The Inspection was conducted to docunenty the extent of PCB contaminationof the waste oil storage lagoons, at the Cam- Or Uestvllle facility as firstevidenced during the Initial Inspection 1n June 1984, and to determinethe company's compliance with the PCB Disposal and Marking Regulations(4 CF«, p«rt 761), as published 1n the May 31, 1979 Federal Regulationsand A»en<taents. August 25, 1982, October 21, 1982 and January 3, 1983.

A detailed description of the coapany background 1s presented in the *nitlalInspection report of June 18. 1984 (File 11245). Those Inspectionresults Identified PCB contamination of two lagoons with the highest con-centration at 69 ppo. Because of the United nature of the sampling effortand the potential magnitude of the Indicated PCB problem evidenced from theInitial Inspection results, a second Inspection was conducted that focusedon better determining the extent of the PCB contamination within the wasteoil storage lagoons at the Cam-Or, Uestvllle facility.

A. Opening Conference

TM Inspectors presented their U.S. E?A credentials as well a* theNotice Of Inspection (AtUchMRt A) «nd TSCA Inspection ConfidentialityMttlce (Attachment B). The purpose *nd scope of the Inspection were explained.ATMS covered Included lagoon description and use history, spills andrecordkeeplng. The inspectors requested *nd received copies of the spillreports (Attachment C) *nd, at a later date, copies of theInvoice records on general waste oil shipments (Attachment D).B* ***** 01- Lagoons

A total of 10 lagoons exHt at the Cam-Gr, Hestvllle facility. The Itlagoons range in area from 0.18 to 0.60 surface areas (Table 1).The depth range among B lagoons 1s approximately 2-20 ft., the petroleumsubstance 1n the remaining two lagoons (SL-1 and SL-2) 1s solid in naturedown to an ari«uterm1ned depth (Table 1).

Most of the lagoons are interconnected with one or more lagoons (Tablt 1,also see photographs 1n Initial Inspection). A cooling pond (0.8 surfaceacres 1n size) also exists at the facility for holding waste water fronthe oil/water fractlonatlon process and run off. The pond also containsa notable amount of oil and oil/water Mixture. The lagoons have not beenused for oil storage for the last four years although the coapany 1s stilldrawing oil fro» the lagoon for processing. For the last four years sooeof the lagoons hae been used for water storage when necessary.

One Interesting note uf Interest was the recent drop 1n oil level at USS-1lagoon that w*s apparent during the Inspection. According to Mr. Ah 1 berg.the cap fro* a drain 01pe located at the east end of the lagoon had recentlybroktn and the liquid fron the lagoon had drained into the adjacent RO-3lagoon. However, samples collected from RO-3 lagoon did not reveal any de-tectable amounts of ^CB even though two samples from USS-1 lagoon containedPCBs at 83 and 57 ppm,

C. Recordkeeplng

According to Mr. Ahlberg, the company does not have any records related tooils stored 1n the lagoons. However,the probable source of the sludge 1nthe USS-1 lagoon 1s U.S. Steel Corporation. A PCB analysis log book has beenMaintained at the facility since 1981. The log book Identifies the PCBanalysis for each waste oil tank load delivered to the facility and oilsamples from storage tanks that are shipped to the facility,the date of deliveryto the facility and the source of the oil. According to Mr. Ahlberg, wasteoil containing 5ppm PCB or greater 1s typically rejected and all oil at orabove SG ppm 1s rejected. Based on the numbers of test results that showed thePCB concent ration at or above 50 ppro, rejection of waste oil by the facility1s relatively common.

0. Spills

According to Mr. Ahlberg, four large oil spills have occurr«dtat the facilitysince 1978. The flrt and largest spill occurred 1n 1978 1n which approximately35,000 c|*U of oil and water escaped from the cooling pond and entered CrookedOt«k. 'in* otn«i* ip11U occurred 1n 1960 (less than 1000 gal spilled) and1n May «nd August 1983(1.000 and 2,250 gal spilled, respectively). Spill r*portbwere prepared for the latter two spills (Attachment C).

Dear Mr. Stallseith,

On May 9th. 1983 the Water Pollution Control Facility atCAM-OR Uestville was officially put into operation. Mr. Richard Cleatonof the Indiana State Board of Health was present on that date to acti^tus in the start-up. Ptior to said start-up DAF sr-pies of influentand effluent ware taken and analysis shoved a 97*' efficiency in theremoval of oils, greases and heavy retals.

On May 16th, 1983 an accidental discharge into receiving watersnamed an unnamed ditch, tributary to Crumpacker Creel: occurre.'. Anestimated 2,250 gallons of DAT effluent was pumped into said ditchbetween 1:15 p.n. and 2:30 p.m.

This discharge occurred unbeknownst to management aud WaterPollution Control facility personnel, and was only discovered whenbrought to our Attention by Mr. Gary Thevenow, Indiana State Boardof Health, Division of Water Pollution Control.

On the afternoon of May 18th. 1983 the DAF (Dissolved AirFlotation) unit was operated to ascertain flows aid efficiencyrating**. This testing was conducted by bath Mr. James Stoll of ClowCorporation and our own personnel. Prior to this operation the DAFeffluent was positioned to bypass the aerator and feed into a largeretention lagoon located on the southwest corner of CAM-0?. Wustville'sfacility. A 3" wire reinforced transport hose va: hooked up to the pro-posed outfall site and run into the lagoon. The place-rent of said hosewas checked by Waste Water Treatment personnel at approximately 12:s5 p.m.SoMtiae between 12:45 p.m. and the initial DAP effluent discharge(1:15 p.».) this hose was moved, thus resulting in the accidental dischargeinto the aforementioned unnamed ditch.
