Conducted by the LEA Required if an initial or continuing residential

placement Must be conducted annually Purpose is to determine if the facility can

meet the goals of the student’s IEP And to determine if the facility can meet the

health and safety standards Records of on-site visits must be retained in

the LEA or SSA for program monitors to review when requested

Confirms, on the basis of all options discussed and considered, that residential placement is required.

Addresses the items/questions on the application

Ensures that a representative from the facility attends the meeting or participates by phone or conference call.
