INTENDED USETB Stain Kits are used to stain smears prepared from specimens and cultures suspected of containing mycobacteria for early presumptive diagnosis of mycobacterial infection and characterization of bacterial isolates.

SUMMARY AND EXPLANATIONThe microscopic staining technique is one of the earliest methods devised for detecting the tubercle bacillus and it remains a standard procedure.1 The unique acid-fast characteristic of mycobacteria makes the staining technique valuable in early presumptive diagnosis as well as in providing information about the number of acid-fast bacilli present.Fluorescent microscopy offers numerous advantages over classic methods for detecting mycobacteria because of its speed and simplicity, the ease of examining the slide and the reliability and superiority of the method.2

The TB Stain Kit K uses the Kinyoun (cold) acid-fast procedure.3

The TB Stain Kit ZN uses the Ziehl-Neelsen (hot) acid-fast procedure.4

The TB Fluorescent Stain Kit M uses the auramine O acid-fast fluorescent procedure.5

The TB Fluorescent Stain Kit T uses the auramine-rhodamine acid-fast fluorescent procedure described by Truant, Brett and Thomas.6

PRINCIPLES OF THE PROCEDUREA major characteristic of mycobacteria is that they are “acid-fast,” i.e., once stained with the aniline dye, basic fuchsin, they are difficult to decolorize, and retain the red color even when treated with a mixture of acid and alcohol.In the Ziehl-Neelsen stain, methylene blue is the counterstain. In the Kinyoun stain, brilliant green provides the necessary contrast.Fluorescent dyes possess the property of emitting visible radiation when stimulated by shorter wavelength ultra violet light, i.e., a mercury vapor lamp or a strong, blue appropriately filtered light source.7

Auramine O and rhodamine B are reported to bind to mycolic acids in the mycobacterial cell wall.8 Unbound dye is removed by the acid-alcohol decolorizer solution. Potassium permanganate serves as a counterstain to suppress nonspecific background fluorescence.

Biosafety Level 2 practices and procedures, containment equipment and facilities are required for non-aerosol-producing manipulations of clinical specimens such as preparation of acid-fast smears. All aerosol-generating activities must be conducted in a Class I or II biological safety cabinet. Biosafety Level 3 practices, containment equipment and facilities are required for laboratory activities in the propagation and manipulation of cultures of M. tuberculosis and M. bovis. Animal studies also require special procedures.11

These reagents are harmful or fatal if swallowed and can cause eye irritation or blindness. In the event of eye contact, flush thoroughly with an eye wash system or tap water for at least 15 min and seek medical advice. The dye solutions and decolorizer solution are highly flammable and vapors may be harmful; use away from open flame. Avoid contact with skin, eye or mucous membranes. Use all reagents in a well-ventilated area, avoiding prolonged inhalation, skin contact and ingestion.

SPECIMEN COLLECTION AND PREPARATIONApply a thin smear of the specimen directly on a clean microscope slide. Sputum (direct specimen or concentrated sputum), spinal or other body fluid (catheterized urine), homogenized tissue and impression slides are satisfactory specimens for smears.1

Allow smear to dry.Fix the smear to the slide by passing the slide through a low flame 2–3 times, avoiding excessive heat, or use an electric slide warmer at 65 to 75 °C for at least 2 h.13

Test ProcedureKINYOUN STAINTB Stain Kit KInstructions:1. Place slides on a staining rack and flood with TB Carbolfuchsin KF for 4 min. Do not heat.2. Wash gently in running water.3. Decolorize with TB Decolorizer for 3–5 s.4. Wash gently in running water.5. Counterstain with either TB Brilliant Green K or TB Methylene Blue (available separately) for 30 s.6. Wash gently in running water.7. Air dry. If using TB Methylene Blue, dry over gentle heat.

ZIEHL-NEELSEN STAINTB Stain Kit ZNInstructions:1. Place slides on a staining rack and place a piece of filter paper, larger than the size of the smear, on each slide. Flood with TB

Carbolfuchsin ZN. Heat gently to steaming and allow to steam for 5 min.2. Remove the filter paper. Wash gently in running water.3. Decolorize with TB Decolorizer with two changes of reagent for 1–2 min until no more red color appears in washing.4. Wash gently in running water.5. Counterstain with either TB Methylene Blue or TB Brilliant Green K for 30 s.6. Wash gently in running water.7. Dry over gentle heat.

MORSE STAINTB Fluorescent Stain Kit MInstructions:1. Place slides on a staining rack and flood with TB Auramine M for 15 min.2. Wash gently in running water.3. Decolorize with TB Decolorizer TM for 30–60 s.4. Wash slides gently in running water.5. Counterstain with TB Potassium Permanganate for 2 min.6. Wash gently in running water.7. Air dry.8. Examine under a microscope fitted with filter sets as described by Chapin and Murray.14

TRUANT STAINTB Fluorescent Stain Kit TInstructions:1. Place slides on a staining rack and flood with TB Auramine-Rhodamine T that has been thoroughly shaken prior to use.

Leave undisturbed for 20–25 min at room temperature.2. Wash gently in running water.3. Decolorize with TB Decolorizer TM for 2–3 min.4. Wash gently in running water.5. LIMITATIONS OF THE PROCEDUREA positive staining reaction provides presumptive evidence of the presence of mycobacteria in the specimen. A negative staining reaction does not necessarily indicate that the specimen will be culturally negative for mycobacteria. RESUME ET EXPLICATIONLa technique de coloration microscopique a été l’une des premières méthodes conçues pour détecter le bacille tuberculeux et demeure une méthode de référence.1 La caractéristique acido-résistante propre aux mycobactéries rend la technique de coloration utile au diagnostic présomptif précoce et permet d’évaluer le nombre de bacilles acido-résistants présents.Le microscope à fluorescence offre de nombreux avantages par rapport aux méthodes traditionnelles de détection des mycobactéries : vitesse et simplicité, facilité d’examen de la lame, et fiabilité et supériorité de la méthode.2

PRINCIPES DE LA METHODEL’une des caractéristiques principales des mycobactéries est leur « acido-résistance ». Une fois colorées avec un colorant à base d’aniline, la fuchsine basique, elles sont difficiles à décolorer et retiennent la coloration rouge, même quand elles sont traitées par un mélange d’acide et d’alcool.Dans la coloration de Ziehl-Neelsen, le bleu de méthylène est le contre-colorant. Dans la coloration de Kinyoun, le vert brillant assure le contraste nécessaire.Les colorants fluorescents ont la propriété d’émettre un rayonnement visible lorsqu’ils sont stimulés par un rayonnement ultraviolet produit par une lampe à vapeur de mercure ou une lumière bleue intense correctement filtrée.7

L’auramine O et la rhodamine B sont décrites comme se liant aux acides mycoliques de la paroi cellulaire mycobactérienne.8 Le colorant non lié est éliminé par une solution décolorante d’alcool acide. Le permanganate de potassium sert de contre-colorant pour éliminer la fluorescence parasite non spécifique.

Les activités de laboratoire impliquant la propagation et la manipulation de cultures de M. tuberculosis et M. bovis nécessitent des pratiques de sécurité biologique de niveau 3, ainsi que les installations et le matériel de confinement correspondants. Les études chez l’animal nécessitent également des procédures particulières.11

Ces réactifs sont nocifs ou mortels en cas d’ingestion et peuvent provoquer des irritations oculaires ou entraîner la cécité. En cas de projection dans les yeux, rincer soigneusement avec un laveur d’yeux ou sous l’eau courante pendant au moins 15 min et consulter un ophtalmologiste. PRELEVEMENT ET PREPARATION DES ECHANTILLONSRéaliser un fin frottis de l’échantillon directement sur une lame de microscope propre. ZUSAMMENFASSUNG UND ERKLÄRUNGDas mikroskopische Färbeverfahren ist eines der ältesten Methoden für den Nachweis von Tuberkelbazillen und gilt nach wie vor als Standardverfahren.1 Das Färbeverfahren ist aufgrund der charakteristischen Säurefestigkeit von Mykobakterien ein wertvolles Hilfsmittel für die präsumtive Frühdiagnose und liefert Informationen über die Anzahl der vorhandenen säurefesten Bakterien.

Die Fluoreszenzmikroskopie bietet zahlreiche Vorteile gegenüber klassischen Methoden zum Nachweis von Mykobakterien, da sie schnell und einfach durchzuführen ist, eine zuverlässige und überlegene Methode2 darstellt und der Objektträger leicht zu untersuchen ist.Das TB Stain Kit K ist für die Kaltfärbemethode für säurefeste Bakterien nach Kinyoun vorgesehen.3

Das TB Stain Kit ZN ist für die Heißfärbemethode für säurefeste Bakterien nach Ziehl-Neelsen vorgesehen.4

Das TB Fluorescent Stain Kit M ist für das Fluoreszenzverfahren für säurefeste Bakterien mit Auramin O vorgesehen.5

VERFAHRENSPRINZIPIENEines der wichtigsten Merkmale von Mykobakterien ist ihre „Säurefestigkeit“, d. h. dass sie nach der Färbung mit basischem Fuchsin – einem Anilinfarbstoff – nur schwer zu entfärben sind und ihre rote Farbe selbst bei Behandlung mit einer Mischung aus Säure und Alkohol beibehalten.Bei der Ziehl-Neelsen-Färbung wird Methylenblau als Gegenfärbereagenz verwendet. Bei der Kinyoun-Färbung wird der erforderliche Kontrast mit Brillantgrün erzielt.Fluoreszenzfarbstoffe können bei Stimulation durch kurzwelliges Ultraviolettlicht (z. B. anhand einer Quecksilberdamp flampe oder einer starken, ausreichend gefilterten blauen Lichtquelle) sichtbare Strahlung emittieren.7

Auramin O und Rhodamin B binden an Mycolsäuren in der Zellwand von Mykobakterien.8 Ungebundener Farbstoff wird durch die Säure/Alkohol-Entfärbelösung entfernt. Kaliumpermanganat dient als Gegenfärbereagenz, um unspezifische Hintergrundfluoreszenz zu unterdrücken.

Das Arbeiten mit klinischen Proben, bei denen (wie bei der Herstellung von säurefesten Ausstrichen) keine Aerosole entstehen, erfordert Laborpraktiken und Verfahren sowie Sicherheitsvorrichtungen der Biosicherheitsstufe 2. Alle Arbeiten, bei denen Aerosole entstehen, müssen in einer biologischen Sicherheitswerkbank der Klasse I oder II durchgeführt werden. Für Laboraktivitäten zur Vermehrung und Handhabung von M. tuberculosis- und M. bovis-Kulturen sind Verfahren, Behälter und Einrichtungen der biologischen Sicherheitsstufe 3 einzusetzen. Tierstudien erfordern darüber hinaus ebenfalls besondere Verfahren.11

Diese Reagenzien sind bei Ingestion gesundheitsschädlich oder tödlich und können Augenreizungen oder Blindheit verursachen. Bei Kontakt mit den Augen mindestens 15 Min lang gründlich mit einem Augenwaschsystem oder Leitungswasser spülen und ärztliche Hilfe hinzuziehen. Die Färbelösungen und der Entfärber sind hochentzündlich und können gesundheitsschädliche Dämpfe abgeben. Nicht in der Nähe offener Flammen verwenden. Kontakt mit Haut, Augen oder Schleimhäuten vermeiden. Alle Reagenzien in einem gut belüfteten Bereich verwenden, längere Inhalation, Kontakt mit der Haut und Ingestion vermeiden.

PRINCIPI DELLA PROCEDURAUna delle caratteristiche fondamentali dei micobatteri è la proprietà di essere “acido-resistenti”; ciò significa che una volta colorati con anilina, fuscina basica, sono difficilmente decolorabili e conservano il colore rosso anche se trattati con una miscela di acido e alcol.Nella colorazione Ziehl-Neelsen, la controcolorazione è costituita dal blu di metilene. Nella colorazione Kinyoun, il necessario contrasto è fornito dal verde brillante. I coloranti in fluorescenza hanno la proprietà di emettere radiazioni visibili allorché stimolati da luce ultravioletta a inferiore lunghezza d’onda, vale a dire una lampada a vapore di mercurio o una fonte di forte luce blu, adeguatamente filtrata.7

È documentato che auramina-O e rodamina-B si legano agli acidi micolici nella parete cellulare dei micobatteri.8 Il colorante non legato viene quindi rimosso dalla soluzione decolorante acido-alcol. Il permanganato di potassio funge da controcolorazione e sopprime la fluorescenza aspecifica di fondo.

Le soluzioni coloranti e la soluzione decolorante sono altamente infiammabili e i vapori possono essere nocivi; usare lontano da fiamme libere. PRELIEVO E PREPARAZIONE DEI CAMPIONIStendere uno strato sottile di campione direttamente su un vetrino pulito per microscopio. Espettorato (campione diretto o espettorato concentrato), fluido spinale o altri fluidi corporei (urina da catetere), tessuto omogeneizzato e vetrini per impronta sono campioni appropriati per gli strisci.1

Attendere che lo striscio si asciughi.Fissare lo striscio sul vetrino passando quest’ultimo su fiamma bassa 2–3 volte, senza esporre a calore eccessivo, oppure usando uno scaldavetrino elettrico a 65–75 °C per almeno 2 h.13

USO PREVISTOLos kits de tinción TB se utilizan para teñir frotis preparados a partir de muestras y cultivos que se sospecha que contienen micobacterias, para el diagnóstico de sospecha precoz de infección micobacteriana y la caracterización de aislados bacterianos.

RESUMEN Y EXPLICACIÓNLa técnica de tinción microscópica es uno de los métodos más precoces diseñados para detectar el bacilo tuberculoso, y continúa siendo un procedimiento estándar1. La característica exclusiva de ácido-resistencia de las micobacterias hace que la técnica de tinción sea útil en el diagnóstico de sospecha precoz así como para proporcionar información sobre el número de bacilos ácido-resistentes presentes.

La microscopia fluorescente ofrece numerosas ventajas sobre los métodos clásicos para la detección de micobacterias debido a su velocidad y sencillez, a la facilidad para examinar el portaobjeto y a la fiabilidad y la superioridad del método2.El kit TB Stain Kit K utiliza el procedimiento de ácido-resistencia de Kinyoun (en frío)3.El kit TB Stain Kit ZN utiliza el procedimiento de ácido-resistencia de Ziehl-Neelsen (en caliente)4.El kit TB Fluorescent Stain Kit M utiliza el procedimiento de fluorescencia y ácido-resistencia de la auramina o5.El kit TB Fluorescent Stain Kit T utiliza el procedimiento de fluorescencia y ácido-resistencia de auramina-rodamina descrito por Truant, Brett and Thomas6.

FUNDAMENTO DEL PROCEDIMIENTOUna característica esencial de las micobacterias es que son “ácido-resistentes”, es decir, que una vez teñidas con el colorante de anilina, fucsina básica, son difíciles de descolorar y conservan el color rojo aunque sean tratadas con una mezcla de ácido y alcohol.En la tinción de Ziehl-Neelsen, el contracolorante es azul de metileno. En la tinción de Kinyoun, el verde brillante proporciona el contraste necesario.Los colorantes fluorescentes poseen la propiedad de emitir radiación visible cuando son estimulados por la luz ultravioleta de longitud de onda más corta, como una lámpara de vapor de mercurio o una fuente de luz azul intensa debidamente filtrada7.La auramina O y la rodamina B se unen a los ácidos micólicos de la pared celular micobacteriana8. El colorante libre es eliminado por la solución decolorante ácido-alcohólica. El permanganato potásico actúa como contracolorante para eliminar la fluorescencia de fondo inespecífica.

Advertencias y precauciones: Para uso diagnóstico in vitro.En las muestras clínicas puede haber microorganismos patógenos, como los virus de la hepatitis y el virus de la inmunodeficiencia humana. Para la manipulación de todos los elementos contaminados con sangre u otros líquidos corporales, deben seguirse las “Precauciones estándar”9-12 y las directivas del centro. Se requiere la utilización de prácticas y procedimientos de seguridad biológica de nivel 2 y equipo e instalaciones para contención cuando se manipulen muestras clínicas sin producir aerosoles, como en la preparación de frotis acidorresistentes. Todas las actividades que generen aerosoles deben llevarse a cabo en un gabinete de seguridad biológica de clase I o II. Se requiere la utilización de prácticas de seguridad biológica de nivel 3 y equipo e instalaciones para contención en las actividades de laboratorio que incluyan la propagación y manipulación de cultivos de M. tuberculosis y M. bovis. Los estudios en animales también requieren la implementación de procedimientos especiales11.Estos reactivos son nocivos o mortales en caso de ingestión y pueden causar irritación de los ojos o ceguera. En caso de contacto con los ojos, lávense a conciencia con un sistema de lavado ocular o con agua del grifo durante al menos 15 min y acúdase al médico. La soluciones colorantes y la solución decolorante son muy inflamables y los vapores producidos pueden ser nocivos, por lo que estas soluciones deben utilizarse lejos de llamas abiertas. Evítese el contacto con la piel, los ojos y las mucosas. Todos los reactivos deben utilizarse en un área bien ventilada, evitando una inhalación prolongada, el contacto con la piel y la ingestión.

RECOGIDA Y PREPARACIÓN DE LAS MUESTRASAplique un frotis fino de la muestra directamente sobre un portaobjeto limpio. El esputo (muestra directa o esputo concentrado), el líquido cefalorraquídeo y otros líquidos corporales (orina por sondaje), los tejidos homogeneizados y los portaobjetos de impresión son muestras apropiadas para los frotis1.Deje que se seque el frotis.Fije el frotis al portaobjetos pasando éste a través de una llama baja 2–3 veces, evitando un calor excesivo, o utilice un calentador eléctrico de portaobjetos entre 65 y 75 ºC durante 2 h como mínimo13.

LIMITACIONES DEL PROCEDIMIENTOUna reacción de tinción positiva ofrece evidencia presuntiva de la presencia de micobacterias en la muestra. Para la identificación positiva de micobacterias hay que utilizar otros métodos (de cultivo, moleculares, etc.).
