1 Republic of South Sudan CHOLERA RESPONSE UPDATES AS OF 17 July 2017 Coordination of cholera response Overall coordination of the cholera response at the national level is coordinated by the National cholera taskforce that meets weekly on Wednesdays from 2pm in the WHO conference hall in Juba. In addition, the cholera coordination meeting in Juba Teaching hospital convenes on Monday starting 11am and Friday starting 9am in the Postgraduate School. At the sub-national level, cholera taskforce committees are coordinating the cholera response in Yirol East and Yirol West, Nyirol, Uror, Tonj East, Kapoeta South, Kapoeta North, and Kapoeta East. Other non-affected states have also initiated cholera preparedness meetings in Aweil, Torit, Wau, Yambio, and Rumbek. Across the border in Sudan, the week 27, 2017 joint situation report by the Federal Ministry of Health and WHO showed that 15 out of the 18 states have been affected by acute watery diarrhoea with active transmission reported in Kassala, Blue Nile, River Nile, Gezira, Senar, Khartoum, Gedarif, Northern, White Nile, South Kordofan, North Kordofan, West Kordofan, North Darfur and East Darfur. All the states bordering South Sudan are affected. This therefore calls for enhanced preparedness in Renk, Western Bank of the Nile, Pariang, Abiemnhom, Abyei, Aweil East, Aweil North, and Raja counties. To enhance cross-border surveillance and response, cross-border meetings are planned starting with the South Sudan, Uganda, and Kenya meeting that is planned for next month in Nimule. Surveillance and case management updates New cholera alerts Kapoeta: Suspected cholera cases with 6 deaths reported in Lokitala, Jie cattle camp, a remote and swampy village in Kapoeta East. The area is said to be a cattle camp and currently inaccessible. A team comprising MOH, WHO, ARC has been dispatched to the area to respond. General cholera trends The locations with active transmission have not change and remains the following counties; Kapoeta East, Kapoeta South, Kapoeta North, Tonj East, Yirol East, Yirol West, Uror, Nyirol, Panyijiar, and Juba. Reports from the Ministry of Health and Partners in Kapoeta show a decline of cholera cases in Kapoeta East, Kapoeta South, and Kapoeta North. However, accessibility remains a big challenge in the very remote areas in the Kapoetas.

CHOLERA RESPONSE UPDATES AS OF 17 July 2017

Coordination of cholera response Overall coordination of the cholera response at the national level is coordinated by the National cholera taskforce that meets weekly on Wednesdays from 2pm in the WHO conference hall in Juba. Similarly, the outbreak has continued to decline in Tonj East with transmission reported to have plummeted in Paweng and Makuach. However, a new hotspot has emerged in Marial lou, in Tonj South where suspect cholera cases have been reported. Low level transmission continues in Yirol East and Yirol West; in Uror (Pieri), and Nyirol (Lankien). In Juba, transmission in Don Bosco IDP camp is controlled following the first round of oral cholera vaccination from 3rd to 6th July 2017. Sporadic transmission continues from widespread areas including Gondokoro, Shirkat, Lologo, Gudele 2, Rock City, Kor William, and Nyakuron. Cholera response updates and field missions At least 50 cholera treatment facilities including cholera treatment centers and units; and oral rehydration points are currently operational in the areas with active cholera transmission. In Tonj East, there are currently seven (7) cholera treatment facilities (4-CTUs and 3-ORPs) in Tonj East. The four CTUs are located in Makuac, Mapara, Paweng and Paliang payams while the ORPs are in Ager bac, Aliet and Aper num. Four more ORPs are proposed to be sited at Marial Lou, Wunbol, Gol guk and Pagour. Three staff have been identified to work in the CTUs and WHO and the partner (CCM) on the ground planned to conduct case management refresher training for staff working in all the treatment centres. A total of 15 cholera treatment facilities have been set up in Kapoeta state, with Kapoeta South having 2 CTUs and 1 ORP; Kapoeta East having 3CTUs and 3 ORPs; and Kapoeta North having 1 CTC and 5 ORPs. Due to increased number of cases in Katiko, ARC decided to shift one ORP allocated to Kapoeta East to Katiko. The ORP was established on Saturday July 08, 2017. Additionally, 2 CHWs, 1 Cleaner and 1 Sprayer have also been recruited for Katiko ORP. A planned training on Cholera case management, infection control and prevention scheduled for Napotpot CTU staffs was postponed and pushed forward to next week because of coordination meetings at the State level. The MoH/WHO cholera rapid response teams continue to support and coordinate the cholera prevention and response activities in Kapoeta and Tonj states. In Juba, a joint MoH, WHO, MSF-CH, and HLSS team assessment last week has resulted in the functioning of the ORPs in Don Bosco, Gumbo, Kator, Lologo, Nyakuron, Al Sabah, and Gurei. Oral cholera vaccination As part of the ongoing cholera response, cholera vaccines have been deployed in Leer, Bor PoC, Malakal Town, Bentiu PoC, Mingkaman IDP settlement, Aburoc IDPs, Bentiu/Rubkona Town, Ayod (Pagil, Tar, Jiech, Karmun, Padek, and Kandak), and Juba (Don Bosco IDPs). During the week, the second round of OCV vaccination was completed in Bentiu and Rubkona Towns. Out of the 544 140 doses secured by WHO in 2017, a total of 261,495 doses have been deployed during the first round and 160,171 doses deployed during the second round in the eted locations as indicated in the table. There are no cholera cases reported from any of the sites where the oral cholera vaccines have been deployed in 2017. Similarly, WHO has secured additional 500,010 doses of OCV from the Global Taskforce for

Cholera Control. In view of this, a one week OCV campaign is scheduled on the 28th July to the 3rd August, 2017 for counties with high active transmission. These include Kapoeta North, Kapoeta South, Kapoeta East and Tonj East and the surrounding payams. A one-day OCV campaign training for MoH staff, Partners and other key stakeholders is scheduled to take place on the 18th July, 2017. An additional two million doses of oral cholera vaccines are required to mitigate the risk of cholera in high risk areas and to interrupt transmission in the areas with ongoing transmission. A total of 13,565 individuals (3536 men, 4689 women and 5340 children) have been sensitized on water treatment at household level, hand washing practices, safe water chain, proper disposal of human excreta and proper health seeking behavior for cholera. Mass campaign has been held in Nanaknak, Machi 1 and Machi 2;

identified water point were 375 stripes for 60 individuals for 1week.

- School hygiene promotion campaign conducted on June 30 at St. Paul Girls primary school in Mingkaman and 200 pupils were reached.

- Disinfected 12 public latrines with 65 stances in Mingkaman used by a total 1831 individuals (M 705, F 775 and Children-351).

- Attended cholera Task force and WASH cluster meetings in

Challenges - NFIs distribution does not meet

required sphere standard.

Recommendation

Check with core pipeline on sufficient supply of NFIs prepositioned in Awerial.

- Religious engagement: Social mobilizers had reached people with effective cholera messages in over 16 churches in during Sunday in the counties of Bor, Twic east, and Duk.

Akobo, Old Fangak, (Jonglei)

Nile Hope, FCDI - In Northern Jonglei counties of Fangak, Ayod and Pigi. Fangak community development Initiative (FCDI), UNICEF C4D supported through its social mobilization network had conducted engage community as part of cholera response. Below were achieved in the last two weeks;

- House to house visit: FCDI social mobilizers had so far reached total of 21,228 individuals (3,435HHs) with cholera keys messages. By educating them with simple five key steps of cholera prevention and control majors.

- School interventions session: The FCDI social mobilizers conducted awareness in Fangak, Ayod and pigi

Counties and it has reached 9 Schools and 3150 pupils with key hygiene behavioral messages on cholera diseases control, through dramas session at school.

Juba Cholera was confirmed in Juba by lab culture. Four cases were admitted to JTH CTC on 8th June, originating from Gumbo, Atabara, checkpoint and Rock city.

As part of mass media, UNICEF, MoH SMoH and partners continue to engage radio stations, Public Address systems and drama groups to inform and educate communities on cholera prevention and control practices in Juba and Yei.

- A total of 14 FM stations continue with airing of radio spots and talk-shows in Juba and Yei. As of 8-14 July, 2017, 840 times radio jingles have been broadcasted reaching over 2 million people across the country.

cholera messages has been ongoing

through Five (5) Public Address

Systems (PAs) in five main areas of

the City namely; Rajaf, Kator, Juba,

Munuki and Nyar-kenyi/Northern

Bari.The PAs conducts 5 hours

announcements per day (3 hr

morning and 2hrs evening) and will

be ongoing for a period of 60 days.
