Record Reviews: resident records,discharge notice, hospitalrecords

Allegation Summary:1. The named resident no longer lived in the home. was interviewed in another setting. The resident stated received goodcare in the home and was unsure why the provider would not let return to the home. Staff was observed to providecare/services to residents during the investigation. Residents were interviewed about the care and had no concerns.2. Per record review the home issued a 30 day discharge notice for non-payment, but the resident was sent to the hospitalbefore the notice was complete and the home declined to readmit the resident based on not being able to meet the resident'sneed. Per interview with the resident and other collateral contacts there was no attempt to reasonably accommodate theresident to avoid the discharge. The resident's record contained no information regarding adjustments/attempt to meet theresident's needs and/or evaluation of the resident after was treated at the hospital for an acute illness. Further record reviewand interview of collateral contacts identified the resident's needs had not changed from original admission to the home. Theprovider and resident manager were interviewed and they identified the reasons for declining readmission, including:nonpayment, no practitioner and issues with the resident's Power of Attorney. However, the home did not attempt to resolve theissues in order to keep the resident at the adult family home.
