Our Method of Services

One of our professional staff will meet with any of the following: family member, a doctor, lawyer, or who ever is in charge of the patient. He or she will review the patient's condition and types of services needed, then discuss our fee for service.

Medical Crew Plus Private Care is available to our clients 24 hours a day, seven days a week. Our services are often covered by Ameri Group, Ameri Choice, Medicaid, Medicare Waiver and Options, long-term care insurance, as well as on a private pay basis. We will gladly work with insurance providers to see if the requested services are covered.

Admission to Salvation Academy Home Health Agency can only be made under the direction of a physician, based upon the patient's identified care needs, homebound status and the type of services required that we can provide dire city or through coordination with other organizations.


CRITERIA FOR ADMISSION


CRITERIA FOR PERSONAL CAREOur personal care services does not need a physician's order and may include but are not limited to: Assistance with nutrition and diet activi ties such as shopping and

meal preparation; Performance of household services such as changing bed linens,

making beds, washing dishes, cleaning the kitchen, dusting and vacuuming rooms, shopping for essential supplies;

Assistance with basic personal care such as bathing, grooming, bathroom and/or bed pan routines, walking, transferring from bed to chair or wheelchair; and

Assistance with self-administration of medi cations. Complete or arrange for completion an assessment.

If we cannot meet your needs or your home environment will not support our services, we will not admit you or will not continue to provide services to you.

Salvation Academy Home Health Agency can provide a service, or a combination of services in your home - all under the direction of a physician, legal, guardian or representative, and family member. Working with your doctor, legal, guardian or representative, a family member our qualified staff will plan, coordinate and provide care tailored to your needs. Our services include:

We will accept Medicaid, Medicare assigned payment as payment in full for the services we provide as long as you meet the qualifying requirements and the services are covered by the Medicaid and or Medicare program.
