n March we hosted the 12th Annual SCMS Health Fair ldquoThe Doctor Is Inrdquo with a goal of promoting Wellness in the Saginaw County population

(see pages 16-19) The Health Fair definitely had a fun energizing feel and was attended by 1000 local residents despite being held the same weekend as the Home Builders Show at SVSUrsquos Ryder Center Twenty-nine physicians were on hand to represent personal passions and professional interests sharing information alongside many of our partners in health care My personal highlights were seeing my patients physicians and allied health partners and what they chose to promote at the time I also enjoyed seeing the CMU medical students taking blood pressures and interacting with their patients peers and mentors A big thanks to Kiron Choudhri at Horizons Joan and Jamie as well as their families for all the work done to make the event a success again this year In April we continue to pursue wellness in Saginaw County now in ourselves We will be hosting the Midland County Medical Society and MidMichigan Physicians as well as inviting our spouses to join us for our educational program featuring Sinai-Grace Psychiatrist Gerald Shiener MD who will be speaking on Physician WellnessBurnout The difference between stress and burnout is the ability to recover in your time off When you are not able

to recharge between work hours ldquohellipan erosion of the soul caused by a deterioration of onersquos values dignity spirit and willrdquohellip can occur according to Christina Maslach and her colleagues developers of the Maslach Burnout Inventory (MBI) tool We expect our profession to come with inherent stress The health care system is increasingly applying system related stress via credentials and MOC EMR and documentation We are asked to make decisions and advise patients that are sick worried or in pain in intense restricted increasingly brief encounters where the documenting can threaten to impair your patient physician relationship when done in real time or consume your personal time if done after the patients are done I read an article that referred to the white-walled exam room being more like a black box with the closed door of an office exam room cutting you off from your team creating a pressure to complete the visit and get back out where you can be accessed

The same article referred to the physician as the rate limiting step in the system of providing services Those stresses are not inherent they are external brought to bear by the system in which we provide care The burnout rate for physicians has climbed from 45 percent in 2011 to 54 percent in 2014 More requirements less reimbursement and messages that we are too slow expensive thorough etc are squeezing the joy right out of our practices Lots of time and study has gone into assessing personal risk for burnout as well as clinical depression and suicide Strategies and tools for developing personal plans to avoid burnout and maintain engagement are becoming increasingly available Come bring your spouse and join us at 630 pm on Tuesday April 19 at Horizons Conference Center and letrsquos see if Dr Shiener can help us to be well avoid physician burnout and find ways to keep joy infused in to our practices Remember to call the SCMS with your reservation at 790-3590

as this ever happened to you Yoursquore visiting one of the states along the southern tier of the country and you

have occasion to tell one of the inhabitants that you are from Michigan to which they invariably say ldquoOh itrsquos cooold up thererdquo When it happens to me and unwilling as I am to reduce the essence of our great state to a figure on a thermometer I usually reply along the lines of ldquoYes but itrsquos also one of the most beautiful areas on the planetrdquo Lately however I have tried to actually give them a feeling of what itrsquos like to live in our area ldquoYes but I live in a community full of the most kind generous giving people you could ever meetrdquo We share Saginaw shares and it shows Of course the Dalai Lama is correct sharing is key and itrsquos sharing that makes us a great community and a great state Itrsquos not temperature certainly Nor is it wealth or scenery or location or natural resources Our legacy is what we do for our community Your legacy is everything you do for your community Here are a few (out of many) people I find outstanding in the sharing department

bull Samantha McKenzie President and CEO of Hidden Harvest whose energy and enthusiasm motivate an organization that moves thousands of tons of food that would otherwise be wasted into the hands of those who need it

bull Jessie Wilson of the East Side Soup Kitchen who tells us doctors the ins and outs of preparing and serving food when we work there (yes doctors are capable of cutting up vegetables) Thanks to her the staff and ESSK Director Pam Cole who has served for over 22 years hundreds of needy people get a decent meal day in and day out come rain or shine Saginawians can depend on her for their next meal

bull Roger (he like Madonna goes by one name and everybody knows who you are talking about) is a retired school teacher who it seems to me

volunteers extensively You see this guy everywhere there is a need He especially loves the Mid- Michigan Childrenrsquos Museum and Saginaw Zoo

bull Past President Eric Gilbertson and the Board of Directors of Saginaw Valley State University who made the decision some years ago to allocate considerable school resources to community education and enrichment The school has since made a major impact on the intellectual and cultural life of our area I especially enjoy their Osher Lifelong Learning Institute where I learned how to use email and Facebook

A full list of the wonderful sharing people who give their time and talents to benefit our community would take many many pages It would include of course most of you reading this article When we all share what we have with our community we create a rich legacy an extension of our own lives throughout the geographical

area and into the future When we help someone become a better healthier smarter person we also like ripples in a pond influence the development of their friends family and children who can carry the legacy forward to the next generation and on and on Sharing is critically important to the development of any community but itrsquos not a given There are many many

communities across the country that appear to ldquohave it allrdquo and superficially may look better than Saginaw in some wayshellip milder climates more generous employers or more name recognition But when you look beneath the surface they lack the altruism we have here They are the poorer for it So the next time someone from another state says ldquoits cold up thererdquo feel free to hit them right back with the reasons why you believe Saginaw is WARM Red-Hot with caring and sharing

All Saginaw County Medical Society members are also Saginaw County Medical Society Foundation members In that regard the Foundation Board would like the opinion of SCMS members on how best to use Foundation funds Article I Section 2 of the Foundation Bylaws currently state Section 2 Powers and Duties The powers of the Board of Trustees shall include but shall not be limited to the following

A To provide or guarantee loans grants and scholarships to deserving medical students para medical students and nurses (or students pursuing any scientific health career)

B To assist charitable or educational institutions not operated for profit whether supported by private donations or public taxation in the promotion development and attainment of health careers

C To promote the science and art of medicine and the betterment of public health through exhibits and other programs of education and advisory service to the public on matters of health and hygiene

D To promote scientific or medical research for the purpose of alleviating human suffering

The Foundation currently grants low interest loans to medical students with ties to Saginaw and forgives all interest if the recipient returns to Saginaw to practice upon completion of their residency The Foundation also hosts an annual Essay Contest for high school juniors and seniors who are interested in becoming a physician in Saginaw County The Board would like your ideas for other ways the Foundation could support health care in Saginaw

The Foundation currently provides the following

bull Assists the SCMS Alliance with nursing scholarships bull Essay Contest for high school juniors and seniors interested in becoming a physician

bull Loans to medical students with ties to Saginaw

7th Annual Covenant Kids Telethon to Air Live from Mid-Michigan Childrenrsquos Museum Raising Funds for Pediatric Services

Easing the anxiety of a child during an MRI Educating a preteen and her family about how to deal with diabetes Providing comfort to toddlers in the emergency care center Giving a voice to adolescents with speech limitations These accomplishments are happening today for kids across the region because of Covenant Kids and the wonderful businesses organizations and individuals who support this worthy cause

Another great thing about Covenant Kids Every penny of every gift to the cause goes directly to support medical equipment and programs for pediatric patients - no administrative costs taken out - and the money stays local To highlight the Covenant HealthCare Foundationrsquos fundraising efforts for Covenant Kids Covenant HealthCare and WNEM TV5 have partnered to present the 7th Annual Covenant Kids Telethon from noon until 6 pm on Saturday April 23 2016 airing live from the Mid-Michigan Childrenrsquos Museum in Saginaw The Telethon will be hosted by WNEM TV5 personalities and Covenant employees featuring stories of local Covenant Kids (past pediatric patients) and their families Presenters will showcase equipment and projects made possible via previous donations to Covenant Kids

This year the community has the opportunity to be involved with the Telethon

The 12th Annual FREE Saginaw County Medical Society Health Fair ldquoThe Doctor Is Inrdquo held on Saturday March 19 2016 at Horizons Conference Center in Saginaw was another success The SCMS planned this great event in cooperation with CMU Health Covenant HealthCare Health Delivery Inc Michigan CardioVascular Institute St Maryrsquos of Michigan and the Saginaw County Osteopathic Society Over 1000 people attended this yearrsquos event and visited 71 vendor booths Numerous SCMS physicians residents and medical students participated in the event as well During the Health Fair vendors performed screenings for balance blood pressure body mass index cholesterol depression diabetic foot exams glaucoma glucose hearing lung function oxygen level posture skin cancer sleep apnea and stroke The SCMS had very positive comments from vendors and participants on everything from location to the numerous screenings that were available Many SCMS members attended the event and either answered questions at the SCMS booth or just made themselves available for questions during the Health Fair Both the vendors and participants commented how nice it was to have doctors available for questions Non-perishable food items for the East Side Soup Kitchen were also collected from those attending The Saginaw County Health Department and Sheriff rsquos Department collected 110 pounds of old unused or unwanted medications Unfortunately the blood drive was cancelled because the Michigan Blood Bus broke down

With physician burnout at 544 percent nationwide according to a recent study the medical world needs solutions nowmdashand the Mayo Clinic is pioneering a model designed to raise camaraderie and increase collaboration to reduce burnout among its physicians Find out why this health system started treating physicians as architects in practice rather than construction workers

A physician-led solutionStephen Swenson MD medical director of the Office of Leadership and Organization Development at the Mayo Clinic and professor of radiology at the Mayo Medical School worked with his colleagues to develop the Listen-Act-Develop model that focuses on physician engagement as a strategy to reduce burnout and involve physicians in the mission of their organization

ldquoIf you have seen the causes of burnout in one unit you have seen the causes of burnout in one unitrdquo Dr Swenson said ldquoThey are unique and variablerdquo

The first staple of the model is establishing a formal listening forum to hear from physicians Dr Swensonrsquos group made sure physicians were in a psychologically safe setting while they discussed top pain points physicians identified such as clerical work process inefficiencies and ways for clinicians to have some control of their daily and weekly schedules

What was ldquoimportant was the simple act of caring and listening and then working togetherrdquo Dr Swenson said This is what he calls participative management ldquoResponse [from physicians] was positive and hopefulrdquo he said ldquoIt is critical once expectations are raised with a survey or focus group to

really and authentically follow through Otherwiserdquo he added ldquo[it] could actually make the situation worserdquo

The Listen-Act-Develop model considers three factors that physicians need to flourish

bull Choice Physicians want to have some control over their lives This comes with granting certain levels of flexibility and placing genuine value on physician input in the process

Organizations can increase flexibility and control for physicians by treating them as ldquoarchitectsrdquo in the design of their care delivery model and not ldquoconstruction workersrdquo who follow someone elsersquos plans Dr Swenson said

bull Camaraderie or social connectedness Taking the time to socialize with team members and colleagues can lift spirits and improve collaboration

ldquoWe led two randomized controlled studies with docsrdquo Dr Swenson said ldquoBoth showed that simply getting together for a meeting or a meal raised camaraderie and lowered markers of burnoutrdquo

ldquoThe teamwork involved in addressing the local drivers of burnout is also a vehicle for growing camaraderierdquo he added

bull Excellence Everyone wants to be a part of something meaningful Organization leaders should establish constructive relationships with physicians and have a ldquogenuine conversation [with physicians] to understand life in their moccasinsrdquo Dr Swenson said ldquoAnd then [create] a partnership to address the opportunities at the frontline and organizational levelsrdquo

ldquoIf [physicians] are treated as employees or cost centers that is how they will behaverdquo he said ldquoIf they are treated as partners in delivering the needs of patients [physicians] will ignore their job descriptions and skyrocket discretionary effortrdquo

CMU Health in Saginaw to Welcome 31 New Resident Physicians

National program matches prospective physicians with residency positions

Each year as they near graduation from medical school thousands of prospective physicians eagerly await results of the National Resident Matching Program (NRMP) excited to learn where they will train for the next three to seven years This process known as ldquoMatch Weekrdquo pairs students with residency programs around the country using an algorithm based on ranked preferences by both the students and programs This year residency programs in Saginaw at CMU Health matched all 31 available spots on the first day of Match Week CMU Health is a partnership between the CMU College of Medicine Covenant HealthCare and St Maryrsquos of Michigan that jointly manages residency programs in Emergency Medicine Family Medicine Internal Medicine OBGYN and Psychiatry in addition to an Emergency Medical Services Fellowship ldquoWe are very pleased that each program matched with so many applicants with ties to our local areardquo said Bethany Figg Graduate Medical Education Administrator ldquoAnd we are extremely excited about the caliber and diversity of students who will be joining us as new residents in Saginawrdquo The new resident physicians will begin their training in Saginaw on July 1 and bring with them a variety of backgrounds and assortment of life experiences including four students from medical schools in Michigan and six from states as far away as Kansas and Florida More than a third of the incoming residents previously trained as students in the Saginaw hospitals

The Impact of Physician Documentation A New Benefit for MSMS Members Physicians may not understand the true impact their documentation has on the reimbursement for services they render Documentation is the written record of what has occurred and physicians must tell the story of the encounter through the documentation in the patientrsquos medical record If it isnrsquot documented it wasnt done Physician documentation is like a story problem and the codes submitted on the claim form are the answer to the story problem Documentation must equal the codes submitted Health plans may perform routine pre- or post-payment reviews of submitted claims The medical record documentation is scrutinized to be sure the physician documented what was billed MSMS developed a new Coding Alert titled The Impact of Medical Record Documentation and Coding on Reimbursement This new alert highlights the importance of physician documentation which highlights documentation health plans look at how medical record documentation impacts reimbursement and newly introduced EampM services which may increase revenue opportunity To download the new Coding Alert please click here (member login required)

MSMS Board Talks Physicians Sustainability Medicaid at MeetingIn March the MSMS Board of Directors met to address the upcoming House of Delegates physician sustainability Medicaid and more Below are some of the highlightsbull The Executive Council of Physician Organizations continues to grow with the addition of three new physician organizations Sparrow Care Network The Physician Alliance and United Physicians Representation now stands at more than 7500 membersbull Medicaid has asked for assistance from MSMS to

Physicians often strive for ldquowork-life balancerdquo but how do you define it Getting the time you need may require a different approach One physician and wellness expert recommends these four self-driven solutions to help you redefine balance and maintain a successful home and work life Family physician Sara Taylor MD has learned valuable lessons of her own as a mother of two teenaged children practicing physician part-time practice owner blogger and wife of a fellow physician When shersquos not balancing this full roster she writes about physician wellness personal development and social media She recently shared some of her insights in this monthrsquos issue of AMA Alliance Publication Physician Family Magazine Here are some of the ways shersquos learned to sort work-life and home-life into two balanced productive spheres 1 Prioritize what you value and plan for it Early on Dr Taylor took a valuable lesson from Stephen Coveyrsquos book First Things First ldquoMy take-away message from this invaluable book is that if we spend more time dealing with things that are important such as planning preparing and personal development we spend less time in the lsquocrisisdeadlinersquo mode that lends itself to feeling frenzied and imbalancedrdquo she wrote Dr Taylor also noted that ldquoCovey cleverly describes urgency addiction as lsquoa self-destructive behavior that temporarily fills the void created by unmet needsrsquordquo Focusing on weekly goals is one way to effectively prioritize ldquoThis does not mean to keep an exhaustive to-do list (like I used to) but rather to write down three lsquomust-dosrsquo each dayrdquo she wrote

ldquoYou can either do this for work-life and home-life separately or as I do approach it more globallyrdquo 2 Still give yourself time to ldquojust berdquo and feel rooted in the moment ldquoIn the book An Eight-Week Plan for Finding Peace in a Frantic World Dr Mark Williams and Danny Penman describe our common state of lsquodoing modersquo versus lsquobeing modersquordquo Dr Taylor wrote Being in a constant state of ldquolsquodoingrsquo ends up depleting us leading to exhaustion and burnoutrdquo she wrote ldquoBy consciously attending to lsquobeingrsquo or mindfulness we are more able to give both our body and mind what they need to be nourishedrdquo One tip she recommended Try meditating to help practice mindfulness and channel a sense of calm 3 Learn to say ldquonordquo to certain tasks ldquoIt not only helps us set boundaries but also prevents us from falling prey to other peoplersquos agendasrdquo Dr Taylor wrote She said saying ldquonordquo is an essential step to finding balance and ensuring you have time to prioritize your well-being 4 Practice self-care focusing on small actionable steps Start by exercising getting proper sleep and setting digital limits from work or peers so you can properly rejuvenate Dr Taylor said If you donrsquot have time for yoga classes or the gym online exercise classes or instruction can be other options Get more solutions and insights from Dr Taylor in this monthrsquos issue

BOARD HIGHLIGHTS The February meeting of the Board of Directors of the Saginaw County Medical Society was held on Tuesday February 17 2016 at CMU Health in Saginaw Danielle C Duncan MD President called the meeting to order at 535 pm The minutes of the January 19 2016 Board meeting were reviewed MOTION Approve the January Board Meeting Minutes MOTION APPROVED Dr Veverka presented the Treasurerrsquos Report for January 2016 MOTION Approve the January Treasurerrsquos Report MOTION APPROVED Dr Veverka then gave an explanation and answered questions of the Board regarding SCMS Equity from the January meeting

OLD BUSINESS1 CMU College of Medicine ndash Dr Vance gave an update on the CMU College of Medicine He said they are preparing for the increase from 26 to 52 medical students in July Combined with the current students there will be over 100 in Saginaw in 2016 and 120-130 in 2017 CMU continues to need Community Educator Faculty for Family Medicine Internal Medicine and Pediatrics There is also a need for elective opportunitiespreceptors for primary care and subspecialties Dr Vance indicated the three visions of CMU are Educational Clinical and Research2 Update on Status of Resolution to MSMS ndash Dr Willy briefly mentioned she was continuing work on a resolution to the MSMS House of Delegates and ultimately the AMA to ask for a study on how physicians will provide services to baby boomers in the future what resources will be required etc She was unsure if she will have the resolution prepared for submission before the deadline3 Health Fair ndash Board members were asked to sign up for one-hour shifts to staff the SCMS booth Dr Vance indicated he could act as attending for the CMU medical

students and residents if necessary Last year members staffing our booth served as the attending4 MSMS Community Service Award ndash At the January board meeting both Drs Farhad Shokoohi and KP Karunakaran were nominated to receive this yearrsquos MSMS Community Service Award MSMS has confirmed both physicians will receive the Award this year5 Nominating Committee ndash Dr Duncan asked for suggestions of physicians to serve on the SCMS Board in the future All attending were given a Position Interest Form they were asked to complete and return to Joan

NEW BUSINESS1 Application for Membership ndash Dr Caroline Scott Interim Secretary presented the following applicant for his first reading (a) Thomas J Campana MD Aleda E Lutz VA Medical Center General Surgery - board certified in 1993 sponsored by Drs Anthony deBari and George J Gugino2 Rescue Mission ndash Dr Ramani stated she was involved in a program to sponsor birthday gifts for residents of the Rescue Mission and would like to invite others to participate She was asked to forward information to Joan for inclusion in the next Bulletin

The next meeting of the Board will be held on Tuesday March 15 2016 in Room 1016 at CMU Health 1632 Stone Street in Saginaw There is no membership meeting in March The meeting was adjourned at 648 pm
