Psychological distress:
depression (20-80%)
anxiety (13-64%)
Alexithymia:

AIMS
a) To characterize the clinical profile of
alexithymic FM patients by comparing
them to non-alexithymic ones on:
pain (pain experience – affective vs.
sensory dimensions – and pain intensity)
psychological distress (anxiety,
depression and emotional distress)
b)To investigate, by means of multiple
hierarchical regression analyses, the
relationship between alexithymia and pain,
controlling for psychological distressPain

MEASURES
Pain evaluation:
Questionario Italiano sul Dolore (QUID): pain experience
Item “Pain” of the Italian version of the Fibromyalgia Impact
Questionnaire (FIQ): pain intensity
Psychological assessment:
Hospital Anxiety and Depression Scale (HADS): anxiety and
depression
Distress Thermometer (DT): emotional distress

RESULTS
CLINICAL AND PAIN EVALUATION
QUID-S and QUID-A = Sensory and Affective classes of Questionario Italiano sul dolore; FIQ-Pain = item “Pain” of the
Fibromyalgia Impact Questionnaire

MEDIATION ANALYSIS

CONCLUSIONS (1)
The results of the comparisons between alexithymic vs. non-
alexithymic FM patients on pain measures and psychological
distress, showed in alexithymic patients:
higher scores on pain experience, on both the
affective and sensory dimensions, and pain intensity
higher levels of psychological distress (anxiety,
depression and emotional distress)
Giornata di Studio del Dipartimento – Marialaura Di Tella 20
CONCLUSIONS (2)
a) Positive correlations were found between alexithymia and the
affective, but not the sensory, dimension of pain experience, and
between alexithymia and pain intensity
b) The results of the regression analyses showed that:
none of the alexithymia factors significantly explained pain
intensity’s variance
alexithymia (difficulty identifying feelings factor) ceased to
uniquely predict affective pain, after controlling for
psychological distress, particularly anxiety
c) A significant effect of anxiety in mediating the relationship between
alexithymia and affective pain was finally found
