    Proposal narrative:

    Agency History:

    The Epilepsy Association of Bangladesh, Inc. (EAB) is an independentnot-for Profit Corporation chartered by the Bangladesh in 1986, andoperating only under this name since its inception. EAB is affiliatedwith, NISH (National Industries for the Severely Handicapped) andBESPA (Bangladesh Epilepsy Services Provider Association). Adedicated volunteer Board of Directors, as well as a ProfessionalAdvisory Board that is made up of area physicians and professionalsled EAB. EAB operates a client case management and medical servicesprogram (Epilepsy Services Program-ESP) that is open to all residentsin concerned districts. Our prevention and education programs have

    offerings that continually promote awareness of persons with epilepsyand disabilities. The Community Based Work Adjustment & PlacementProgram, funded by and large by the Brack Bank, was successfullycompleted in 1997-98.The current program: Community BasedSupportive Employment Skills Training &Placement Program has beenin progress since July 15, 2005, and has effectively resulted in 48clients with epilepsy and other disabilities successfully andcompetitively employed (at the time of this proposal). These 48 clientshave completed the program and another 24 clients are currentlyworking towards their employment goals via this program. We hadprojected in our first proposal that we would successfully have 28-

    33clients employed at program completion. This program has beenextremely successful to date and is a result of the EmploymentCoordinators disclosure of her epilepsy.

    Mission Statement:

    The Epilepsy Association of Bangladesh, Inc. (EAB) is dedicated toimproving the quality of life of persons affected by epilepsy/seizuredisorders. EAB identifies, defines and advocates the needs of personswith epilepsy/seizure disorders, and plans and implements strategiesto meet those needs. EAB provides information and education topersons with epilepsy and their families, educators, employers, themedical community and the general public in order to further theirunderstanding of epilepsy and the needs of persons with epilepsy and

    to increase the communitys responsiveness to those needs. EABassists persons with epilepsy/seizure disorders in securingaccurate/timely information, diagnosis, treatments and rehabilitationservices so that they may take/maintain their place in society asproductive citizens.

    EABs proposal directly addresses our mission by:

    improving the quality of life of persons with epilepsy,

    client advocacy,

    meeting the needs of persons with epilepsy (Fostering self-sufficiency, vocational assistance and job placement),

    and informing/educating the community (employer educationand job creation) of these needs, along with disability awareness.

    Area and Population Served:

    EABs Employment Program serves individuals Bangladesh. TheEpilepsy Services Program serves clients in 6 districts area inBangladesh. An estimated total population ofover 2 million peopleresides in these districts and according to the Bangladesh CensusBureaus data for 2009 estimates; over 55000 persons with disabilitiesreside in these 6 districts. Our mission is to serve the estimated 5,180

    plus individual affected with epilepsy/seizure disorders in this area andmore specifically, facilitate a minimum of 77 persons with epilepsy tobecome employed or advancing in their current employment. We arehappy to assist any person or persons affected with disabilities andencourage referrals from other service providers.

    Project Plan & Description:

    Our qualified Employment Coordinator will carry on the communitybased work placement/skills training program and add essentialtraining portions to the program, which we have found to be necessaryto increase selfsufficiency.This expanded program would begin July 16,2011 so as not to overlap current funding and allow for the additionaltraining programs that have been requested

    and delivered on a very limited basis. Personal Financial Fitness andTransportation Training are essential to increasing the employmentrate for individuals with epilepsy and disabilities. EAB currently hasprograms addressing epilepsy education, medical care, and socialservice counseling. The proposed program extension would work inconjunction, compliment and enhance existing EAB programs.

    Under this project we will provide the following services to at least 110clients in the coming year:

    Setting a Vocational Goal & Identifying Work Values

    Personal Financial Needs and Basic Financial Planning/BudgetingSkills Training

    Epilepsy/Disability and the Essential Duties of the job

    Epilepsy/Disability Disclosure

    Transportation to Work and Transportation Training

    Resources for Identifying Employers, Job Skills Training, &Resume Writing

    Interviewing, Follow-up and What Employers Want

    presentations toparticipants, successful community peers (employed individuals withepilepsy or other materials as needed on a participant basis.

    Targeted individuals for this program would include, but not limited to:

    Current and future clients of EABs existing programs, referralsfrom area

    Physicians and service providers and disabled individuals seeking

    employment skills training and transportation training;

    Those who lack previous work experience and the confidence towork due to their disability;

    Those that have demonstrated previously the inability toretain a job for a reasonable period of time due to theirdisability.

    Expected Project Outcomes:

    The twelve-month goals of this proposed expanded program are:110+ total program participants with a projected employment of77+individuals.

    Secondary goals are for participants to complete the program with greater self-

    esteem, self-worth, and the skills necessary to maintain permanent employment

    along with a greater understanding of their disability and how to cope with itindependently.

    Measurement Activities:

    Each participant will have a case manager action plan to assistwith medical services and medications in order to decreaseseizures and stabilize the clients epilepsy.

    Each participant will have an Employment Program Action Plan

    (EPAP) designed for him or her by the employment coordinatorthat will be monitored for success;

    The total number of program participants will be recorded alongwith pre/post testing for knowledge and skills of each participant;

    Measurement tools and checklists will be in place to followparticipant

    accomplishments and activities along the path to employment;

    Participant surveys for to measuring success and additionalsupportive needs;

    Amount Requested/Total Project Cost:

    The BRAC BANK funding request is $34,780.00 with a total programcost of $78,073.00. The EAB match equals $43,293.00.

    Other Sources of Funding:

    This program will result in at least 77 clients employed and it will becost effective because the EAB belong with CJF (Central Jakat Fund).Foundation will provide financial assets and funds for the proposedproject. EAB will provide the office space, training space, officeequipment, local telephone access, insurances, audit fees and licenses.Through program, corporate, individual and fundraising efforts, EABwill are able to match the funds needed to complete this mostimportant program.

    Plans for Future Funding :

    Private fundraising, county, state being sought to continue allprograms. EAB is successfully completing a current program and weare confident .EAB programs and we raised over Tk.10 lack in

    fundraising in 2005, and tk.9 lack+ in Foundation funding to continueto support our additional program expenses and assist with additionalcase management costs, client transportation costs, prevention andeducation costs.

    .We have also requested funding from the CJF Foundation and the DBDLFoundation, to cover the costs of staff training and counseling, and the cost ofsupporting community-based projects respectively. District Governments haveagreed to cover the direct material grants. A letter from each of the DistrictGovernment heads to this effect is included as Appendix 5.
