCM has a systematic, institutionalized approach to achieve its mission to provide “Remarkable care for our family, friends and neighbors – Always!”

Medical Center of Lewisville

Medical Center of Lewisville (MCL) is focused on providing high-quality care to its community by preventing, diagnosing and treating a variety of medical conditions. The staff is dedicated to providing family-centered care with a talented team of healthcare professionals, the latest technology and a customer-friendly environment. Built in 1976, MCL has grown into a full-service medical and surgical facility offering 24-hour emergency services. Excellent customer service and patient safety are the top priorities. The supportive environment, advanced technologies, and innovative patient care allow physicians and healthcare associates to best treat their patients with the highest quality health care in a comfortable and compassionate setting. MCL uses systematic processes to deliver quality patient care that contributes value to its patients and stakeholders. Multiple approaches are used to listen to the voice of its patients, build patient relationships, and use the information to identify opportunities for improvement. Some of these methods include real-time staff and leadership rounding, bedside shift turnover, comment cards, HCAHPS surveys with peer comparisons, discharge phone calls, Patient Advisory Board, a formal complaint management process, participation in outpatient service offerings, educational seminars, and community events. Also, various methods are used to engage and develop all employee groups. These employees live the organization’s mission to “provide high quality, compassionate, family-centered health care to [the] community.”

Schertz-Cibolo-Universal City ISD

The Schertz-Cibolo-Universal City Independent School District (SCUC) is a fast-growth district with a small-town atmosphere. SCUC is located on the northeast side of San Antonio within a 73-square mile area that includes Bexar and Guadalupe counties. As more and more people discover the high quality of life and education available for residents of the SCUCISD, the district continues to grow. During the last 15 years, the enrollment in the district has more than doubled. The district now serves approximately 13,000 students with a student-teacher ratio of 15-to-1. During the 2010-2011 school year, SCUC celebrated its 50th Anniversary. The district is proud to be amongst a very few in the area to hold a charter for an International Baccalaureate (IB) diploma program. The student is very much at the focus of the efforts of SCUC. SCUC uses a series of common assessments to monitor student learning and adjust district, campus, classroom, and student plans. Performance Excellence has been deployed throughout the organization all the way to the students in the classroom using Continuous Classroom Improvement methods. SCUC has innovative educational programs and services that actively engage their students. The implementation of technology initiatives, such as smart boards, iPads and eBooks in the classroom, support a high level of engagement for the students.

The Texas Quest for Excellence two-day conference will be held June 25-26 at the Embassy Suites Hotel and Conference Center in Frisco, Texas and will feature current and former TAPE and MBNQA recipients. The 2012 Texas Award for Performance Excellence and Achievement Level recognitions will be presented during the Texas Awards Banquet scheduled for the evening of June 25. The Engagement, Commitment, Progress Level recognitions will be announced in a Press Release on April 20 and will be presented during the Lunch Ceremony scheduled for June 26.
