Executive Summary TheUniversityofWisconsin-StevensPoint(UWSP)isapublicUniversityservingtheeducationalneedsofresidentsinCentralandNorthernWisconsinthroughacommitmenttobuildinghealthy,engaged,andvibrantcommunities.In2014theUniversitybeganexploringgrowthofaDoctorofPhysicalTherapy(DPT)onitscampusinpartinresponsetotheconcernsfromlocalemployersunabletomeettherehabilitationneedsofourcommunities.TheUniversityexploredmanyoptionstogrowcapacitylocally,andultimatelychosetodevelopastand-alonephysicaltherapyprogramontheUWSPcampus.Thereisalocalworkforceneedthatrequirespartnershiptoaddress.SouthernWisconsinhas131licensedphysicaltherapistsforevery100,000residents;CentralandNorthernWisconsinhas72.CentralandNorthernWisconsinrepresentthemoreruralportionsofthestateandthepopulationisgenerallyolderandmanagingmoreanddifferenthealthconditionsthanoururbanareas.Asaresult,theemploymentdisparityhasalargerimpactthansimplenumberswouldproject.Allofthephysicaltherapisteducationprogramsinthestate(publicandprivate)arelocatedinsouthernWisconsin,andasstudentsoftenpracticenearwhethertheyattendedschool,ourmoreruralportionsofthestatecontinuetohaveworkforceneedsthatarepersistentlydifficulttofill.TheprogramatUWSPisintentionallydesignedtorecruit,support,andgraduateastudentinterestedinservingruralcommunitiesasahighlyqualifiedprimarycareprovider.Inconjunctionwithanadvisoryboardcomprisedoflocalandregionalemployers,educators,andpractitioners,theUWSPDPTprogramhasdevelopedamodelfortheDPTthatissustainable,innovativeandengagedwithourlocalcommunities.BecauseUWSPisapublicuniversitywithintheUniversityofWisconsinSystem,theprogramisaffordable,andwillbebuiltontheexistingstrengthinhealthscience,healthpromotionandbehaviorchangeforwhichUWSPisnationallyknown.TheprogramhastheaffirmedsupportoftheUniversitycommunity,theUniversityofWisconsinSystemBoardofRegents,andthelocal/regionalemployerandpractitionercommunities.TheproposedDPTprogramatUWSPwillmeettheneedsofemployers,addvaluetothemissionandvisionofUWSP,andprovidecosteffectiveaccesstophysicaltherapyeducationforresidentsofWisconsinandbeyondwhoarecommittedtomeetingtheneedsofourruralcommunities.

PROGRAM VISIONTo transform societal health through physical therapy education by

creating professionals who collaborate to serve the evolving, diverse needs of rural communities.

UNIVERSITY MISSIONThe DPT program extends the mission of UW-Stevens Point to build Healthy Communities by developing programs that emphasize the

health care and wellness needs of central and northern Wisconsin.

REGIONAL NEEDSThe Northcentral Wisconsin population is older, and with a greater incidence of cardiovascular, cerebrovascular, and neoplasm related diseases than those in southeastern Wisconsin, further exaggerating the rehabilitation imbalance in this part of the state.

Northcentral Wisconsin reported 72 licensed physical therapists per 100,000 residents compared to 131 per 100,000 in Southern Wisconsin.

The Wisconsin Department of Workforce Development projects an increase in employment for physical therapists statewide of 15% with a current projection of 277 open positions annually.

STRENGTH OF EXISTING RESOURCESThe College of Professional Studies School of Health Care Professions

has a 53% acceptance rate of students who apply to PT programs through PTCAS.

The College of Professional Studies offers degrees in Nursing, Athletic Training, Clinical Laboratory Sciences, Communication Sciences and

Disorders and Audiology. Adding Physical Therapy supports growth of health professions in an interprofessional environment.

CURRENT STUDENT DEMANDPhysical Therapy programs nationwide report 330 qualified applicants for a class size of 45 students. In Wisconsin, applicants have increased 2.4% in the last year alone, without an

increase in spaces available to support those students.

The University of Wisconsin-Stevens Point is the right place to build a new DPT program and is prepared to meet the needs of the residents, our community of learners, and the

profession.

Introduction

History of the University of Wisconsin-Stevens Point

TheUniversityofWisconsin-StevensPoint(UWSP)isapublicuniversity,partofthecomprehensivecampusesoftheUniversityofWisconsinSystem.Thecampus,locatedinStevensPoint,Wisconsinenrollsapproximately7500undergraduateandgraduatestudents.WhatisnowUWSP,beganasStevensPointNormalSchoolandenrolleditsfirstclassof201studentsin1894dedicatedtoteacherpreparation,domesticsciences,andconservationeducation.Today,UWSPoffersover100undergraduatemajorsintheCollegeofFineArtsandCommunication,theCollegeofLettersandSciences,theCollegeofNaturalResources,andtheCollegeofProfessionalStudies.TheDoctorofPhysicalTherapyProgramisproposedtobehousedintheCollegeofProfessionalStudies,SchoolofHealthCareProfessions.ThecampusislocatedinCentralWisconsin,andservestheCentralandNortherngeographicregionofWisconsin,whichisthemoreruralportionofthestate.

Planning for the Doctor of Physical Therapy

TheCollegeofProfessionalStudies,SchoolofHealthCareProfessionsbeganformalplanningforthestand-aloneentry-levelDoctorofPhysicalTherapy(DPT)in2017.ThisfollowedanextensiveexplorationofdevelopinganexpansionprogramontheUWSPcampusinpartnershipwitheithertheUniversityofWisconsin-MilwaukeeortheUniversityofWisconsin-Madison.Ultimately,expansionwasnotconsistentwiththemissionorvisionofeitherUWsystemschool,andadeterminationwasmadetoexploreastand-aloneprogramontheUWSPcampus.TheUniversitypreparedaNoticeofIntenttoPlantheDPT,submittedtotheUniversityofWisconsinSystemBoardofRegentsinDecember2017.TheNoticeofIntentiscirculatedtootherUW-Systemschoolsforcommentandfeedbacktoallowthoseorganizationsopposedtothisgrowthopportunitytorespond.Astheprogramreceivedunqualifiedsupport,theNoticeofIntentwasapprovedbytheBoardofRegentsinFebruary2018.IntheUWSystem,theNoticeofIntentisrequiredtocompleteamorefullyvettedneedsanalysisincludingananalysisofinternalandexternalresourcerequirements.TheNoticeofIntentallowsaninstitutiontoexploredevelopment,butdoesnotobligateaninstitutiontoexecuteaplan.

FollowingapprovaloftheNoticeofIntent,UWSPhiredKathrynZalewski,PT,PhD,MPAasanEducationalConsultanttotheSchoolofHealthCareProfessionswiththestatedobjectiveofcontinuingtoexploredevelopmentoftheDPTontheUWSPcampus.TheneedsassessmentfortheDPTprogramwasconductedinparalleltothepreparationoftheAuthorizationtoImplement,thenextstepintheapprovalprocessforprogramdevelopmentintheUWSystem.TheprocessbeganinMay2018,andconcludedtheformalassessmentprocessNovember11,2018.Theneedsassessmentgatheredinformationfromvariedconstituentgroupsincluding:

a) TheUniversitycommunitywhichincludesfacultyoftheCollegeofProfessionalStudies,SchoolofHealthCareProfessions,Universityleadershipandrepresentativesinfacultygovernance;

b) Thelocal(Central/NorthernWisconsin)andregional(stateofWisconsin)professionalcommunityincludingemployersandphysicaltherapypractitioners;

c) Thelocal,regional,andnationalemploymentworkforcedata

d) Theregionalandnationaladmissionsdataincludinginformationoncostofphysicaltherapyeducation,and;

e) Theregionalphysicaltherapyeducationcommunity.

Local and Regional • AnalysisfromtheStateofWisconsinDepartmentofWorkforceandDevelopmentfinds4755

physicaltherapistsemployedinthestatein2016(themostrecentnumbersavailableatthetimeofthisreport)withaprojectedemploymentof5480in2026(percentchange+15.25%).5Whenconsideringannualexits,annualtransfersandannualgrowth,the2016annualtotalopeningsforphysicaltherapistslocally/regionallywere277.

• TheWisconsinHospitalAssociationreportsthatthevacancyrateforPhysicalTherapistsstatewideis3.7%,with14.2%ofthepracticingphysicaltherapistsaged55andover.6Usinganattritionmodelof3.5%profession-wide,theAmericanPhysicalTherapyAssociation(APTA)projectsashortageofapproximately26,000licensedphysicaltherapistsby2025.7

• Statewide,Wisconsinhas103licensedphysicaltherapistsper100,000Wisconsinresidents,placingitinthemiddlethirdofthenationalavailablephysicaltherapyworkforce.8

• SouthernWisconsinhas131licensedphysicaltherapistsper100,000residentswhilecentralandnorthernWisconsinhave72physicaltherapistsper100,000residents.9

Enrol lment trends local/regional/national • TheCommissiononAccreditationinPhysicalTherapyEducation(CAPTE)reportsthatin2017

theplannedclasssizeforaDPTcohortwas45,andthatprogramsreported330qualifiedapplicantsforthese45positions.Forpublicuniversitiesonly,CAPTEreports368qualifiedapplicantsforaplannedclasssizeof42.Applicantshaveexceededavailablenationalcapacityforthelast10years,andthisdifferencehasincreasedineachofthepast5years.11

Program description and anticipated chal lenges to starting and sustaining the DPT program

Program Description

TheDPTprogramisanentry-levelgraduateprofessionaldegree.Theprogramcurriculumisplannedtoinclude112creditstocompletionoftheDoctorofPhysicalTherapydegree.Thedevelopingcurriculumistracked,andstudentsenrollasacohort,matriculatingthroughtheprogramineight(8)academicsemesters.Thecurriculumisaproposedcombinationofbasicscience,clinicalscienceandbehavioralsciencecourseswithintegratedclinicaleducationandsummaryfulltimeclinicallearningexperiences.StudentsareadvisedbytheDPTprogramfacultywhoarelicensedphysicaltherapists.Thecurriculumwillhaveopportunityforinter-professionaleducationemphasizingthecollaborationsnecessaryforhighimpactcareinruralenvironmentsincludingpartnershipswithathletictrainers,audiologists,nurses,nutrition/dieteticsspecialists,socialworkers,speechpathologists,andteachers–professionsofferedthroughUWSP.

AdmissiontotheDPTprogramwillprioritizestudentswhohaveexperiencewithruralcommunitiesandadesiretomeettheiruniqueneeds.ItisanticipatedthatthemajorityofthestudentswhoenrollwillberesidentsofWisconsin.Studentswillneedtomeetthepre-requisiterequirementsrecommendedbyCAPTEandtheAmericanCouncilofAcademicPhysicalTherapy(ACAPT),whichincludecompletionofabachelor’sdegreepriortomatriculatingintotheprogramandcompletionofprerequisitecourserequirements.TheprogramwillprocessadmissionsthroughthePhysicalTherapyCentralizedApplicationService(PTCAS).

Universitymissionfit,infrastructure,andstudentresourcesarewelldevelopedtosupportthegrowthoftheDPT.Primaryareasforcontinueddevelopmentinclude:(1)operatingbudget,(2)space,(3)faculty,and(4)clinicaleducationresources.Eachoftheseareaswillbeaddressedindividually.

Operating Budget

TheDPTwillbedeliveredasacost-recoveryprogramatUWSP,meaningthatUWSPwillchargestudentstakinggraduatecoursesintheDoctorofPhysicalTherapyProgramarateof$463percredit.SegregatedfeeswhichcoverUniversityservicesincludinghealthcare,willbechargedforyears1and2ofthetrackedcurriculum,butwillnotbecollectedduringthethirdtermwhenstudentswillbeprimarilyoffsiteparticipatingintheirterminalclinicalexperiencesfull-time.Theprogramwillcollecta$50/credit

distanceeducationfeeforthosecoursesofferedentirelyon-line,butwillnotchargestudentsthisfeeforcreditsassociatedwiththeirclinicaleducation.Studentswillpayanon-refundableprogramadmissiondepositonceanadmissiondecisionhasbeenmadebytheAdmissionsCommittee.Forstudentswhomatriculate,thesefeeswillbebudgetedtosupportengagementinprofessionalorganizationsincludingconferenceattendanceandtravelduringtheirtenureintheprogram.Thus,thetotalcostforthisprogrambasedonratesofthe2018-19academicyear,includingsegregatedfees,programfees,andtuition,is$55,064.74.Ofthis,theprogramwillcontribute16%ofnetrevenuetotheinstitution.

Space

TheCollegeofProfessionalStudiesisdevelopingspacerecentlyvacatedbytheBiologicalSciencesfaculty.Approximately10,000squarefeetofspaceincludingfourinstructionallabsofapproximately1600squarefeeteach,7offices,reception,aworkroomandstoragewillberenovated.Thespaceplanisintheapprovalprocessandarenovationbidisindevelopment.Budgetforspaceandequipmentrenovationwillcomefromexternaldonors,theStateofWisconsin,andbudgetedmoniesfromtheUniversity.Spaceplansasdescribedmeettheinstructionalneeds,programoutcomes,andfacultyoutcomesfortheDPTprogram.

Faculty

TheDPTprogramhasapprovalfromtheUniversitytohire7faculty/instructionalacademicstaff(7FTE)onceapprovalfromtheBoardofRegentsisreceived.HiringtimelinesarebasedonthetimingofCAPTEaccreditation,butwilllikelybegininthe2019calendaryear.Dr.KathrynZalewski,PT,PhD,MPA,consultanttotheDPTprogram,willbeengagedwiththehiringprocess,understandsthequalificationsofindividualsnecessarytosupporttheprogram,andwillassisttheUniversitywithbuildingaqualifiedfaculty.

TheoperatingbudgetfortheDPTprogramincludesrevenuestosupportfacultygrowthandprofessionaldevelopment.Locally,thereisaqualifiedandinterestedclinicalcommunitywhowillsupportpart-timeinstructionincontentneeds,labassistantneeds,andclinicalinstructionneedsfortheDPTprogram.Non-clinicalfacultyincludingthoseinbiologicalsciences,exercisephysiology,pharmacology,behavioralandpublichealthalreadyexistoncampusandwillbeutilizedtosupportthedevelopingprogram.Theuniversitywillrecruittalentedfacultytocomplementthisexistingresource.

Clinical Education Resources

PreliminarydiscussionswiththeclinicalcommunityhavesupportedthegrowthoftheclinicaleducationresourcesnecessarytomeettheDPTprogramvision.BecauseUWSPispartoftheUniversityofWisconsinSystem,contractsalreadyexecutedbyUWSPandotherUWSystemprogramsareavailabletoextendtotheDPTprogramontheUWSPcampuswithamemorandumofagreementtoclarifyresourcesnecessarytosupportclinicaleducation.PreliminaryconversationswiththeDPTAdvisoryBoardandearlyplanningparticipantsoutlinedintheprevioussectionaffirminterestinparticipatingintheDPTprogram.

TheinitialcurriculumvisionfortheDPTwouldinclude30-weeksoffulltimeclinicaleducationatthecompletionofthedidacticcourseworkcoupledwithintegratedclinicaleducationexperiencesbeginninginthefirstsemesteroftheDPTprogram.Theseintegratedexperienceswillbedeliveredinpartnershipwithlocalandregionalproviders,andwillincludethedevelopmentofexperiencesthatcomplementtheneedsofthelocalcommunityincludingthedevelopmentofpost-rehabilitationactivityprogramsthatprovidesupportedexerciseaspartoftherecoveryprocess.

WiththecurrentsupportevidencedtotheUWSPDPTprogram,theUniversityisconfidentintheabilitytobuildtheclinicaleducationresourcesnecessarytosupporttheneedsoftheprogram.

Summary Assistingcommunitiestobecomemorevibrant,healthy,prosperous,andsustainableisthefoundationoftheUWSP’sstrategicplan,the“PartnershipforThrivingCommunities”.13TheproposedDPTreflectstheUniversity’sgoaltopartnertoimprovethehealthoflocalcommunitiesandwasdesignedinclosepartnershipwithlocalphysicaltherapyeducators,providersandemployers.TheDPTatUW-StevensPointwillcontributedirectlytothemissionoftheUWSystembyengagingwithourcommunitiestosupportthehumanresourceneedsofourhealthsystems,bydevelopingstudentsandfacultycommittedtohealth-relatedresearch,andbyextendingresearchoutcomesbeyondtheuniversitythroughapplicationanddisseminationofresearchfindingswhichcancontributetoimprovedhealthoutcomesofourcommunities.

Themarketdemandforphysicaltherapistscontinuestoremainstronglocally,regionallyandnationally.TheneedincentralandnorthernWisconsinishallmarkedbyamuchloweravailabilityofaphysicaltherapyworkforcerelativetootherpartsofthestate.Anumberoffactorscouldinfluencethisdisparity;however,weobservetheexistingphysicaltherapyeducationprogramsinthestateareclusteredinsouthernWisconsin.Highdemandfromanagingpopulation,whoarelivinglonger,aremoreactiveinlife,andmorelikelytohavemobilityneedscoupledwitharegionalimbalanceinaccesstoresourcesresultsinongoingdifficultiesforresidentsofcentralandnorthernWisconsintoaccessneededcare.

TheUniversityofWisconsin-StevensPointhasnotentertainedthisgrowthlightly.PlanningandresourcedevelopmenttobuildtheprogramthatmeetstheneedsofthiscommunityhasbeenpartofthestrategicdevelopmentoftheCollegeofProfessionalStudiessince2014.Needsofourlocalcommunity,resourcesavailabletomeetthoseneeds,andthesupportofthelocalandregionalpracticecommunitycombinetoassurethatthisisgoodgrowth,attherighttime,andintherightlocation.TheDPTatUW-StevensPointwillcontinuetoserveourcommunitiesandtheprofession.
