There will be an expectation that this post will work across both primary care and secondary care boundaries with clinical commitment to community hospitals within Moray.

A CCT or equivalent is required for these posts.

Three multi-disciplinary teams will provide the service across Grampian from a variety of different venues, including Jobcentre Plus offices, clients’ homes (when appropriate) and other community venues. Teams work closely with Jobcentre Plus colleagues and other employment focused services within the CHPs and local communities. The final phase of the recruitment process is underway and NHSG now offer the following posts which will form part of the Condition Management Programme Team.

Practitioners Band 6 Will provide an advanced health, social and occupational assessment of Incapacity Benefit Customers presenting with mild to moderate health conditions including but not limited to musculoskeletal, cardio-respiratory and mental health conditions.
