Preprinted Inserts $5,000 Preprinted Inserts - Will be placed inside publication’s poly bag. Maximum
size is 8½” × 11”, single sheet.
Trim Size - Publication trims ⅛” off top, bottom and outside edge. Final trim
size is 8½” × 11”. Live matter should be a minimum of ½” inside trimmed edges, and a minimum of ½” should be allowed for the bind.
Resolution - Image resolution for all graphics should be 350 dpi or greater.
No RGB or Indexed color mode graphics. It is recommended that you submit files 1 week prior to the published materials deadline.
Electronic Files - High-resolution PDF files are preferred. PDFs should be
created using the Press Optimized settings, and all fonts must be embedded. Please verify journal trim and bleeds. File must be set up for correct journal trim, and bleeds must be set at ⅛” beyond the trim.

Terms & Conditions
Advertising/Promotions Website advertising and company logos consist of a clickable image that links out to an advertiser’s website via CSA’s website (csahq.org). Promotional material for meetings is a one-page, 8.5”x11” flyer. Advertiser agrees to adhere to the CSA Advertising Policy. See ‘Artwork’ for size and file types of images accepted.
Advertising/Promotional Insertion Order Form A completed Order Form is required for placing website ads or logos on CSA’s website (csahq.org). Orders fromauthorized advertising agencies will substitute for this document. By signing an Order Form, advertisers agree to the CSA Advertising Terms & Conditions and Policy.
Eligibility CSA reserves the right to accept or refuse advertisers and/or materials, and require prepayment before listing an ad on its website. All advertising must be in accordance with the CSA Advertising Policy and adhere to the Advertising Terms & Conditions. CSA reserves the right to change its advertising terms and conditions at any time.
CSA does not accept advertising for credit cards/debit cards, insurance, or travel. The advertiser agrees to assume all liability for content of ads printed and must be fully authorized for use of the ad’s content, including, but not limited to, names, pictures, testimonials, and any copyrighted or trademarked material. In consideration of the publication of advertisements, the advertiser will indemnify and hold the publisher harmless from any loss or expense arising from an advertisement.
All claims for errors in advertisements must be made in writing and received within ten (10) days of publication and will be considered only for the first insertion of the advertisement containing the error. An error, which does not materially affect the value or content of an advertisement, will not qualify for a credit, nor will any credit be allowed if the customer has seen and approved a proof, or a proof was not supplied.
Payment Full payment is due along with submission of a completed Order Form. Payment must be received before ad or promotional items are placed. Rates are listed on Order Forms. Additional fees may be incurred if ads are submitted but not considered ready for distribution. Payment by check or credit card are accepted. Please make checks payable to: California Society of Anesthesiologists. Advertiser authorizes CSA to charge its credit card in the full amount owed, given a signed copy of an Order Form along with credit card information.
Cancellation A 30-day written notice is required for all cancellations prior to removal. A refund will be available in the pro-rated amount of the remaining balance less a 15% cancellation fee.
Artwork – Web Ads File types accepted: JPEG, PNG Max ad size: 100KB Deadline: Artwork is due along with submission of Insertion Order Form.
