Students who received notification of their acceptance must report to the J.A.C.C. (Supplemental instructions for enrollment for Summer Institutes, mini-courses, and workshops will be made available by the appropriate program directors.) If you wish to make any changes to your course schedule, you must obtain the approval of the departmental advisor.
All new or returning students who did not receive notification that all of their records (including transcripts) were completely processed, must report to the office of the director of the Summer Session, Room 310, Main Building (Golden Dome) before attempting to enroll.
3. Permission for late enrollment must be obtained in advance from the director of the Summer Session and will be granted under special circumstances only.
4. At the J.A.C.C., in exchange for the completed enrollment form, you will receive your copy of the enrollment form and your Summer Session "Student Class Schedule" which is to be used for identification.

NUMBERING OF COURSES Courses numbered 100 to 499 are undergraduate courses. Courses numbered 400 through 499 are advanced undergraduate courses which may be taken for graduate credit with the permission of the chairman of the department. Courses 500 and above are graduate courses.

Any additional "Drops," "Adds," or "Audits" may be processed through Friday, June 22, 1990, which is the. last date for changes without penalty. 1) Report r
to the office of the department chairman which offers the course you wish to "Drop," "Add," or "Audit," to obtain permission.
2) Take the approved form to the Office of Student Accounts (102 Main Building) to adjust your account. 3) Then go to the Office of the Registrar for final processing of the change.

SUMMER SESSION FINAL EXAMINATION SCHEDULE The last day of the Summer Session is Wednesday, August 1.

Your signed copy of the enrollment form and Summer Session "Student Class Schedule" will serve as your student identification for the Summer Session.
TRANSCRIPT REQUESTS
You may fill out a Transcript Request form in the Registrar's Office or send a letter to:

A request shall include your full name, dates of attendance, social security number, address of person, school, or company where the transcript is to be sent, your return address, and your signature. There is a charge of $2.00 for each transcript. A transcript may not be ordered by telephone.
You may send the request by FAX machine instead of mail. However, the transcript itself will be mailed. Our FAX number is (219) 239-5872.
Transcripts are normally mailed, but can be picked up in the Registrar's Office, two to three days after receipt
of the request Transcripts given directly to students arc stamped ISSUED TO STUDENT.
The University issues only transcripts of academic work taken at Notre Dame. Requests for transcripts of work taken at other institutions should be directed to those institutions.
Requests for transcripts will not be honored if you are financially indebted to the University.
In general, most evaluative information such as courses completed, grades earned, etc., cannot be issued to an outside party unless the student has given the Registrar permission to release it to that specific party. The Registrar may, however, disclose directory information without the student's stated permission. This includes full name, dates of attendance, certification of current or previous enrollment, full or part-time status, college/major or level, and degree(s) received.
