ISD 271 - SLAM REGISTRATION INFORMATION AND DETAILS

1. Online @br.thatscommunityed.comBeginning at 9:00 a.m. on March 15, 2016. Available 24 hours a day until classes are full or registration closes for each program. (Fee assistance recipients cannot use the online regisration system. Fee assistance applicants must request applications and mail in application and registration forms together).

2. By MailMail a completed blue registration form with your payment, either by check, money order (made payable to the “ISD 271”) or credit card information (complete credit card portion of the registration form, including expiration date).

Grade Registration programs use the child’s grade for the fall of 2016.

Transportation is NOT provided for SLAM classes.

Participants must be dropped off and picked up from the class site. Those needing child care before or after SLAM classes may sign up for Extended Care. See page 42.

Attendance: Attendance will be taken in SLAM classes. You will NOT be notified if your child is absent from a class unless you specifically request this information.

SLAM DETAILS AND POLICIESFor programs on BLUE pages

Sickness Policy This policy has been put into place to protect your child(ren) and the other participants in the program. Please use common sense when deciding to send your child to one of our programs. If in doubt, please do not send them.

Please do not send your child to our programs if your child has:

• Had a temperature of 100 degrees or more within the past 24 hours.

• Had a communicable illness such as strep throat, pink eye, chicken pox, measles, etc... unless they have been on antibiotics for longer than 24 hours or past the doctors recommended exclusion period.

• Lice, unless they have been treated and no longer have any nits or eggs visible.

• Vomited within the last 24 hours.

If we discover that one of the above has occurred or the child becomes sick while at one of our programs, you will be asked to make arrangements to pick the participant up immediately from the participant’s location.

Concussion Policy A concussion is a brain injury. Concussions are caused by a bump or blow to the head. Even a “ding,” “getting your bell rung,” or what seems to be a mild bump or blow to the head can be serious. You can’t see a concussion. Signs and symptoms of concussion can show up right after the injury or may not appear or be noticed until days or weeks after the injury. If your child reports any symptoms of concussion, or if you notice the symptoms yourself, seek medical attention right away and please contact the SLAM office to notify us of the injury.

Lost and Found Please label your child’s belongings with both first and last name. This includes things such as sweatshirts, water bottles, sunscreen, swim suits and beach towels. Items without names will remain at the program site until the end of the program.

Severe Weather Policy: SLAM will hold classes indoors if it is an extremely rainy day. If severe weather approaches, children will take shelter in the designated severe weather shelter.

Disability and/or Medical SupportBloomington Community Services actively supports and welcomes people with disabilities and/or medical conditions* to participate in our recreation programs at no additional cost to the participant. If your child would like to participate in a Bloomington SLAM program and feel assistance is needed, please follow the steps below:

1. Select YES on the registration form that states the participant has a disability and/or medical condition* and needs support.

2. Complete the Participant Information Portion on the Registration Form

3. After we receive the registration form and profile, you will be contacted by staff for more information and to establish an inclusion plan, if needed.

Please note: Accommodations can potentially take up to two weeks to put into place after all forms are received. SLAM reserves the right to delay enrollment due to processing registration, the need to increase staffing or to gather additional information about the participant.

*Some medical conditions may require Inclusion Services due to the intensity of the need and/or the medical equipment that will need to be utilized (needles, vials, etc.). Due to liability concerns and safety of participants, inclusion services must be provided even if the participant is self-sufficient with this type of equipment. Inclusion services will provide monitoring and guidance if the participant is self-sufficient. If not self-sufficient, inclusion services will provide the full support needed.

Fee Assistance Limited Fee Assistance is available for SLAM classes at Olson Middle School based on qualification criteria. Partial assistance may be awarded for one class/per child/per quarter, or until fees are unavailable.

Assistance is available to qualifying Bloomington households. Email [email protected] for the application. Applicants must apply before registering for programs. It may take up to two week to process your application.

Extended care is available at Olson Middle School from 6:30am – 6:00pm. A breakfast snack, afternoon snack, and daily activities are included in your tuition. SUMMER MUSICALSLooking for a way to keep your child’s math and reading skills sharp over the summer? Try GRASP!GRASP is a nine week correspondence program for grades K - 8 which helps students maintain reading and math skills while they are on summer vacation. Each lesson is constructed to reinforce or reteach typical basic skills generally found in most curricula at a particular grade level. • One lesson per subject is completed each

week for nine weeks. • Each lesson should take approximately one hour per

week to complete. • Completed lessons are mailed to a scoring center in

the envelopes provided. The lessons are scored, results recorded and returned to the children. Children successfully completing at least seven lessons (70% accuracy) are awarded a certificate. Those successfully completing all nine lessons receive a medal soon after Labor Day.

• Instruction sheets found in each packet provide all the necessary information to complete each lesson.

• Children attending Bloomington Public Schools will have materials sent home with them the last week of school. All others will be mailed in early June.

We suggest parents order the grade level just completed. If there are extenuating circumstances, order the grade level that is appropriate for your child.

SPARKS offers a variety of dance and music lesson for all ages & abilities.

Dance classes are scheduled at various times throughout the year at the Pond Dance Studio. Ballet, Tap, Jazz, Hip-Hop, Zumba and Creative Movement classes are offered by qualified, experienced dance instructors. A year-end recital is held each Spring, for those students participating in the full-year performance dance program, September through May.

Private Music Lessons for piano, voice, guitar, and select band instruments are available after school at a variety of Bloomington school locations throughout the school year and the summer months. Affordable, weekly lessons are 30-minutes in length and taught by experienced, enthusiastic music instructors. To be eligible to register for private lessons, students must be at least 7 years of age for piano, 9 years for voice and guitar, 10 years for all other instrument lessons.

Community Services shall not be liable for any claims, injuries or damages, of whatever nature, incurred by the

participant(s) which are directly or indirectly attributable to the negligence, whether passive or active, of Bloomington

Public Schools, their agents or employees, arising out of, or in connection with the activity or programs. On behalf of

the participant(s) and myself, I expressly release and discharge the City, Bloomington Public Schools, Art Center, their

agents or employees from any such claims, injuries or damages. I also understand this waiver includes any injuries that

may result from the condition of facility used in the activity or program.
