FOREWORD

Federal support for categorical construction programs is a rela­tively new principle. In fact, prior to 1963, no Federal legislationexisted to support ~onstruction of facilities designed specificallyfor the mentally retarded.

Since the enactment in 1963 of the Mental Retardation Facilitiesand Mental Health Centers Construction Act (P.L. 88-164), three differentbut interrelated construction programs for the retarded have been ini­tiated: Research Centers, University-Affiliated Facilities and CommunityFacilities. As of January 1971, a total of 12 Research Centers, 20University-Affiliated Facilities, and 362 Community Facilities had beenapproved and funded. On October 30, 1970, President Nixon signed intolaw the Developmental Disabilities Services and Facilities ConstructionAct (P.L. 91-517). This new law extends the construction authority formental retardation university-affiliated facilities and communityfacilities for the mentally retarded.

The three construction programs discussed in this booklet weredesigned to provide assistance in vital ar~as of concern: continuingresearch into the causes and means of prevention of mental retardation;inter-disciplinary training of professional personnel for researchand service careers in both present and newly emerging programs; andestablishment of a network of facilities where the retarded can obtainservices in their own communities. This publication reviews the currentstatus of these three construction programs for the mentally retarded.

INTRODUCTION

The ~eport of the President's Panel on Mental Retardation in 1962 resultedin a breakthrough of real significance for mental retardation program4.This Report was a culmination of the efforts of a group of distinguishedAmericans who studied the problem of mental retardation and reported ontheir findings. The Report outlined a number of areas for action, and pro­vided a blueprint for planning and implementive programs of comprehensiveservices to the retarded. The Report in large measure was responsible forsubsequent enactment of a number of Federal laws affecting the retarded,including authority for new construction programs.

Subsequent to issuance of the Report, a special message on mental retarda­tion and mental health was sent to Congress in 1963. The message outlinedareas of concern and suggested possible approaches. Three areas givenspecial attention were research and prevention, manpower and communitybased services. In the same year Congress enacted the first Federalcategorical construction programs for the mentally retarded: liThe MentalRetardation Facilities and Mental Health Centers Construction Act of 1963"(P.L.88-164).

The Developmental Disabilities Services and Facilities Construction Actof 1970 (P.L. 91-517) amended the Mental Retardation Facilities and MentalHealth Centers Construction Act of 1963, to provide the states with broadresponsibility for planning and implementing the comprehensive programservices and to aid the states in the construction of community facilitiesfor the mentally retarded with a combined formula and project grantprogram covering both construction of facilities and the provision ofservices to persons with developmental disabilities. The scope isbroadened to include not only the mentally retarded. but also personssuffering from other serious developmental disabilities originating inchildhood, including cerebral palsy and other neurological handicappingconditions. In the Act of 1963 (Title I, Part A, P.L. 88~164), projectgrants were authorized for the construction of public or non-profitcenters for research that would develop new knowledge for preventing andcombating mental retardation. The new act (P.L. 91~5l7) does not continuethe authorization for construction of these research centers.

Title I, Part B, of the Act of 1963, authorized project grants to assistin the construction of public or nonprofit clinical facilities for thementally retarded, associated with a college or university, which would:(1) provide, as nearly as practicable, a full range of inpatient and out­patient services; (2) aid in demonstrating provision of specializedservices for diagnosis, treatment, training, or care; and (3) aid in theclinical training of physicians and other specialized personnel neededfor research, diagnosis, treatment, training, or care. The new legia­lation (Title II, P.L. 91~5l7) extends the present authority to constructuniversity-affiliated facilities for the mentally retarded through June30~ 1973, and authorizes a new project grant program to cover the costs

of administering and operating demonstration facilities and inter­disciplinary training programs in such facilities.

Title I, Part C, of the Act of 1963 authorized Federal grants to statesto assist in the construction of 5pecia11y designed public and nonprofitcommunity facilities to provide diagnosis, treatment, education,training, care, and sheltered workshops for the retarded. Title I ofP.L. 91-517 replaces this authority to aid in the construction of com­munity facilities by authorizing formula grants to states for planning,administration, services, and construction of facilities for thedevelopmentally disabled. In other words. the previous authority toconstruct community mental retardation facilities is replaced by abroad new federal~state grant-in-aid program to assist the states indeveloping and implementing a comprehensive plan for meeting the needsof persops with developmental disabilities. States may use these fundsto construct facilities, provide services, support state and localplanning, provide administrative and technical assistance, trainspecialized personnel. and develop and demonstrate new servicetechniques.

MENTAL RETARDATION RESEARCH CENTERS

Research and research training in mental retardation and related aspectsof human development present unique demands in terms of facilities andresources. Recognition of these needs led. in 1963, to legislationauthorizing construction grants for facilities in which biological.medical. social, and behavioral research relating to human developmentcould be conducted to assist in finding the causes and means of preven­tion of mental retardation, and for finding means of ameliorating itseffects.

Acceptance of applications for construction grant awards under the Actauthorizing the Mental Retardation Research Centers closed on July 1.1967. During the authorized four years of the Mental Retardation ResearchCenter Construction Grant Award Program, a total of twelve centers wereawarded to outstanding scientific institutions. During its constructionphase this program was jointly administered by the Division of ResearchFacilitie~ and Resources and The National Institute of child Health andHuman Development (NICHD). continuing responsibility for the researchprogram in the centers is carried on by NICHD.

The facilities constructed through the Mental Retardation ResearchCenter Program will provide research facilities in particular environmentswhere a cohesive program of research and research training can beaccomplished. Most of the centers are large complex facilities insettings where a broad spectrum of research on mental retardation canbest be carried out. A small number of specialized centers which havea concentrated scientific focus on a particular aspect of mental re­tardation are also supported by the program.

The Child Development and Mental Retardation Center at the University ofWashington encompasses both a research center and a university-affiliatedfacility (see page 53). The Center has been designed to bring together themany scientific and professional disciplines concerned with the understand­ing and management of mental retardation and other handicapping conditionsin children.

The Center consists of four units. In the Medical Research Unit, there aremajor research programs in developmental biology, perinatal biology andneurological sciences. In developmental biology, research is in progressin the areas of genetics, embryology, developmental biochemistry andimmunology, teratology and dysmorphology. In general, this research isdirected toward understanding and control of factors responsible fordevelopmental abnormalities. The general objective of the perinatal biologyprogram is greater understanding of the factors operating in utero andduring the process of birth to produce abnormalities in 'th;-neonate. Thisprogram includes investigators from the Departments of Anesthesiology,Obstetrics and Gynecology, Pediatrics, Pharma~ology, Radiology and Surgery.In the neurological sciences program, research is conducted on centralnervous system development, convulsive disorders, neuropathology - parti­cularly in prematurity - and on the visual system.

The Behavioral Research Unit also encompasses three major programs. Thepsychology program has been concerned with research on learning and perceptualdevelopment~ attention and memory processes in retardates. The speech andaudiology group has been engaged in research on the early stages of languagedevelopment~ imitative responses, the role of modality and habituation onresponses to sensory stimulation and the effects of drugs on EEG audiometry.The psychiatric program has involved basic studies of social behavior inprimates, studies of mother-child interaction and studies of the effects ofearly stimulation on infant development.

The Experimental Education Unit is engaged in research related to thedevelopment and evaluation of educational materials and classroom proceduresfor use with handicapped children. Special emphasis has been placed on thedevelopment of reading, arithmetic aud vocational training materials and onmethods for development of social behavior.

The Clinical Training Unit conducts clinicalimproved methods of diagnosis and treatment.

CHILDREN'S HOSPITAL, CINCINNATI, OHIO

The Mental Retardation Research Center of Cincinnati (Institute for Develop­mental Research) is a part of the Children's Hospital Research Foundation andthe Department of Pediatrics of the University of Cincinnati, College ofMedicine. This research center is focusing, at the present time, on bio­medical research.

Areas of study include congenital malformations with special emphasis ondevelopmental defects of the central nervous system. Genetic as well asenvironmental causes are under investigation and mechanisms leading to con­genital anomalies are being studied. Many animal models are available forstudy of the effects of environmental hazards upon the development of embryosand fetuses. A number of investigations deal with tests and methods formeasurement of metabolites which are involved in brain chemistry or errorsof metabolism. Electroanalytical methods for studying brain chemistry havebeen developed and are being applied to investigations of the physiology ofthe normal and the damaged brain. A Tissue Culture Laboratory is availablefor chromosome analysis, biochemical studies and electronmicroscopic in­vestigations of tissues from children with mental retardation. An ensymelaboratory dealing with various lysosomal diseases is closely associatedwith the tissue culture facilities. Head injury. a frequent cause of mentalretardation of postnatal and perinatal origin, is analyzed by studies ofcerebral blood flow and its relation to intracranial pressure, glucosemetabolism and oxygen utilization by the acutely injured brain. Althoughthere are no residential facilities for children in this center, the

WALTER E. FERNALD STATE SCHOOL, WALTHAM, MASSACHUSETTS

This Center is located on the grounds of an institution for the retardedand will pla~e heavy emphasis on the retarded person as the object ofstudy. Interchange with investigators in other fields will be encouragedand research will focus on prevention and amelioration. Among the dis­ciplines included in this research program are neurology, psychiatry,pediatrics, epidemiology, experimental psychology, cytogenetics and educa­tion. An unusual feature of this project is the close cooperation betweenthe Fernald School, Massachusetts General Hospital, and the Harvard MedicalSchool.

GEORGE PEABODY COLLEGE, NASHVILlE, TENNESSEE

This private college of education has a long and productive history of re­search and training in the field of mental retardation and related aspectsof human development with a special emphasis on the behavioral sciences andeducation. The College rests within a large university center (also includ­ing Vanderbilt University, Meharry Medical College, Fisk University, andScarritt College) which provides the rich variety of university resources re­quired for a major research center.

The John F. Kennedy Center for Research on Education and Human Developmentat Peabody College makes possible a program of interdisciplinary researchand training including strong cooperative research programs with VanderbiltUniversity Medical Center, Meharry Medical College, Fisk University and theMetropolitan Nashville Public Schools.

The research and training program at the Kennedy Center is directed at basicand applied research in the behavioral sciences aspects of the problem ofmental retardation and developmental disability. Programs of research,evaluation and demonstration are currently ongoing with mentally retardedchildren, children with severe behavior disorders, culturally disadvantagedand environmentally deprived children, physically handicapped and sensoryhandicapped children and special studies of communication, cognitive develop­ment, early educational intervention programs and psychobiology studies.The research program is closely linked to the training activities of theDivisions of Human Development and Education.

The Center acquired two new facilities Which were dedicated in March, 1968,and now house the research and training activities of 72 doctoral level researchfaculty from a variety of disciplines and institutions of higher educationin the Nashville area.

The UCLA Mental Retardation Center houses a Mental Retardation ResearchCenter and a University Affiliated Facility under joint administration. Itis the only such facility in the country within a university department ofpsychiatry. Housed above the Neuropsychiatric Institute, the Center alsoincludes extensive clinical research in the Community Liaison and Evaluationof Programs section, the Inpatient Service, the NPI School and the OutpatientService. A high degree of researcher-clinician interaction is encouraged andis further aided by a pre- and post-doctoral training program designed toutilize· the Center's interdisciplinary setting to its maximum advantage.

The laboratory research program now includes scientists from such diversefields as Developmental Neurobiochemistry, Developmental Neurobiology,Neurophysiology and Socia-Behavioral Studies who work with clinicians togain new knowledge about mental retardation. Some of the research investi­gations include studies on: PKU and other metabolic disorders, effects ofcertain hormones on the functional maturation of brain cells, protein syn­thesis in the neonatal brain, the effects of nutrition on brain development,the electrophysiologica! bases of learning and motor activity, actention inman and "animal, anthropological and etiological aspects of mental retarda­tion, and the adaptive behavior concept.

The Center enjoys clo5e research collaboration with pacific State H05pitalin Pomona, California. The two facilities share personnel, support,research interests, and data on problems of both mentally retarded childrenand those with other kinds of impaired competence. Cooper~tive projectsinclude studies on: epidemiology, program evaluation, adaptive behavior,chromosomes and genetics, data processing, electrophysiology and neuro­biology. A newly developed information storage and system will enhancethe Center's research, administrative and clinical capabilities.

CHILDRENrS HOSPITAL MEDICAL CENTER, BOSTON, MASSACHUSETTS

This Center is closely affiliated with the Harvard Medical School and ~roposes

an interdisciplinary research program directed toward the understanding. pre­vention. and amelioration of the handicapped and the mentally retarded. Thereare research progr~ms in the interrelated biomedical areas of: (1) Neuro­science, (2) Genetics and systemic metabolism, and (3) Behavioral Science.Within each field. there are research projects at the clinical level supportedby more basic projects of investigation. In addition. there are a number ofspecific instances of collaboration among investigators of different programareas.

Activities of the Neuroscience program involve investigation of the problemsof cerebral fluids and solute transport, as well as studies of human hydro­cephalus, electroneurophysiologic studies using computer averaged evokedpotentials in humans supported by single unit microelectrode studies inanimals. There are biochemical studies of brain enzymology and transmitterfunction. and a number of interdisciplinary projects centering about braindamaging conditions of the paranatal period.

The principal effort of the Genetics program consists of a continuing studyof the assignment of genes to human chromosome and studies of the nature ofregulatory events in mammalian cells, utilizing the techniques of cytogenetics,tissue culture. hybridization, etc. The metabolic laboratories 'concentrateon clinical problems relating to inborn defects of amino acid, lipid andmucopolysaccaride metabolism.

The primary emphasis of the behavioral science program is on human studies ofneonatal rhythmic motor function and dysfunction as a possible predictor ofbrain damage and intellectual disorder, recognition of the important issueof early detection as a first step in management of the retardate. Anadditional focus of interest directs attention to minor degrees of intel­lectual dysfunction with regard to juvenile delinquent behavior, and inves~

tigation of male-female differences in cognitive style and reaction to braindysfunction. The behavioral science program also includes studies of animalperformance correlated with brain catecholamines in the experimental labora­tory.

The Mental Retardation and Human Development Program represents the largestsingle commitment of resources in the institution's history to research ina single field.

UNIVERSITY OF KANSAS, LAWRENCE, KANSAS

The University of Kansas has a unique plan for the location of three researchand research training units - at the University of Kansas Medical Center, themain campus of the University at Lawrence. and the Parsons State Hospital andTraining Center, an institution for the retarded.

The long-range multidisciplinary research program is broadly conceived andfocuses on biomedical and behavioral research relevant to mental retardation.Each of the three settings emphasizes specific areas of research. Studies atthe Medical Center include reproductive physiology, biochemistry, neurophysi­ology and fetal and neonatal pathophysiology as well as studies on learning,language, and social behavior. At the Lawrence campus, research features pre­school studies of behavior, and the processes of socialization, communication,and learning. The program at Parsons stresses research in training of childrenwith deficits in language. socialization and adaptive behavior, in academicattainment and vocational skills.

The design of the research center is based upon several basic issues. Thefirst is that mental retardation is a dynamic. developmental disorder andlends itself to intervention of a preventative, ameliorative and/or educa­tional nature starting in infancy and extending throughout the life cycle ofthe individual. The second is that the problem is both biological and behav­ioral and calls for a number of research approaches that combine personnelfrom the several relevant fields. The third is that the problem is broad andcomplex and calls for a sustained, coordinated effort by investigators whostudy tha retardate in both controlled and naturalistic environments. Thefourth is that there are a large number of professionals and non-professionalswho need the products of research centers and who require that the productsare given to them in a form which they can understand and use. The fifth isthat, since the number of researchers and settings undertaking significantresearch with the retarded are relatively few in number, the center shouldundertake to train additional research specialists to plan and executeresearch in other settings. Research at the Kansas Genter thus approaches the

problem of mental retardation from conception to the optimum development ofadult skills. The rationale is thus to touch on the whole continuum -- todesign, develop, demonstrate and disseminate data stemming from research inthe several thematic areas noted.

The center in Colorado is the smallest of the functioning research groupscreated under P.L. 88-164. It Is also unique in that it operates as a divisionof the Department of Pediatrics- rather than-as an autonomous academic unit.This arrangement has had two cons~quences: in the first place, it has encour­aged links with other departments in the school, notably with Biochemistry andthe divisions of neurology and endocrinology in Medicine; in the second, it hasled to a series of program8 Which, whilst deploying fundamental technology,are all very directly concerned with problems of development in children andespecially with preventing mental retardation.

The program is largely biomedical in focus, with groups working in tracemetal nutrition, inborn errors of metabolism, the gene defect in diabetes andthe biochemical growth of the mammalian brain. The mass spectroscopy labora­tory is primarily concerned with extending our diagnostic capacity in metabolic'disease. The only embryology laboratory in the Medical Center is in theStolinsky Laboratories. A cytogenetics unit utilizes automated technicswith the capacity for relatively extensive surveys and more detailed morpho­logical karyotype analyses.

There is a program in developmental pediatrics and a substantial PKU clinic.The objectives of this group are very practical - namely, to developincreasingly effective technics which can be applied by non-professionalpersonnel, to screen for developmental retardation in pre-school children.It is appropriate that there is a strong educational research program in theassociated care center orientated to this age group.

This National Mental Retardation Research Center has two major units:Biological Sciences Research Center and the Frank Porter Graham Childment Center.

The Biological Sciences Research Center addresses itself to the problems ofdevelopment and maturation of the central nervous system and to the manifoldcauses of biological mental retardation. Its research programs include repro­ductive physiology, experimental embryology, developmental neurochemistry,neurophysiology, and neuroendocrinology. Applied laboratory research on theapplication of analytical, chemical techniques to the detection of metabolicdisorders of ~netic origin is also in progress.

Included in the Biological Sciences Research Center is a clinical diagnosticand training program (Division for Disorders of Development and Learning)which sees some 150 patients each year. A pilot day nursery program forthese patients has been established.

The Division of Child Psychiatry operates a research inpatient unit foremotionally disturbed children. Research in this unit centers aroundbehavior modification and the use of parents as co-therapists.

Investigators at the Frank Porter Graham Child Development Research Centerare engaged in a series of programs focusing on the study and stimulation ofdevelopmental processes of the young child. particularly the child from

limited resource environments. Current investigations involve a varietyof disciplines in studies which include the investigation of environmentalstimulation on the development of the child, the design of curriculummaterial to stimulate thinking processes. and the study of the transmissionof infectious diseases in young children. A population of 40 children cur­rently are seen daily in day care and preschool settings that provide thebase for such studies.

Additional planning is being carried out to execute similar multidisciplinarywork, long range research programs in the stimulation of young infants, theestablishment of a progr~ and materials development and evaluation unit forearly childhood programs, the study of the development of cognitive stylesand sets in preschool children and the impact of individualization ofinstruction on primary school age children.

The Joseph P. Kennedy, Jr., Mental Retardation Research Center is located inthe Silvain and Arma Wyler Children's Hospital at the University of Chicago.The-Center has been designed to place in a University medical school a balancedprogram to provide care. training and research in an integrated fashion. It isparticularly hoped that all of the activities of the Center will be correlatedwith other programs within the medical scbool so that maximum advantage mightaccrue from the setting in a major medical school, medical center, and researchcenter. A part of the research center is contiguous with the Chicago Lying-InHospital which provides a resource for studies of prematurity and obstetricalconditions leading to retarded development. The Mental Development Clinicalso serves as a" source of patient material for research activities of thevarious basic science groups. This greatly facilitates our goal of achievinga close correlation of basic science and clinical medicine.

The components of the program are: Basic science research in biochemistry.genetics, cell differentiation, neuroendocrinology, neurochemistry, virology,and the operation of the Mental Development Clinic. The goal of these programs

with respect to the practical andsocial significance of mental retarda­tion may be outlined as follows: 1)bring to the care and prevention ofmental retardation as rapidly as pos­sible the fruits of medical and bio­logical research, 2) develop basicknowledge in" the area of growth anddevelopment so that a more basic under­standing can be brought to bear on thefundamental problems of mental retarda­tion, 3) where possible, to moredirectly apply techniques of biochem­istry and cell biology to elucidationof the etiology and treatment ofmental retardation, 4) afford modelcare for a limited population of men­tally retarded children, 5) providetraining in the care of patients withmental retardation at different levels,6) assure exposure of a maximum numberof medical students and pediatriciansto tbe problems of mental development.7) train a limited number of physiciansas specialists in the care of the re­tarded, 8) p~ovide for the interchangeof information and interests betweenpersonnel working directly with patientsand basic scientists in order to bringa maximum of intellectual effort onthe problems of mental development.

The University of Wisconsin has under construction a combined research anduniversity-a££iliat~dcenter containing laboratories and facilities for bio­medical and behavioral research, experimental classrooms and sheltered workarea. The center 1s committed to the philosophy of interdisciplinary research,where needed and feasible.

The Biomedical Unit has three differentiated but coordinated programs. Neuro­physiology is investigating the problems of cerebral localization in sensoryand motor systems, the interrelations of different systems and the contribu­tion of each level of the brain to neural functious and behavior. In neuro­endocrinology, emphasis is directed toward the role of the central nervoussystem in the regulation of hormonal factors in reproduction. The effects ofinborn errors of metabolism on intellectual functioning are well establishedand an ongoing part of the research program.

The Behavioral Unit is composed of sections concerned with infant developmentand the behavioral, social, communication and treatment processes. Studieswill focus on early detection of mild retardation and the acquisition ofadaptive capabilities; cognitive and personality development; learning,reinforcement, attention and environmental influences; speech reception andreproduction, language acquisition; social processes and organizations; coun­seling, education and rehabilitation processes.

The Rose F. Kennedy Center is an integral, interdepartmental unit of theAlbert Einstein College of Medicine of Yeshiva University, It occupies anewly constructed 10 story research building in which are conducted multi­disciplinary research, research-training and clinical research services witha broad approach to mental retardation and other aspects of development dis­ability. Patient facilities include an inpatient clinical research unit, agenetic counselling clinic, a Children's Evaluation and Rehabilitation Clinic,and an outpatient clinic for follow up of patients being studied by investi­gators in the Center.

The staff of senior investigators from the faculty of the College of Medicineand from other professional schools of Yeshiva University has been drawn fromthe behavioral sciences, the biomedical sciences and the biosocial sciences.

Important research highlights include:

- Work of a pioneering nature in the study of int~rneuronal communicationmechanisms in developing cell systems and of abnormal processes that inter­fere with the establishment or formation of direct cell to cell communication.

- Development of testing procedures for the early identification of deaf·ness through behavioral and neuropsychologic techniques.

UNIVERSITY·AFFILIATED FACILITIES CONSTRUCTION PROGRAMFOR THE MENIALLY RETARDED

The program of Federal assistance for the construction of university­affiliated facilities for the mentally retarded was authorized underP.L. 88-164, tiThe Mental Retardation Act of 1963," and extended until1973 under the Developmental Disabilities Services and FacilitiesConstruction Act of 1970 (P.L. 91-517). This program includes provisionof grants for the construction of university-affiliated facilities withprograms for the developmentally disabled. Twenty university-affiliatedfacilities have been approved for funding under the authority of P.L.88-164. Nineteen of these received funds from Title I, Part B of thatAct and one from Title I, Part A.

The primary purpose of the University-Affiliated Facilities Constructionprogram is to provide faciliti~. for the clinical training of professionaland technical personnel essential for diagnostic services and the care,education, training, and rehabilitation of the mentally retardedindividual and his family. Each facility is encouraged to conduct acomprehensiv~ interdisciplinary training program integrating andcoordinating the full range of professional and technical personnelconcerned with mental retardation so that each discipline may be fullyfamiliar with its own contributions and those of related disciplines tothe total effort in the field of mental retardation. A full range ofindividual and group services and demonstration of new techniques andconcepts in services for the mentally retarded are considered importantelements in university-affilia~edfacility programs, and researchincidental or related to activities conducted within the facility isauthorized.

This construction program is administered by the Division of MentalRetardation. Rehabilitation Services Administration, Social andRehabilitation Service. U. S. Department of Health, Education, andWelfare. The following descriptions review the types of facilitiesand programs which have been supported with these funds and the currentstatus of their construction. The partlow State Hospital, located adjacent to the Tuscaloosa campus, withan inpatient population of 2 J OOO, will utilize to maximum advantage thebehavioral sciences faculty at the ~in University. Interdisciplinary train­ing utilizing joint appointments in the University and the facility and thedual relationship with the University and Partlow School, will result in theapplication of knowledge of recent advances in research, training and servicefor the mentally retarded.

Training programs are coordinated through an advisory council consisting ofthe Deans of Medicine, Dentistry. Arts and Sciences, Education and theGraduate School. An interdisciplinary advisory committee consisting of the

The Mental Retardation University-Affiliated Facility of the University ofCalifornia Neuropsychiatric Institute is housed in a new four-story additionto the existing UCLA Medical School Complex.

The Mental Retardation Unit provides three 20-bed wards, one for youngseverely retarded children, one for ambulatory retarded children, and one forolder children with varying degrees of retardation. The fourth floor housesthe school facilities.

All children coming to the UCLA Medical Center, both inpatients and out­patients, are screened in the general pediatric clinic, and mentally retardedpatients· are referred to the Neuropsychiatric Institute fbr extensivediagnostic studies and evaluation. The outpatient department functions asa part of the total program at the present time. The major effort in mentalretardation has~ been based at the pacific State Hospital at pomona, and thecenter will provide a facility to bring widely distributed services andtraining sites together.

The Center is adjacent to an existing day care center located on the maincampus of the Medical School. It is a three story building providing spacefor pediatricians, nurses, clinical psychologists, social workers, nurses,nutritionists, dentists, audiologists and speech therapy personnel in train­ing. A special feature of the Center is a large indoor play area Bnd schoolroom where children may be observed.

Students from the Schools of Social Work and Psychology of the DenverUniversity will be assigned to the center. Students from the Departmentsof Physical Medicine and Special Education of Colorado State Universitywill also be provided with training opportunities in an interdisciplinaryteaching environment.

The importance of the Center in the overall State effort is exemplified bythe Center's designation as a Regional Center in Mental Retardation. Train­ing programs are coordinated through the Director of the Center who has aMedical School appointment. Interdisciplinary research and clinical effortsin the Stolinsky Laboratories associated with the Center are -being conductedto discover the causes, prevention and treatment of developmental deviationsand their attendant emotional disorders.

The University-Affiliated Center~ which is an integral part of the Medical'School complex, will be directed toward meeting the needs of the trainingeffort related to: (a) more complete diagnostic evatuations, (b) longitudinalmanagement and rehabilitation of the mentally retarded, (c) training of medicalstudents, physicians and nurses. The social behavioral components in traininginvolve other local universities, e.g., psychology students cQme from CatholicUniversity, special education students from George Washington University, andsocial work students from Howard University under a consortium agreement toassure interdisciplinary training in which training content is determinedby the affiliated universities. A small preschool nursery is staffed by theSt. Johns Developmental School, a community facility. The District ofColumbia. Virginia. and Maryland.Departments of Vocational Rehabilitationwill provide patient prevocation evaluations and will be closely allied withthe Center.

Emphasis in the intramural phase of the program will be on early diagnosisand comprehensive programming for the preschool child. Diagnostic classroomswill be an essential resource for the program, with a special interest inindividuals having both auditory and visual handicaps. Training in inpatientcare will be provided in a 29-bed unit.

The Center will offer training in the fields of social work. special educa­tion. sociology, theology, and law, in addition to the biomedical professions.in an interdisciplinary approach to the problems of mental retardation. Anextramural program directed at existing community resources of high qualitywill allow training programs and services to be integrated into the compre­hensive needs of the mentally retarded in the metropolitan Washington area.

The Mailman Child Development Center will be located on the campus of theSchool of Medicine of the University of Miami. within the immediate vicinityof the Jackson Memorial Hospital and the National Children's cardiac Hospital,which are teaching hospitals for the Medical School.

The Center provides an opportunity to make training in mental retardation apart of the learning experience of all medical, nursins/ psychology, socialwork, special education. speech and law students. Students from the Univer­sity of Miami. Florida State University, Barry College and the University ofFlorida will participate in the training programs. The programs will bedevoted to seeking the causes and means of prevention as well as the methodsof ameliorating psycho-social and medical effects of mental retardation andother handicapping conditions.

Training programs are under the supervision of a Director appointed by thePresident of the Main University. The Director serves as chairman of theinterdisciplinary training committee whose members are selected by the Deanof the appropriate schools or colleges in the University.

Patients are admitted by referral from community. and social agencies as wellas private physicians. They will range in age from infancy to the olderadults. and all levels of retardation w~ll be seen and evaluated. The bulkwill come from Dade and Broward Counties; however. there will be nogeographical restrictions on admission.

Atlanta

The University Affiliated Center for the mentally retarded will be a part ofa large state residential Mental Retardation Center in Atlanta. This compre­hensive residential facility will serve approximately 1,000 resident retardates,and provide day care services for an additional 500 retardates. The followingcolleges and universities will be affiliated in the training programs of theAtlanta facility in the disciplines as indicated: l~ Atlanta University ­special education and social work; 2. Interdenominational Theological Center- religious therapy; 3. University of Georgia - speech and hearing, recrea­tion for the ,handicapped, special education, child development, guidance andcounseling, vocational rehabilitation, psychology, and sociology; and4. Emory University - pediatrics, psychiatry, physical medicine, physicaltherapy, occupational therapy, special education, nursing, psychology,sociology, religious therapy, and speech and hearing. All of the affiliatedtraining programs ~ill be at the graduate level, and the facilities will bemade available insofar as practicable for undergraduate instruction.

Training programs are conducted through an Advisory Board for Training chairedby the Assistant to the Superintendent for the Georgia Retardation Center withmembers representing each university or college affiliated with the Center.

The Athens Facility at the University of Georgia will accommodate fortyshort term residents and forty retardates in the day care program. Thefacility will serve as a laboratory to teach diagnostic and therapeuticmeasures for speech pathologists in language, speech, and hearing problemsof the mentally retarded. The program will be administeredby two working committees and a steering committee under the direction ofthe Vice President for Research and Advanced Study at Indiana University.These working committees are the Professional Advisory Committee. RileyChild Development Center, and the Professional Advisory Committee.

Riley Hospital Child Development Facility, Indianapolis

This Facility will become a part of the Riley Hospital for Children, whichis the only hospital in the state devoted entirely to the care of children.It has served as a referral Center for all difficult cases of pediatricdiagnosis and treatment, and over 50% of the children are referred fromthroughout the state to the hospital. At least 40% of the children seenin the outpatient cl~nic are diagnosed as mentally retarded. Family inter­actions that are critical to mentally retarded youngsters have long beenexplored.

The physical plan of the Facility takes into account the latest trends inpatient care, particularly ambulatory services. The consultative diagnosticand treatment clinic serves as a comprehensive consultation service tophysicians within the state and also serves as a major educational activityat the Medical School. Pediatric residents are assigned to the Facilityfor three-month periods, and senior medical students are assigned to theclinic as part of a six-week pediatric training program. Students fromthe School of Nursing and the allied health professions, Le·., social work,psychology, audiology, occupational therapy, and speech therapy, attendsessions within the Facility.

As each patient is diagnosed and evaluated, an effort is made to developcontinuity care and the designation made of specialty clinics which willbe beneficial to the retardate after his return to the referring physicianor his home.

Indiana University Developmental Training Facility. Bloomington

This Facility is located adjacent to the laboratory school.

The Bloomington Facility will house 48 children in eight "home units"arranged as "row houses." Each unit will be autonomous in that it willhave the normal features found in a home and an attached apartment forcotta~ parents who will provide a home atmosphere and individuality foreach unit.

These Facilities will be adjacent oradditive to the three buildings comprising the Kansas Center for Researchin Mental Retardation and Human Development currently under construction.The multiple locations of the Kansas combined Center facilities make possiblethe utilization of varied research and clinical training resources. Thethree locations allow for the exploitation of resources of: (1) the academiccommunity with its faculty and student populationi (2) a modern medicalcenter located in the heart of a metropolitan areai (3) a progressiveresidential construction project. Construction funds for the Kansas Centerfor Research in Mental Retardation came from Title I. Part A, P.L. 88-164.The combined Center in Mental Retardation will be coordinated by andadministratively responsible to the Bureau of Child Research of the Univer­sity of Kansas. Since 1957, the Pars~ns State Hospital, the Kansas Univer­sity Medical Center. and the Bureau of child Research have been engaged inbehavioral and biological research and interdisciplinary training on acooperative basis. The new construction will make possible the expansionand long-term projection of these activities.

The Medical Center serves the State of Kansas and adjoining areas as amajor referral center for individuals with all types of behavioral anddiagnostic problems. The University-Affiliated Facility will provideservice and training programs on each campus to improve comprehensiveevaluation for patients. In addition both patients and their familieswill have access to a comprehensive longitudinal care program centrallydirected.

Training programs are conducted in an interdisciplinary environment inpediatrics, special education. psychiatry. nursing, speech and hearing,psychology, dietetics and nutrition. and social service.

The Lawrence Facility will draw upon all of the resources of the Universityand will include a large number of" trainees which will serve as a primaryresource for recruitment and for pre-professional training. The staff ofthe Lawrence Facility will share responsibilities at the other two settingsin the training of disciplines mentioned above, and an emphasis ondevelopmental factors will be stressed in the following areas; parentcounselling) behavioral training, the importance of community and environ­ment) social and educational skills, and learning processes.

Parsons State Hospital and Training School

The Facility at the Parsons State Hospital and Training School will con­slst of a medical evaluation tfeatment section, a behavioral and evaluationsection, and a section for audio-visual production. The plan for trainingclinical specialists at both Parsons and Kansas City will enable thetraining coordinators to draw upon resources of both the Research Centerand the University-Affiliated Facility.

The program within the Facility stresses,the importance of the habilitationof the mentally and physically handicapped child. Special emphasis isplaced on the treatment of both the child and family by flexible use ofinpatient and outpatient services with the primary aim of returning thepatient to the community as a functioning member of society. The Institutemakes full use of the multidisciplinary clinic approach by providing a fullrange of diagnostic, evaluation and therapeutic services. Such serviceshelp develop a better understanding of the problem and make possible betterlong-term management of the child by the family •

The Children's Hospital Center is the principal pediatric teaching hospital ofHarvard University and a good source for securing referrals of patients who arementally retarded. The Center serves as a teaching facility in social service,psychology, physical therapy. audiology. and special education for traineesassigned from Simmons College. Northeastern University. Wheelock College andBoston University. In 1929 the Children's Hospital Center created the firstinpatient unit in the country where retarded children could be studied andevaluated by a team composed of pediatricians. neurologists, psychologists,and social workers with consultation available in other appropriate specialtiesincluding orthopedic surgery, psychiatry, and neuosurgery. The School ofNursing already provides teaching in the care of the mentally retarded. TheSimmons College School of Physiotherapy is based at the Children's Hospitalwith special training being provided for the retarded child with orthopedichandicaps. Teaching in the field of communication disorders is provided. TheUniversity-Affiliated Facility is located in the Children's Hospital MedicalCenter complex.

Patients of the University-Affiliated Facility will be referred by variousclinics and divisions of the hospital and by physicians and community agencies.The Children's Hospital Center serves most of the New England region and isa community hospital for a significant portion of the pediatric population inthe Boston area. Where additional consultation in neurology, psychiatry.neurosurgery, otolaryngology, and orthopedic surgery is required. theseservices will be provided. The University-Affiliated Facility provides anursery schaol for intensive observation or for diagnosis and evaluativepurposes. After multiple studies have been concluded, evaluation of thepatient and needs of family will be discussed and determined and a definitiveprogram will be established.

The University Affiliated Facility maintains close liaison with several schoolsspecializing in the training of nursery school teachers and programs of specialtraining of teachers in the field of mental retardation. A modest program invocational counseling, under the auspices of the Society for Crippled Children,has been in effect for the past several years.

The Facility is principally staffed by the departments of medicine, psychiatry.communication disorders. neurology and the divisions of psychology and speechand hearing of the Hospital Center. Affiliated training programs with otheruniversities are supervised through the director of the Center with theappropriate heads of departments.

The Walter E. Fernald State School for the Mentally Retarded, which is affil­iated with the Massachusetts General Hospital and through it, with HarvardUniversity, is the site of both a University-Affiliated Facility and a Centerfor Research on Mental Retardation. The Center for Research received aseparate award of funds under Part A of P.L. 88-164. These two new units makeup the Eunice Kennedy Shriver Center. The Center has two main purposes:(1) To establish research and professional training programs in all thedisciplines concerned with the care of the retarded; and (2) To utilize thetalents and accomplishments of the research and professional training staff todevelop outstanding service programs for both the outpatient and the resi­dential units.

Research programs are conducted in biochemistry, dentistry, epidemiology,genetics, neurology, neuropathology, pediatrics, psychology, psychiatry andspecial education. There are professional training programs in these samedisciplines and also neuroradiology, nursing, nutrition, occupational therapy,opthalmology, physical therapy, social service, speech pathology and audio­logy. In addition to the affiliations with Harvard University, joint trainingand research programs have been established with Boston College, BostonUniversity, Brandeis University, Emerson College, the Massachusetts Instituteof Technology, Simmons College and Tufts University.

Service programs are being carried out in close cooperation with the recentlydeveloped statewide plan. Emphasis has been on the development of communityresources, particularly those which offer alternative patterns of care toresidential placement for the mildly and moderately retarded, and the provi­sion of a therapeutic residential environment which will encourage theemotional, social and intellectual development of the profoundly and severelyretarded.

The University~Af£iliated Facility is an integral part of the New York MedicalCollege complex. The Medical College has operated a retardation center since1950 and has an ongoing inter.disciplinary approach to training~ service. andresearch in the field. The University·Affiliated Facility will allow anadditional 500 persons to be accepted for evaluation and service annually. A20-bed inpatient wing will provide intensive diagnostic, treatment, and train­ing resources, and opportunity for observation of family-patient interaction.

New York Medical College will improve the clinical competence of its trainingprograms by utilizing the Facility to provide a focal point for the diagnosis,care, treatment and rehabilitation of the retarded. With the additionalfacilities, it is expected that the Facility will accept 500 additionalpatients annually for evaluation and service. An inpatient wing of the pro­posed Facility will provide 28 beds for intensive diagnosis, treatment,services and training. Children from infancy through adolescence will beadmitted; however, the Facility will serve patients at all levels of retarda­tion--profound, severe, moderate. mild and border line.

The Administrative Director of the Facility is appointe4 by the Dean of theMedical school. The Director is responsible for the coordination of trainingfrom the affiliated universities: Columbia University, Fordham University,New York University.

Training programs will be conducted in pediatrics, psychology, neurology,psychiatry, social work, speech, nutrition, nursing, vocational rehabilita~

tion, special education, physical and occupational therapy, dance, musicand art therapy. The Training Program in mental retardation will includeall levels of retardation. A variety of individuals, from profoundly tomildly retarded, with associated handicaps, will be represented in theclinical population. Provision is made for re-evaluation. longitudinalstudies and management.

The Divisioh for Disorders of Development and Learning (D.D.D.L.) is a partof the Child Development Institute1s Biological Sciences Research Center(B.S.R.C.). This center is located in the University of North CarolinaSchool of Medici complex, which includes the following Schools: Medicine,Nursing, Public Health, Dentistry and Pharmacy. As a division of the B.S.R.C.,the D.D.D.L. is an integral part of the Medical School program.

The primary function of the D.D.D.L. is the training of professional andspecialized personnel in the diagnosis, treatment and management of retardedchildren. Other major responsibilities include working with practicingphysicians and state-supported clinics throughout the state, and problempatients from these sources will be accepted and returned to the communityafter a comprehensive work-up. The D.D.D.L. will be responsible to theprogram director of the B.S.R.C. It will consist of two related units:(1) the outpatient facility for intensive evaluation, and (2) a day carefacility for retarded children. The outpatient facility will be staffed bya multidisciplinary team to evaluate deviations from normal development andthe establishment of treatment programs for the retardates and their families.Longitudinal follow-up will be a part of the program. The day care centerwill be utilized as part of the treatment program and will likewise involvea wide variety of necessary disciplines.

Approximately 150 new retarded patients will be seen and evaluated annually,and a follow-up system will be instituted to cover a minimum period of fiveyears and return visits will eventually increase from 1,000 to 1)200 annually.

This University-Affiliated Facility will provide programs in twenty-six areasconcerned with prevention and problem solving in mental retardation. TheFacility will also contain programs to train child care aides. home instruc­tors, developmental and remedial physical educators. and will conduct a widerange of tutorial seminars and training sessions for currently active pro­fessional and non-degree candidates.

Coordination with the State mental retardation plan for Ohio insures that theprogram will meet the realistic needs of the community and the region. Thetraining and planning has been interwoven with the c~unity needs and isbased upon interdisciplinary evaluations. management. and long-term planning.Training programs will be coordinated through a training committee whichrepresents the appropriate schools and colleges. all of which are on the samecampus.

The various colleges within the University have been quick to grasp theopportunities which the Facility training program provides: extension ofthe breadth of current training programm and introduction of significantareas of research for more effective prevention and limitation of the impactof mental retardation on the child.

Referral of children and their families. or young adults will be made byhealth, educational, or social agencies as represented by the familyphysician. -the public health nurse, the school principal. a special classteacher, the county welfare agency or a professional person with a voluntaryhealth agency. The children to be seen will range from the mildly to theprofoundly retarded. The age range is expected to be 0-21 years, however,older persons may be considered for assessment.

From the ambulatory or outpatient evaluations area. arrangements can be madeto admit a patient to one of the 22 inpatient beds further studies. Otherindications for admission will be the occurrence of acute illness, childrenwho are being cared for but cannot be managed at home, and social crisis ofshort duration in the family.

The Center will consist of two Facilities, one in Portland on the MedicalSchool campus and one in Eugene on the University of Oregon campus. The proposed inter-campus Center plan is predicatedon the successful experience in a s~ilar program in speech pathology and on18 months experience with an ongoing pilot project in mental retardation beingcarried on. at both campuses.

The crippled Children's Division of the Medical School will implement thepolicy of the University-Affiliated Facility and the Oregon School of Educa­tion at Eugene will plan and execute the curriculum and course work for jointtraining of special education personnel in mental retardation and handicappingconditions.

The School of Social Work at Portland State College and the State Departmentof Special Education will participate in the training program to meet thetraining needwof the State. A physician with specialized training in thefi~ld of pediatrics will be assigned by the Medical School to the Eugenefacility to be responsible for the medical and biological aspects of theteaching program on the University campus.

functions as an information and referral agency~ and also serves as a clinicaltraining facility for students enrolled in several Midsouth universities andcolleges. A large percentage of the patient population is in the lowestsocio-economic group which constitutes the high-risk segment of the mentallyretarded population within the state. The staff of the Center has been activein the Memphis area in developing special education classes in public schoolsfor the educable and the trainable, the development of a day care program forthe severely retarded~ the development of a preschool program for the mildlyand the moderately retarded~ the development of sheltered workshops, and thedevelopment of special classes for children with visual-perceptual motorhandicaps.

Training is offered in several medical specialties, dentistry~ nursing,speech pathology and audiology, psychology, nutrition, social work, specialeducation, rehabilitation counselling~ occupational therapy, and physicaltherapy. Training efforts are directed at both graduate and undergraduatelevels. In addition to inpatient and outpatient service programs and specialeducation classes, a day care program and a preschool program are planned.

Students from the Department of Special Education, Utah State University, andIdaho State University, will receive specialized training in educationaldiagnosis of retarded children that will be extended to master degree candi­dates in remedial reading and in mental retardation. Students in psychiatryat the College of Medicine at the University of Utah will participate inshort-term internships established in cooperation with the Director of ChildPsychiatric Services at the College of Medicine. Students in nursing trainingat Weber State College will participate in short-term internships for trainingin medical training required by retarded persons who receive comprehensivediagnosis and evaluation.

The administration of the Facility will be under the direction of the Dean ofthe College of Education who will be assisted by the Head of the Department ofSpecial Education. An advisory board will be established whose functions willconsist of the development of long-term program plans for future expansion ofthe Facility; clarification of problems involving interagency c~unication;

review of training proposals; and consultation in hiring of personnel.

The University-Affiliated Facility at the University of Washington is acomponent of the Child Development and Mental Retardation Center. Clinicaltraining and behavioral research units are housed in one building; anexperimental education unit occupies a second building; and a third providesthirteen apartment units for short-term residence.

The Clinical Training Unit has a full- and part-time staff of 70 profession­als, representing ten disciplines. Eleven hundred patients from throughoutthe state made over 9,000 visits to the unit during the past year. Inaddition, a field team maintained liaison and provided in-service training~nd consultation to 12 regional clinics and various communities. Over 800students received training in the diagnosis and management of handicappingconditions such as mental retardation, learning disabilities and others.Included in this number were medical students, pediatric residents and post­doctoral fellows, speech and hearing trainees and fellows, nutrition traineesand fellows, psychology graduate students and interns, psychiatry residentsand fellows, nursing undergraduates, pre- and post~stersl nursing studentsand graduate social work students.

The Experimental Education Unit provides 13 classrooms for children rangingin age from 2-21 years. During the past year. 172 children from 17 differentschool districts were enrolled. Most of these were children so severelyhandicapped by mental retardation, emotional or behavior problems or bylearning disabilities, that they were unable to participate in regular orspecial educational programs in their own school districts. The schoolprogram places emphasis on behavior modification principles and utilizes adetailed individual analysis of learning and behavior problems. continuousmeasurement of pupil performance and modification of materials and incentivesto meet the needs of the individual child. The Behavioral Research Unit, together with the staff of the ClinicaiT'raining Unit, is engaged in a broad program of research related to theenvironmental. psychological and psychiatric factors involved in deviant'developme~t.

A residential facility provides short-term housing for families of childrenbrought in for evaluation. It is also utilized for research on familyinter-action. for the training of professional students in home-managementproblems, and for the training of parents.

The Community Facilities Construction Program was authorized under Title I,Part C of the "Mental Retardation Facilities and Community Mental HealthCenters Construction Act of 196311 (P.L. 88-164) and provided Federal grantsto states to assist in the construction of specially designed public orother nonprofit facilities for the diagnosis, treatment, education, train­ipg, or persona~ care of the mentally retarded, including sheltered work­shops which are part of facilities providing comprehensive services. TheDevelopmental Disabilities Services and Facilities Construction Act of1970 (Title 1, Public Law 91-517), replaces the authority of Part C, TitleI, Public Law 88-164. Title I of the Act of 1970 authorizes formula grantsto states for planning, administration, and construction of facilities forthe developmentally disabled. Further summarized information about PublicLaw 91-517 is in the Introduction on pages v and vi.

A list of definitions describing both the facilities and the services isincluded on pages 91-92. The program is administered at the state levelby an officially designated state agency. participation in the programrequires the development of a state plan for the construction of communityfacilities for the mentally retarded based on an inventory of neededadditional services and facilities. Construction projects are approvedin accordance with the provisions of the state plan. Designated stateagencies and allocations for construction of community facilities forthe mentally retarded are shawn on pages 92-95.

As of December 31, 1970, 362 projects had been approved for Federalassistance. The projects are classified by function; additional datais included in tabular form on pages 60-90. Of the 362" projects, 108have been completed, 9S are under construction, and 170 are in the initialapproval stage. The total estimated cost is approximately $208.4 millionand the estimated Federal share is $74.6 million. Upon completion theprojects will provide services to more than 114,000 retardates of which57,000 have not previously received services, and 57,000 will be providedimproved and more extensive services.

This construction program is administered by the Division of MentalRetardation, Rehabilitation Services Administration, Social andRehabilitation Service, U. S. Department of Health. Education, andWelfare.

DEFINITIONS

I. Facilities:

A. D&E Clinic (Diagnostic and Evaluation Clinic): A facility providiogdiagnostic and evaluation services only.

B. Day Facility: A facility operating for care and treatment of thementally retarded on less than 24 hour-a-day basis, providing diag­nosis (and evaluation), treatment, education, training, personalcare or sheltered workshop services.

C. Residential Facility: A facility operating for care and treatmentof -the mentally retarded on a 24 hour-a-day basis providing diagNnosis (and evaluation), treatment, education, training, personalcare, or sheltered workshop services.

II. Services:

A. Diagnosis and Evaluation: Coordinated medical, psychological, socialservice, educational, speech pathology, audiology, nursing, occupa­tional therapy, physical therapy, nutrition, vocational and otherrelated services carried out under the supervision of qualifiedpersonnel to assess the individual's abilities and disabilities, andfamily and community resources. Based on this assessment a plan tomeet the needs of the individual and his family is developed andmethods of implementation recommended. Periodic reassessment of theindividual where needed is an essential aspect of this Service.

B. Treatment: Appropriate medical, dental, physical therapy, occupa.tional therapy, hearing therapy, psychotherapy, social service andrelated services carried out under the direction of qualifiedpersonnel to bring about improvement in effective physical, psycho­logical or social functioning of the mentally retarded individual.

c. Education: Services based on a structured curriculum of instructioncarried out under supervision of teachers qualified in specialeducation to meet the needs of the retarded in preschool, primary,intermediate, ,prevocational, vocational and other appropriate areas.

D. Training: Services carried out under the supervision of qualifiedpersonnel which provide for training of the retarded individual inmotor skills, activities of daily living, vocational training, per­sonality development, socialization, recreation and other relatedcomponents.

E. Personal (Custodial) Care: Services providing for the dietary,shelter, clothing, medical care, nursing and other daily personalneeds of the retarded to insure proper personal care during thetime the individual is utilizing the various other servicesoffered by the facility.

F. Sheltered Workshop: Services carried out under the supervision ofqualified personnel involving a program of paid work involving workevaluation, work adjustment training, occupational skill training,supervised transitional employment, placement and other relatedcomponent services.
