DOCUMENTSAssessments / Documents can only be created when an active Referral is selected and the banner is displayed, showing details of the client and the selected Referral.

WHERE CAN I FIND DOCUMENTS?Documents / Assessments can be found in the following modules / screens within RAISE:

Referrals / Open Cases Assessments / CIN Children in Care Child Protection – Details Pathways Team Adoption – Prospective Adopters / Special Guardians, Children for Adoption Foster Carers Setup – Fostering / SC / SL

1. Ensure you have the selected the client in the Referrals module2. From the Module Bar / Childrens Services

Select one of the modules, eg Assessments / CIN, Pathways Team3. If necessary select the Document tab

CREATE A NEW DOCUMENT/ASSESSMENT

1. Select the Document from the drop-down list available

2. Select/enter a Start Date3. Select the Insert button

COMPLETING A DOCUMENT/ASSESSMENTWhen the selected document is opened any pre-defined fields will be automatically populated, eg Name, Address, DOB4. Complete the necessary free text boxes, eg health, education, family history, analysis5. Use the Spell Check facility to check for spelling errors6. Preview the document by clicking on the HTML Preview button7. Save permanently records the details on the Assessment document8. To print the document

Select the View in Word buttonFrom the File MenuSelect Print…Check options, eg number of copies, print range, printer nameClick on OK

9. On completionSelect the Close button

Personal details are refreshed each time the Assessment document is opened.

If you make any changes to the document whilst Viewing it in Word these changes will not be saved to the original document. If you wish to work on the document off line then use the Export & Import functions – see page 9.

Once an Assessment has been finally ‘closed’ details can be viewed but not amended

To ‘re-open’ a document simply clone it to the same client

UPDATE A DOCUMENT/ASSESSMENTYou can locate an Assessment in a number of ways:

Locate by Client Locate by Referral Locate by Assessment

LOCATE BY CLIENT1. Ensure you have the client selected in Personal Details

If necessary - using Client Search, search for the Client, eg Daisy Flower2. From the list displayed in the lower pane in Personal Details

The Task column indicates where you can find the document eg CIN – Core Assessment / Assessments CIN3. From the Module Bar, select the appropriate module, eg Assessments / CIN4. Double-click the document to edit, save, print and close

LOCATE BY REFERRAL1. From the Referrals / Open Cases module

Search for the Client, eg Daisy Flower2. Select the required Referral3. From the Module Bar

Select Assessments / CIN4. Double-click the required Document, eg CIN – Single Assessment & Intervention5. Edit the document6. Save, Print and Close the document

LOCATE BY ASSESSMENT – SEARCHES THROUGH ALL CLIENTS1. From the Assessment / CIN module

Search for the Client, eg Daisy Flower – if necessary2. From the list displayed in the lower pane

Double-click the required Assessment, eg CIN - Core Assessment3. Edit the document4. Save, Print and Close the document

This method is particularly useful when looking for a particular

type of Assessment commenced or

completed in a certain time frame.

In this example all CIN – Child in Need Plans

commencing during April will be displayed.

CLONING DOCUMENTSWHERE CAN I CLONE DOCUMENTS?

The Cloning button is found in the following modules: Referrals / Open Cases – Document tab Assessments / CIN Children in Care – Document tab Child Protection – Details

Document tab – Record of outcome of Section 47 enquiries document only Other CP Documents tab – Strategy Discussion document only

Pathways Team Adoption – Prospective Adopters / Special Guardians, Children for Adoption – Document tab Foster Carers Setup – Fostering / SC / SL – Document tab

CLONE A DOCUMENTThe cloning process allows the details from a Document / Assessment that has already been created for the primary client to be copied to secondary clients. This is particularly useful when having to write up the same details for a number of children in a family.

The document must have been inserted / created / saved before you can clone it

1. Open the client (primary) who you wish to clone the document fromSelect one of the modules eg Referrals / Open Cases (double-click to open), CiC, Pathways TeamSelect the Document tab if necessarySelect the document to clone – one click

2. Select the Clone button

The system will remind you that this document has been cloned and that you may need to edit the details

Use this method to ‘re-open’ a document by cloning it to the current client

Once opened the document can be ‘viewed in word’ and printed off

DOCUMENT VERSION H ISTORYThis button is found on all screens where documents/assessments are stored, eg Assessments/CIN module. Selecting this button will show a list of current and previous saved versions detailing the date, time and by whom the document was edited. On opening an entry the document which was saved at that time will be displayed. You cannot revert back to a previously saved document but contact RAISE Support who might be able to accomplish this from the database tables.

Ensure you select the document before selecting the Doc Version History button

EXPORT & IMPORT DOCUMENTS – NOT USED BY POOLEThe Export process allows you to save documents, outside of the RAISE database, so that it can be updated off-line. Once the document is exported, it is marked as Checked out to the exporting user, and no more changes can be made to that document within the RAISE system. The exporting user will update the document offline and then Import it back into the RAISE system. At that point it will become freely available to all system users for updating.

WHERE CAN I IMPORT & EXPORT DOCUMENTS?The Export and Import buttons can be found within the following modules/areas:

Referrals / Open Cases – Document tab Assessments / CIN Children in Care – Documents tab Child Protection – Details

Document tab Other CP Documents tab

Pathways Team Adoption – Prospective Adopters / Special Guardians, Children for Adoption Foster Carers Setup – Fostering / SC / SL

EXPORT FUNCTION The Document must have been created before you can export it

1. Open the Client whose document / assessment you wish to export2. From the appropriate screen eg Document tab, Pathways Team screen

Select the document / assessment to be exported3. Select the Export button

Save your work frequently to avoid loss of data

Contact the management Information Officer to re-activate a document that has been lost

4. In the Save As dialogue boxSelect the location and type in the name of the fileClick on Save

5. Open the document offline and complete the relevant details

When working offline the buttons at the top of the document are slightly different.

IMPORT FUNCTIONOnce changes are made to the Exported document, the document must be imported to bring it back into the RAISE system to make it available to all users. There are 2 ways of doing this; one from within Raise and the other from within the open document

IMPORT FUNCTION IN RAISE1. Open the Client whose document/assessment you exported previously2. From the appropriate screen, eg Document tab, Leaving Care screen

Select the document/assessment that was exported3. Select the Import button4. Select Browse – to locate the file5. Click on Import

IMPORT FUNCTION FROM WITHIN THE OPEN DOCUMENT1. Open the document previously exported2. Select the Import button3. Click on OK

The Save locally button changes to Save and the Import button is greyed out

4. You are now back on lineSelect the Save buttonEdit the document if necessary - don’t forget to Save againClick on Close

Team Managers have permissions to close Documents / Assessments

AUTHORISING DOCUMENTS / ASSESSMENTSWhen all the details for the document/assessment/letter are recorded, the document must be completed. The document will have a nominated completion date and when a date is entered in this field a warning is displayed.

Ensure the client has been selected in the Referrals screen

1. From the appropriate screen, eg CiC / Document tab, Pathways Team screenOR1. From the Document Search screen – search for ALL documents – see next page. OR1. From Personal Details, scroll through the documents in the lower grid

2. Open the document to be closed – double click 3. Enter / select the completion date4. Select the Save button

5. If the date was entered in error Click on OK Delete the entered date before selecting the Save button

5. If the date is correct

Click on OK Select the Save button

6. Click on the Close button When the document / assessment / letter is completed in this way, it remains available for viewing. However, the document

is locked for editing and no further updates are possible.

RE-OPENING DOCUMENTS Clone the document to the current client and then send an email to RAISE Support and ask for the non-cloned document to

be deleted.

DOCUMENT SEARCHUse this facility to search for all documents with specific words / contents for a particular client. Team Managers / Senior Practitioners might find this particularly useful when authorising documents. To search for Attached documents use the Attached Documents Overview module.

Ensure you have a client selected in Referrals or Personal Details

1. From the Module Bar, under Childrens ServicesSelect Document Search

RELATED PAPER FILESThis button allows you to record the location of any paper files associated with a Client. Date of entry, description of paper file and its location are filled in the appropriate fields.

ATTACH DOCUMENTSThis button allows you to attach electronic files associated to Clients, eg scanned documents of court hearings, medical certificates. A caption describing the attachment, it’s location in the electronic filing system and the date attached is recorded on the form in the appropriate fields.

Emails holding information on Clients can also be attached as documents by saving them first – see page 21.

When completing the caption to follow the guidelines as set out in the file - found in RAISE Procedures on the LOOP

Once you have attached a document you will need to delete the original document from the folder in your user space

CASE NOTESCase Notes are used to record the day-to-day activity on cases such as a record of visits, telephone calls. In RAISE they also capture types of activity such as resource panel notes, etc.

WHERE CAN I FIND THE CASE NOTES BUTTON?The Case Notes buttons can be found in the following modules of RAISE:

Referrals / Open Cases Adoption – Prospective Adopters / Special Guardians, Children for Adoption Foster Carers Setup – Fostering / SC / SL

Ensure you have selected the client in the Referrals module first.

Select the Case Notes button

The Case Notes List screen is displayed showing you a list of case notes for that particular module, eg Assessments, recorded on the client to date.

Select the Show All check box to view all case notes recorded on the client to date.

MARK A CASE NOTE TO ADD TO CHRONOLOGYWhen recording new case notes you can tick the Add to Chronology ‘yes’ check box. This will then insert a green tick in the Case Notes Listing screen. Existing Case Notes can also be highlighted as a significant event by selecting the case note (tick the check box as if you are printing) and then select the Add to Chronology button at the bottom of the screen.

These case notes are imported in the Chronology (IC) Report.

SHOW ALL CASE NOTESWhen the Show All field is selected, all Case Notes for this client are displayed. Case Notes from all Referrals and from any module to which the user has access will be included in the list.

The list of case notes displayed shows the Referral number and the name of the module in which the case note was originally created.

A case note can only be cloned to a client whose relationship has already been defined in the Personal Details module and has a current referral

CLONE A CASE NOTEA case note is always recorded in a selected module for a specific client. If the same contact needs to be recorded on a related client’s details, then RAISE allows you to copy the recorded case note to the related client’s record. This is done using a process called Cloning.

1. From the Case Notes List screenSelect the note to be copied across to a related client referral

2. Select the Clone button3. In the window

Select the Client / Referral you wish to clone to ORTick the Select All check box

4. Select the Clone button5. Click on OK6. Select the Back button

Note - you are able to clone across to closed Referrals

EDITING A CLONED CASE NOTE1. Search for the client, eg Henry Higgins2. From the Case Notes List screen

COPYING AN EMAIL INTO A CASE NOTEUse this facility to copy the contents of the email into a case note. Grammatical content and spelling should be checked first as case notes occasionally may be needed for Court hearings, family meetings, etc.

STEP 11. From the appropriate screen

Select the Case Notes button2. Insert a new case note or open an existing one3. Complete all relevant sections - except the Case Recording free text box4. Leave RAISE open – drop the screen onto the Taskbar

Select the Minimise button on the top right corner

STEP 25. Open up Outlook – if not already open6. Double-click the email whose contents you wish to copy7. On the Standard toolbar

Select the Forward button8. Select the text to be copied – include the From, Sent, To, CC section (at the top

of the message) as this will give details of dates and times the email was sent, who sent it and the recipients to whom it was sent
