Policy for Issue of Duplicate Mark! Grade sheet! Degree Certificate

Read:(1) Resolution No. 06 - Governing Body Meeting dated 27th May 2019.(2) Resolution No. 09 - Board of Management Meeting dated 21st May 2019.(3) Resolution No. 03 - Academic Council Meeting dated 16th May 2019.(4) SPU/Acad/Duplicate Cert/2019/409 dated 18thApril 2019.

It is notified for information of all concerned that the Governing Body, in its meeting held on27th May 2019 vide Resolution No. 06, considering the recommendations of the Board ofManagement and Academic Council, has resolved to approve the norms for issue of duplicateGrade/Mark sheet/Degree Certificate in case of loss of the original certificate as attachedherewith to be effective from the Academic Year 2019-20 and onwards.

Procedure for Obtaining Duplicate Mark/Grade-sheet/Degree Certificate

The Applicant need to apply in the prescribed format to the Controller of Examinationsfurnishing the following details/information:

(a) An affidavit duly signed and notarized by Notary public/First Class Magistrate detailingthe circumstances under which the Original Marksheet/Degree certificate waslost/destroyed/misplaced. (as per the attached sample affidavit)

(b) A photocopy of the Marksheet/Degree certificate for which duplicate certificate isrequired, if available.

(c) Acopy of the FIRregistered in the Police Station for the loss of the OriginalMarksheet/Degree certificate.

'~ (d) Processing Fee @ Rs. 750/- and 1500/- for Marksheet and Degree certificaterespectively.

The Exam Office shall process the application for issue of the duplicate Marksheet/DegreeCertificate ensuring that the applicant receives the same within a month of the Application. Theword "Duplicate" will invariably be annotated on the top of the Marksheet/degree certificate

to be issued.

In view of the above, I request you to issue me a Duplicate Marksheet/Degree Certificate. I amalso attaching a Demand Draft of Rs. 750/1500 or proof of online payment towards theprocessing of the Duplicate Marksheet/Degree Certificate. The necessary details for the sameappended below:

Affidavit Format for loss of Mark/Grade-sheet/Degree Certificate(to be executed on non-judicial stamp paper of Rs. 50/-)

immediately submit the same to Sankalchand Patel University.
