Our development of a biomedical device will measure milk as a transfer system and the infant’s regulatory breastfeeding behaviors as a screening assessment of neurodevelopment. My ultimate goals are to train researchers in biopsychosocial science, to advance nursing knowledge for improving breastfeeding outcomes in the U.S. and the world, to advocate for women, and to give infants a voice that they need support to learn how to breastfeed and other complex behaviors in the future. RESEARCH NARRATIVE_______________________________________________________________ My primary scholarly goal is to shift the breastfeeding paradigm from a maternal-only psychological

approach to an infant-maternal biopsychosocial science. Using the three common reasons for breastfeeding cessation – ongoing breast and nipple pain during breastfeeding, maternal perception of milk insufficiency, and a lack of infant satiation – my science investigates breastfeeding cessation using biopsychosocial mechanisms within the dyadic interaction between infant and mother.

Breastfeeding discomfort is expected during breastfeeding initiation, but it is unclear how many women experience pain during and continuing beyond the first week. My NINR-funded pilot study evaluated a breastfeeding self-management intervention for women who experienced breast and nipple pain while collecting biopsychosocial data on the molecular-genetic basis of risk for pain. The intervention provided cloud-based educational modules, nurse-led text-based coaching and reflective journaling to mothers in their home during the critical two weeks after delivery when the peak of breastfeeding cessation occurs. The intervention significantly reduced pain, compared with controls and identified an OXTR single nucleotide polymorphism minor allele as a suggestive “breastfeeding pain” phenotype for women. I submitted an RO1 to establish the efficacy and clinical sustainability trial of the intervention, which I plan to complete in 5 years. My 10-year goal is a large-scale implementation trial in practice settings. An additional application of the molecular risk of pain and addictive behaviors is our ongoing study, funded by the American Nurses Foundation, which explores pain sensitivity in women who are self-administering marijuana for management of pain during pregnancy.

An emerging area of exploration is maternal perception of milk insufficiency. My work heretofore suggests that pain has a neurohormonal effect on milk production. Within 10 years, my goal is to explore the underlying molecular basis of milk insufficiency in the presence of pain.

Breastfeeding cessation due to lack of infant satiation (poor weight gain, signs of hunger after a breastfeeding session) is caused by infant biometrics – poor mouth seal, low intraoral pressure, or dysregulation of breathing and sucking. Infant biometrics may lead to an ineffective triggering of the mother’s neurohormonal cascade necessary for milk sufficiency. In December, 2019, I submitted a NSF SBIR application and in April 2020, a patent for a Breastfeeding Diagnostic Devise (BDD) to measure infant biometrics and maternal milk flow during breastfeeding. The BDD data will create a clinical algorithm to identify the reason(s) for breastfeeding cessation - infant biomechanics, milk insufficiency, molecular risk for pain, or all three. In the next 10 years, the clinical algorithm will elucidate the differences between low-risk (full-term) and high-risk (preterm, or infants with a familial history of autism spectrum disorder diagnosis) infants to identify a newborn phenotype (biometrics and molecular genetic risks) and refer infants for neurobehavioral assessment and, even, early intervention.

My ultimate goals are to train researchers in biopsychosocial science, to advance nursing knowledge for improving breastfeeding outcomes in the U.S. and the world, to advocate for women, and to give infants a voice that they need support to learn how to breastfeed and other complex behaviors in the future.

My teaching goals begin with providing opportunities for my students to gain the skills to become lifelong learners, active listeners, critical thinkers, and communicators. My strategy is to involve each student to apply the content they are learning to their level of practice. I want students to demonstrate their ability, to explore their ideas and questions and find answers for their research or clinical question. As a teacher, I use the Socratic method of questions and answers throughout my lectures and interactions with students. This method engages students to ascertain their basic level of understanding and to scaffold their thinking into new knowledge. I use multiple approaches to encourage discussion and evaluation of problems until the concept has been explored and defined, and facilitated with peer-group learning. Beyond that, I want them to become adept at identifying reputable websites and new sources related to their practice. As their clients and patients approach them, they will have an understanding of the popular knowledge and have reputable resources for their patients and clients to use. Thirdly, as a teacher I want to cast a vision for my students to practice and conduct research with depth, breadth and empathy. For students in my lab, I want them to educate themselves about their topic with leveled (environment, behavior, cell, genetic) understanding, to innovative and address their problem of interest. Above all, I want build my students’ critical thinking skills that after they graduate, they are able to think for themselves to question status quo practice and provide them with confidence to apply best scientific research and health evidence to practice.

An effective method of teaching is by example. I am dedicated to involving students in research, so that they learn by experience. By students working in research, they understand the scientific process, the small steps of research that lead to discovery and gives them skills as a nursing leader. Each semester, I have between 8 to 10 undergraduate students working in my lab. My vision is for these students to participate in each step of the research process – beginning with ideas, performing a literature search, designing a study, collecting data, cleaning and analyzing data sets, analyzing the data and writing up the results. I plan my office time to coordinate when students will be on campus to support impromptu meetings, along with office hours, and scheduled meetings. Many students work in my lab for several semesters and we plan for them to present their work at the undergraduate research events, at conferences, and be co-authors on manuscripts.

In didactic teaching, I have demonstrated innovation in teaching and learning for two courses. In the undergraduate course, NURS 3444: Pediatrics, for which I was the faculty of record from 2014 to 2019, I created a novel assignment, which was funded by the University’s Center for Excellence in Teaching and Learning (CETL). Through the assignment, Seeing beyond childhood illness through film, students reflected on how contemporary films portray common subtle societal messages concerning sick, differently abled, or dying children. Such messages include that vulnerable children are objects to be acted upon, with no will or voice of their own. The innovation of the assignment was not the use of film, but rather, the disruption of common and pervasive societal messages to opening up in students the possibility moral insights concerning the children for whom they might one day be their nurses. Along these lines, I had students sing children’s

songs with me in class to address the after-lunch post-prandial slump and to give them a repertoire to use in the clinical setting. Over my time as the lead instructor, the students consistently demonstrated higher scores on the ATI standardized testing.

Another demonstration of innovation is in NURS 5811: Genetics, a graduate course, I am the first to use Genomic Nursing Concept Inventory (GNCI), a research instrument which assesses baseline and completion of the genetic and genomic knowledge. In Spring, 2020, I worked with the Center for Excellence in Teaching and Learning at UConn to conduct a Quality Matters assessment, the gold standard evaluation for online courses, to revise course objectives to be measurable and align with course assignments. Every semester, I ask a peer expert to evaluate my facilitation of the course material and online community. From the results of these innovations and feedback, I pivot content and apply evidence-based practices to facilitate our graduates to be clinical nurse leaders in genetics.

SERVICE______________________________________________________________________________ Service to School

As a member of the School of Nursing, I served by mentoring the Class of 2019 as a faculty advisor and as Faculty Counselor for Mu Chapter of Sigma Theta Tau, celebrating students as future colleagues. As a visionary for the advancement of the School of Nursing in research, innovation, clinical expertise and training future leaders, I have served on the Recruitment and Selection Committee, the Courses and Curriculum Committee, and the PhD admissions committee. Service to University

As a citizen of the University, I have built connections between multidisciplinary academia and the community. During my three years on the executive committee for the Collaboratory on Coordinated School and Child Health (CSCH), I reviewed internal seed grants, planned the community and university networking CSCH Encore Conference, and represented CSCH at Research Connections, a research networking event for students. As an UConn KIDS (Kids in Developmental Science) executive committee member, I interviewed and selected our Child Research Recruitment Coordinator who through community fairs and outreach events recruits families and children of Connecticut to participate in research studies at the university. In the last year, I serve as the School of Nursing’s representative on the Provost’s Library Advisory Committee. Service to Profession

As a nurse scientist at the University of Connecticut, I serve at several medical centers and on local, national, and international organizations. For four years, I held a dual role between the School of Nursing and as a nurse scientist at Connecticut Children’s Medical Center. I mentored nurses at the bedside in evidence-based research. During that time, the hematology/oncology research workgroup was funded by the Daisy Foundation to create and trial their End of Treatment survey, which is now being implemented in the clinical setting. Locally, I served on the research committee of the Connecticut Breastfeeding Coalition which facilitates breastfeeding within our state using policy and evidence-based research. I also served as the external nurse scientist on the Hartford Hospital Lactation Committee, during its re-designated as a Baby Friendly Hospital.

Nationally, I served on the research review committee for the American Nurse Foundation. I also served on the research advisory board for the Association of Women’s Health Obstetric and Neonatal Nursing (AWHONN) and collaborated on AWHONN’s comment to the HHS Healthy People 2030 objectives.

Internationally, I have reviewed for peer reviewed journals and abstracts for the 2018 International Society of Research in Human Milk and Lactation conference. Presently, I serve on the education committee for the International Society for Nurses in Genetics to identify and create genetic resources for nurse clinicians and educators. My future professional goal is to serve on AWHONN’s board of directors and on ISRHML’s international committee to advocate for women and children. In summary, I have faithfully served the school, the university, and the profession, and in so doing, I have built an international reputation.
