    LEGAL MANDATE

    Act No. 3753 or the Civil Registry Law has for its purpose, the establishment of a civil registrar

    in the Philippines wherein acts, events, legal instruments and court decrees concerning the civil

    status of persons shall be recorded.

    Administrative Order 1 for the Office of the Civil Registrar General Implementing guidelines of

    the rules and regulations of the said Act was promulgated for the information, guidance and

    compliance of Act No. 3753 for all concerned.

    2. Backside of COLB (Affidavit for Delayed Registration of Birth) must be accomplished and

    notarized or subscribed by authorized Officer;

    3. Certificate of No Record issued by Civil Registrar;

    4. Negative Certification issued by National Statistics Office;

    5. Any two of the following documentary evidences, showing Name of Child, Name ofMother and Name of Father, to wit:

    6. If illegitimate during birth of child, and the mother is not the applicant/informant attached;

    7. Affidavit of two disinterested persons attesting to the circumstances of the birth of the

    child

    REQUIREMENTS

    1. Accomplished Certificate of Marriage (COM) in four (4) copies, original signatures affixed;

    2. Certificate of No Record issued by Civil Registrar;

    3. Negative Certification issued by National Statistics Office;

    4. A copy of the Marriage Contract obtained by the couple from the marriage ceremony with

    License or Affidavit of Art. 34 1 original & 2 photocopies5. Affidavit executed by the Church/Solemnizing Officer OR by the couple attesting to the

    existence of the marriage ceremony and the reason for the delayed registration 1 original & 2

    photocopies;

    6. Payment/Fee:

    REQUIREMENTS

    1. Accomplished Certificate of Death (COD) in four (4) copies, original signatures affixed;

    2. Backside of COD (Affidavit for Delayed Registration of Death) must be accomplished and

    notarized or subscribed by authorizing officer;

    3. Certificate of No Record issued by Civil Registrar;

    4. Negative Certification issued by National Statistics Office;

    5. Death Certificate issued from the Church;

    6. Affidavit executed by the Hospital/Clinic Administrator OR attendant at death if deathoccurred in the said institution of under the care of the attendant; in default of the

    REQUIREMENTS

    a) Affidavit to Use the Surname of the Father (RA 9255)

    1. Three (3) Original Copies (with original signature) of the Affidavit to Use the Surname of

    the Father (AUSF), typewritten and duly signed by the father;

    2. Three (3) Original Copies of Affidavit of Consent of the Mother;

    3. Two (2) photocopies of birth certificate (Form 1A) or Certificate of Live Birth of the child;

    4. Any document showing clearly the paternity between the father and the child: (Strictly

    required when the name of the father does not appear in the birth record of the child), i.e.;

    1. Employment records (with benefeciaries)2. SSS/GSIS records

    3. Insurance

    4. Statement of Assets and Liabilities

    5. Individual Income Tax Return (ITR) of the father

    6. Others (showing relationship between father and child)

    1. Three (3) Original Copies of Affidavit of Acknowledgement/Recognition duly signed by thefather (Strictly required when the father does not appear in the birth record of the child);

    2. Three (3) copies of Affidavit of Consent of the Child, if the child is 18 years old and above;

    3. Show valid IDs of both parents (i.e. SSS/GSIS, Voters ID, Postal ID, Drivers License,

    Passport, etc.)

    b) Affidavit of Recognition (for birth records of those born before August 3, 1988)

    1. Three (3) original copies of Affidavit of Acknowledgement/Recognition duly signed by the

    father (must be typewritten/computer printout);

    2. One (1) original and two (2) photocopies of Birth Certificate (Form 1A) or Certificate of

    Live Birth;

    3. Any document showing clearly the paternity between the father and the child, i.e.;

    1. Employment records (with benefeciaries)

    2. SSS/GSIS records

    3. Insurance4. Statement of Assets and Liabilities

    1. Show valid ID of the parents (i.e. SSS/GSIS ID, Voters ID, Postal ID, Drivers License,

    Passport, etc.)

    c) Affidavit of Legitimation by Subsequent Marriage

    1. Three (3) original copies of Affidavit of Legitimation duly signed by the parents (must betypewritten/computer printout);

    2. Three (3) original copies of Affidavit of Recognition by the father (if the fathers name does

    not appear in the original birth certificate);

    3. One (1) original and two (2) photocopies of the Marriage Contract of the parents;

    4. Certificate of No Marriage CENOMAR, from the National Statistics Office of both parents;

    5. Any document showing clearly the paternity between the father and the child, i.e.;

    1. Employment records (with benefeciaries)

    2. SSS/GSIS records

    3. Insurance4. Statement of Assets and Liabilities

    5. Individual Income Tax Return (ITR) of the father

    1. Show valid ID of the father (i.e. SSS/GSIS ID, Voters ID, Postal ID, Drivers License,

    Passport, etc.)

    d) Supplemental Report of Birth

    1. Any document showing clearly the paternity between the father and the child, i.e.;

    1. Name of Document Owner

    2. Date of Birth, Marriage or Death

    3. Registry Number

    4. Entry/entries omitted during the time of registration5. Reason for omission of the entry/ies during the event
