PASSPORT AMENDMENT REQUIREMENTS (as of 01 January 2010)
Application for amendment of name in passport must be filed in person.
y Application Form for Amendment of Name (maiden to married surname) in passport;
y Other requirements:
For minors (below 18 years old)
y Personal appearance of either parent.
y if minor is not travelling with either parent, submit an original and photocopy of
DSWD Clearance and Aff idavit of Support and Consent indicating the name of the
travelling companion and relationship to the minor.
y If both parents are abroad, such an aff idavit must be authenticated by the nearest
Philippine Embassy or Consulate General.
y If both parents are abroad, person applying in behalf of the minor must submi t
Special Power of Attorney authenticated by the nearest Philippine Embassy or
Consulate General.
y Passport and photocopy of the passport of person travelling with the minor.
For adopted minors:
y Original and copy of Birth Certif icate with appropriate annotation by the Local Civil
Registrar printed on security paper by the National Statistics Off ice (NSO) and
authenticated by the Department of Foreign Affairs (DFA);
y Original and copy of Certif icate of Finality issued by the Court authenticated by the
DFA.
For Legitimated Children due to subsequent marriage of parents
y Authenticate Birth Certif icate with annotation of the amended surname of the child
and Marriage Conctract of parents
For women who got married in the Philippines and chose to adopt the surname of their
husband:
y Marriage Contract issued by the NSO in Manila duly authenticated by the
Authentication Off ice of the DFA, Manila;
y Photocopy of the data pages of Passports of both husb and and wife.
For women who were divorced by their foreign husbands:
y Original and photocopy of the dispositive portion of the court judgment granting the
divorce;
y Copy of the applicants birth certif icate issued by the NSO and authenticated by the
DFA;

y Photocopy of the data page of Philippine passport.
For women whose marriage was annulled by Court:
y Copy of the Court decision declaring the annulment of marriage duly authenticated
by the DFA;
y Original and copy of Certif icate of Finality issued by the Court authenticated by the
DFA.
y Copy of the marriage contract with the previous spouse with annotation of the
annulment printed on security paper by the NSO and duly authenticated by the DFA;
y Copy of the applicants birth certif icate issued by the NSO and authenticated by the
DFA;
y Photocopy of the data page of Philippine passport.
For widows:
y Death certif icate of deceased spouse. If husband died in the Philippines, the D eath
Certif icate should be authenticated by the NSO in Manila and the DFA;
y Copy of the applicants birth certif icate issued by the NSO and authenticated by the
DFA;
y Photocopy of the data page of Philippine passport of widow.
For Muslim applicants
y For those whose births were registered, follow requirements for new applicants.
y For those whose births were not registered.
o Certif ied True Copy of Late Registered Birth Certif icate from the National Statistics
Off ice.
o Original and photocopy of Voter's Aff idavit or NBI Clearance or other supportingdocuments indicating date and place of birth and citizenship.
o Certif icate from the Off ice on Muslim Affairs for Muslim convert
o For Muslim converts, submit annotated Birth Certif icate in Security Paper (SECPA)
o Court Order to change name.
o Shari'ah Court Order.
Additional supporting documents may be required when necessary.
