Her medical license enables her to diagnose illness and injury, provide treatment, prescribe appropriate medication, and work closely with the SMART behavioral health provider(s) to support our “Whole Child, Whole School” focus. Kathryn’s most recent experience was gained at Drs. Concannon & Vitale’s Pediatric and Adolescent Medicine in Cranston, Rhode Island. As a Licensed Medical Provider she assessed and advised pediatric and adolescent clients, caring for 15-20 clients a day. This direct experience with the age base at Roger Williams Middle School will be exceptional in helping her establish mutually acceptable plans and productive rapport with students. Kathryn earned a Bachelor of Science in business hospitality from the University of Massachusetts and a Bachelor of Science in Nursing from Our Lady of the Elms College. Her Master of Science in Nursing, Family Nurse Practitioner, was completed at Simmons College in Boston, Massachusetts. Her current licensing includes RI Registered Nurse, RI CSR-APRN, RI APRN CNP Family/Individual Lifespan, and ANCC Family Nurse Practitioner Board Certification. Kathleen Pena, LCSW, CGS Kathleen brings over six years of experience in behavioral health and therapeutic treatment for individuals of all ages. She earned her Master of Social Work from the Rhode Island College School of Social Work with a Certificate in Childhood and Adolescent Trauma. She is an award-winning clinician, receiving, in 2018, both the Anthony E. Ricci Social Work Practice Award and the National Engaged Leader Award. She possesses expertise in a wide range of treatment arenas including family services, human rights, boundaries and ethics, and SBRIT training. Working in collaboration with our Licensed Medical Provider, the SMART Team, and the student, Kathleen will define expected patient outcomes, describe appropriate resource utilization, and develop individual goal plans. Kathleen’s role within the SMART Team will maximize SMART’s capacity to support a trauma-informed framework and healing, in keeping with the SMART “Whole Child, Whole School” approach.

Meet Your Roger Williams Middle School SMART Team

Kathryn Oberacker, Licensed Medical Provider, (FNP-BC), BS, MSN Kathryn’s career as a Licensed Medical provider spans over 12 years. A Certified SMART Student Health and Wellness Provider

Damaris Rosales, Utilization and Engagement Manager For over seven years, Damaris has served as Navegante Coordinator for Clínica Esperanza/Hope Clinic (CEHC). During her tenure at CEHC, Damaris earned practical knowledge within the realm of a health care clinic. Through hands-on experience, she gained proficiencies in team management, training programs, balancing detailed schedules, community outreach, and customer service skills, all with a passion for client advocacy. Her community connection and desire to serve will

enhance her role as a crucial link between the students/patients and care delivered by the SMART Clinical Staff. She will support SMART’s purpose to improve academic outcomes, school climate, and culture by enabling and enhancing the SMART Team’s capacity to deploy Active Access to Active Care. She intermittently represented CEHC at community partner meetings and is well integrated within the local non-profit sector. Damaris’ educational accomplishments include a variety of relevant coursework and certifications in medical office skills, medical terminology, diabetes prevention, peer recovery, and medical interpretation. She is bilingual and certified as a Community Health Outreach Worker.

Teresa Nico, BSN, LPN Teresa brings over two decades of experience in the health care field. Her background in mental health, pediatric, and adolescent care will be especially valuable in effectively serving our students at Roger Williams. Teresa is also a Certified Spanish Health Interpreter and a 20-year resident of Rhode Island. Teresa enjoys fulfilling community needs, which makes this a most gratifying transition for her. As an LPN, Teresa will care for Roger Williams’ students by supporting our SMART Licensed Medical

Provider, Kathryn Oberacker, and our Behavioral Health Provider, Kathleen Pena. She will promote continuity of care and triage students in keeping with the SMART “Whole Child, Whole School” approach. Teresa holds a Bachelor of Science in Nursing from Interamerican University. She also holds an Associate of Arts and a Diploma in Practical Nursing from the Community College of Rhode Island.

A Certified SMART Student Health and Wellness Provider

Yolanda Santiago, Health and Wellness Coordinator Yolanda brings over 22 years of experience in communication, management, and organization within an office setting. Her dedication and determination have earned her advancement from an entry-level position to her most recent responsibilities as Senior Administrative Assistant and Medical Interpreter. Yolanda has proven success within the ongoing responsibilities of a clinical setting including opening and closing a clinic, maintaining medical records, data entry, insurance

verification, medical software and confidentiality, coordination of various schedules, newsletter design, and general office oversight, all while providing exemplary customer service. Yolanda possesses strong experience in collaborating with medical professionals and additional support staff as well as utilizing her bilingual proficiencies in various health care settings.

As a graduate of East Providence Vocational High School, Yolanda is looking forward to serving her community through the SMART Model at Roger Williams Middle School.
