PLEASE PRINT CLEARLY GENERAL INFORMATION HOTEL & TRAVEL Meeting Space: All lecture sessions will be held in the Emory Amphitheatre located on the Garden Level of the Emory Conference Center Hotel. Attire: Attire is business casual. Temperatures in the hotel can fluctuate so please dress accordingly. A jacket or sweater is recommended. Buffet Lunch: Only paid registrants displaying an official conference name badge will be allowed free entrance to the dining room for the complimentary daily luncheon. Little Participants: Quiet infants-in-arms are welcome but we regret that we cannot allow babies older than 6 month or any strollers in the conference room. As a courtesy to other participants, we ask moms to sit near an exit and attend immediately to any situation that may distract others, including happy noises. If attempts to quiet your baby are unsuccessful, please leave the session and return when the infant is settled. In order to fully participate in the educational sessions, some mothers find it easier to bring a support person along to provide care for their children outside the meeting room. Mother/Baby Room: A room with comfortable seating will provide privacy to express milk (bring your own pump). Stop by the conference registration desk for location information. Handouts will be available for download beginning one week prior to the conference for registered attendees. The login instructions for accessing the handouts will be included in your confirmation e-mail. CONFERENCE DESCRIPTION The 28th Annual Conference was specifically designed to broaden perspectives and increase competence of those working in the area of breastfeeding and human lactation. Opportunities abound for caregivers, clinical experts, and researchers to interact with one another making it a rich, lively forum for learning and sharing. Purpose: To protect, promote, and support breastfeeding by providing up-to-date information and evidence-based strategies for those working with breastfeeding families. Objectives: At the conclusion of this conference, participants should be able to achieve the objectives listed under each topic. The conference will be presented in lecture format with question and answer sessions. Who Should Attend? This interdisciplinary meeting is for anyone involved in the care of the breastfeeding family including lactation consultants, physicians, nurses, nurse practitioners, midwives, dieticians, and others involved in perinatal care.

Objective: Implement a simplified breastfeeding initiation strategy designed to reduce the incidence of the three most common problems during the first weeks after birth. Abstract: This presentation begins with a chronological review of recommended approaches to positioning and latch during the newborn period. Current research will be examined to identify which approaches make it easier, which make it more difficult, and why. Included will be the information needed to implement a simplified breastfeeding initiation strategy designed to reduce the incidence of the three most common early problems.

Objective: Discuss what information parents need when evaluating milk-sharing options. Abstract: This session examines the range of milk-sharing options and the ethical and practical considerations for parents and breastfeeding supporters. Options covered include milk banks (for-profit and nonprofit), milk-sharing organizations (such as Eats on Feets and Human Milk 4 Human Babies), and the Wild West of milk sharing, online milk sellers and buyers.

Objective: Identify inherent factors during hospitalization that increase the risk of a newborn fall/drop and potential interventions to prevent newborn falls. Abstract: Dropping a baby after falling asleep or caring for an infant when an accidental injury occurs can be an emotional and life-changing experience for families. Learn how healthcare facilities can make a difference in newborn events by incorporating prevention methods such as family awareness, staff monitoring, and education for both staff and families.

Objective: Identify the unique barriers to breastfeeding to mothers and families in the African-American community and gain a better understanding of how to help mothers of color reach their breastfeeding goals. Abstract: Families of color face unique barriers to breastfeeding due to social, political, and economic factors. This presentation will discuss how to help African-American mother-baby dyads meet their breastfeeding goals.

1:40-1:50 Break 1:50-2:50 Breast Pain: Assessment, Causes, and Treatment Nancy Mohrbacher, IBCLC, FILCA

Objective: Summarize the evidence indicating the most likely causes of breast pain in lactating women. Abstract: Strategies for assessing likely and unlikely causes of breast pain will be provided with an overview of effective treatment. Content will answer the question: Can shooting or burning pain between feedings be caused by ductal candidiasis or are there other possible causes to consider?

Objective: Review the normal anatomy and physiology of infant feeding at the breast. Abstract: The anatomy and physiology of an infant’s oral-motor mechanism is proof that they are born to breastfeed. In this presentation we will review the normal development, anatomy, and physiology of infant feeding. Within this context, we will also explore infant-based feeding difficulties including disordered swallowing.

Objective: Compare and contrast the health outcomes and cost of mothers' own milk (MOM) versus donor human milk (DHM) for premature infants in the NICU. Abstract: Donor human milk (DHM) is the recognized "second choice" when mothers' own milk (MOM) is unavailable for feeding premature infants during the NICU hospitalization. Although multiple research and quality initiatives have tended to include both DHM and MOM within a common "human milk feeding" category, the outcomes and costs of the two feeding types differs in clinically significant ways. This presentation will focus on the evidence and mechanisms for these differences, with an emphasis on the "lack of fit" between MOM from early lactation and DHM that has been pumped during later lactation and subsequently pooled, pasteurized, frozen, thawed, fortified and fed via gavage feeding. Implications for practice and research will be delineated.

9:05-9:15 Break 9:15-10:15 Which Breast Pump for Which Mother? Using the Evidence to Individualize Breast Pump Technology Paula Meier, PhD, RN, FAAN

Objective: Incorporate the degree of breast pump dependency and stage of lactation into decisions about individualizing breast pump technology for mothers. Abstract: Cochrane and other systematic reviews on breast pump technology include a statement about "individualizing" breast pump technology to the specific needs of mothers and infants. However, until recently there were no evidence-based recommendations about how to achieve this individualization. This presentation focuses on incorporating the degree of breast pump dependency (minimal, partial, complete) and the stage of lactation (initiation, coming to volume, maintenance of established lactation) into these decisions. The emphasis throughout is on matching the pump type with the infant's unique and malleable role in milk removal from the breast.

Objective: Provide guidance describing the infectious disease related contraindications against and benefits of human milk consumption and/or breastfeeding to women and colleagues. Abstract: Human milk is the optimal and complete nutritional source for infants offering the additional benefit of immune protection. However, certain situations may arise when the risks of breastfeeding and/or consumption of human milk may outweigh the benefits. These include maternal infection with select pathogens, maternal consumption of medications and other substances, or human milk intolerance in the infant. There are also some situations where maternal immunization may be beneficial to the breastfed infant. This discussion will address the infectious disease related contraindications to and opportunities for human milk consumption and/or breastfeeding. It will also focus on the current guidance regarding human milk consumption and/or breastfeeding in situations where a woman may have an infection with a high consequence pathogen such as Ebola virus or with a Zika virus infection.

Objective: Describe maternal and health care provider outcomes of breastfeeding peer counselor practice in the NICU. Abstract: The Rush Mothers' Milk Club has employed breastfeeding peer counselors (BPCs) as the primary lactation care providers for NICU families since 2005. All BPCs are parents of their own NICU infants, and have completed extensive NICU lactation training. Whereas multiple research publications have addressed the impact of BPCs on lactation outcomes for NICU families, newer studies have highlighted that the BPCs serve a unique and equally valued position as role models for self-described "NICU MOMs" as the new mothers acquire maternal role identity. This presentation will describe the conceptualization, practice and outcomes of breastfeeding peer counselor practice in the NICU.

In this session, we will explore the various scopes of practice—similarities and differences—prevalent in the field.

Professional biography: Tracy E. Herring is a lead Speech-Language Pathologist at Children’s Healthcare of Atlanta at Scottish Rite Hospital. She received her Master of Arts degree in Speech-Language Pathology from Northwestern University in 2001. With 15 years of experience in inpatient and outpatient tertiary care, Tracy specializes in pediatric feeding, swallowing and voice disorders, including fluoroscopic and endoscopic imaging diagnostics. In 2007, she became a Certified Lactation Counselor in order to augment her ability to diagnose and treat feeding disorders in infants. Tracy has served on a variety of multi-disciplinary taskforces to promote increasing Kangaroo Care and breastfeeding rates in the surgical NICU at Scottish Rite. She has presented at conferences both locally and nationally, including the American Speech-Language Hearing Association National Convention.
