FOREWORD Background DOMS stands for ‘Distribution Operation & Maintenance Strategy’ and is being used by Scottish Water to:

safeguard the quality of water to customers ensure that Scottish Water apply due diligence to their Scottish Water distribution

network The DOMS procedures and work instructions affect all functional staff who work on the network, which includes internal employees and employees of external organisations (i.e. planners, operators, delivery teams, Framework Contractors, or any other contractors). Through the application of the DOMS principles in day-to-day network activities, Scottish Water and their approved contractors will operate and maintain their water distribution networks through operational ‘best practice’. This will allow Scottish Water to meet the following objectives:

to maintain water quality; to maintain continuity of supply; to maintain water supply hygiene; to undertake water quality incident management; to maintain a cost-effective water supply. to ensure a course of action for timely network intervention (Capex and/or opex)

to maintain statutory obligations and customer satisfaction. Responsibility The responsibility for adoption and implementation of all DOMS procedures and work instructions rest with Scottish Water General Managers for Scottish Water staff and senior managers of external organisations. It is the responsibility of all Scottish Water employees and those of external organisations to comply with DOMS procedures and work instructions.

3. DESCRIPTION

• The local Customer Service Delivery Water Operations Team & or Leakage Delivery Teams will manage the interventions on the operational water network, which they manage and control through DOMS IAF Impact Assessment Forms.

• The majority of the day-to day operational interventions will be general maintenance and minor new works

e.g. burst repairs and new house connections. These tasks will be assessed and managed by appointed Customer Service Delivery Water Operations Team & or Leakage Delivery Teams and managed via Click.Only those jobs, which are deemed to have a high or critical impact on the Network, will be individually authorised by the Team Leader and or Regional Manager. In those instances, where a WOA or WSZ boundary valve has to be operated the Regional Manager or his designated staff must authorise the work.

• Capital projects will be authorised by the Customer service Deliver Water Operations Team Leader or

Regional Manager or their designated representative. • External Contractors Access / Transfer Certificates must be issued to Contractors, who require access or

temporary control of an operational site e.g. Service Reservoir or Pumping Station. − Refer to: Health and Safety Arrangements for External Contractors Access Certificate, (OHS/HAS/13). − The following categories do not require an Access Transfer Certificate:

∗ Authorised SW personnel from other locations ∗ Authorised visits by members of the public. ∗ Visits by Statutory Authorities. ∗ External contractors visiting the site on a routine basis e.g. security firm, where a pre-determined

operating procedure has been established. ∗ Roads and Streetworks

IMPORTANT CHANGES TO THE HYGIENE CODE OF PRACTICE & DOMS PROCEDURES – RESTRICTED ACCESS AREA DEFINITION.

Restricted Areas - are those where direct contact may be made with partially or fully treated water within water treatment works or with treated water at any stage of its distribution to the point where it is made available to customers and areas where there is no direct contact with water but where a high degree of hygiene is still required. Examples of Restricted Areas are – Filter tanks

Contact tanks

Service reservoirs

Pumping stations when any surfaces normally in contact with treated water are exposed

Surge vessels

Tanks associated with waterworks sludge treatment where water is re-cycled to the works

Excavations where the internal surfaces of existing mains/services (and new mains tie-ins) are exposed to atmosphere and/or there is any possibility of a contaminant entering the system.

Any other area where treated or partially treated water is exposed to the atmosphere Boreholes

Areas where there is no direct contact with water but where a high degree of hygiene is still required e.g. filter walkways around tanks, valve galleries and chemical treatment plants.

For short term, occasional visits by external agencies (telephone companies, non-intrusive surveys by SWS staff etc) Access Certificates, including an associated job specific risk assessment, carried out by the nominated Responsible Person, must be issued for every job. All access by external agencies should be subject to Scottish Waters’ Security Policy.

Any person working on hatches, cabling or instrumentation associated with Restricted Areas must be subject to Access Certificates, risk assessed by the nominated Scottish Water Responsible Person and ideally supervised whilst the work is being carried out. Where these jobs have the potential or actual contact with treated or partially treated water, then the person/s working on the job must hold the appropriate Authorisation Card and have had DOMS / HCoP training.

. Non - Restricted Areas - are those areas where no direct contact may be made with partially or fully treated water, e.g. control panels within pumping stations, the grounds, offices, and toilet facilities at water installations, etc. It is not a requirement to have an Authorisation Card or DOMS / HCoP training to access these areas.

Visitors, who are not expected to carry out work that could potentially affect the quality of the water supply, are permitted access to Restricted and None Restricted Areas provided that: (a) They are properly supervised by a competent Scottish Water employee (b) Arrangements are implemented to ensure that the recommendations of the Hygiene Code of Practice are not compromised; including a risk assessment of any proposed activity is carried out by the nominated Responsible Person. If there is a need for further information or assistance with interpretation of Restricted Area Access, contact the

relevant operational area Public Health Team who will provide further guidance. If guidance is required outside of office hours the Public Health Team are on call 24 hours / 7 days a week and contact details are available though the Operational Management Centre and / or the Corporate Satellite System Standby Log.

DOMS Training Guidelines – Evidence of DOMS Training

In 2007, Scottish Water set the goal that all staff working on the water network receives the relevant DOMS training by the end of the year. The result - over 9,000 people have currently completed DOMS training to support their network activities since it has been introduced within SW.

All staff, both internal and external, who work on the water network, MUST complete the relevant Distribution, Operation and Maintenance Strategy (DOMS) training associated with their specific network activities and as a result MUST carry two cards which confirm their training.

DOMS training is available through a number of approved Training Providers and the registration of the training units taken by individuals is administered through EU Skills.

The first card relates to training given around the Hygiene Code of Practice (HCoP) and the card usually coloured light blue, confirms the cardholder has completed this element, which includes a health screening questionnaire.

The second card, usually coloured white, relates to training given around DOMS and the training units shown on the back of the card detail the type of DOMS related site activities that the cardholder can undertake.

Both cards detail the name of the cardholder, along with their picture, which will allow confirmation that the cardholder is the person holding the card.

Scottish Water staff may and will challenge all internal and external staff working on the water network at anytime to produce both their cards to ensure that cardholder has the relevant training and understanding of DOMS Procedures and Work Instruction for the specific task that they are undertaking.

The following guidelines should be followed for deciding the course of action to take when either internal of external staff were found to be working on the water network, or any infrastructure that will be connected to our network, without the requisite HCoP/DOMS cards confirming training

1) New starts - expectation that they will have their necessary HCoP/DOMS training organised by their employer within the first 4 weeks of them joining their company. They will not be allowed to work on the water network without the HCoP/DOMS training, even though they are being supervised by a person who has gone through the training. There is a minimal health risk here, which the health questionnaire, completed during the HCoP training, will confirm as acceptable or unacceptable.

2) Completed HCoP/DOMS Training but no card issued by EU Skills - staff in this situation can work on the water network as long as they can demonstrate that they have a 'cover note' issued by training providers. If a 'cover note' is produced, then the person challenging can ensure that 'cover note' refers to the actual person by checking the EU Skills web site against name or reference number.

This will be done in conjunction with the relevant DOMS Working Group representative.

3) Person challenged on site and person has no HCoP/DOMS relevant training - if this scenario occurs then the person who has made the challenge will remove the person from working on the water network and contact the relevant business unit DOMS Working Group representative as the first point of contact. Together, the site contact and the DOMS Working Group representative will make the decision on whether work can or cannot continue after discussions with the employee’s relevant organisation.

4) Person challenged on site and cannot produce HCoP/DOMS relevant training cards, but claims to have undergone training - if no cards can be produced, then the person challenging can not confirm that the person has had the relevant training by checking the EU Skills web site against name or reference number. This will be done in conjunction with the relevant DOMS Working Group representative.

5) Person challenged on site; DOMS CARD EXPIRED. If a DOMS card has expired and the person is waiting to attend a SW DOMS training course, a signed and dated cover note, with the course reference confirmation number and Registration Status clearly shown, must be issued by the person’s line manager, in order to allow access to the network.

The person must present this cover note on site.

(Failure to provide cover note will result in the individual being removed from working on the network until a cover note is issued, or the individual is retrained and re-registered with EUSR) The cover note will only be valid for 3 months after the original card expiry date. After 3 months has elapsed, if the person has not re-registered and is NOT in possession of a new card or a cover note stating the person is awaiting delivery of a new card, then access to work on the network will be refused.

All internal and external staff working on the water network must have completed the relevant training. If the site challenges are made by SW staff (and on occasions the DWQR) for all those working on the network, the message will be sent clearly that

'IF YOU HAVEN’T HAD THE TRAINING – YOU CANNOT WORK ON OUR NETWORK OR ANY INFRASTRUCTURE THAT WILL BE CONNECTED TO OUR NETWORK.’ While we all want to balanced and professional in our company approach to this matter, water quality and customer satisfaction in our product is our paramount concern.

Finally, can all Scottish Water staff note that for out-of-hours advice on public health issues, a representative from the Public Health Team can be contacted 24/7 through the ICC.

DOMS Impact Assessment Form.

A DOMS Impact Assessment Form (MCL 2628) is the control method for Scottish Water to authorise access to all parties onto its water distribution network. The DOMS IAF must be completed for all planned and unplanned intervention on the water distribution network.

Any DOMS IAF completed must Now also be Risk Assessed For Maintaining Supplies to include any alternative supplies overland requirements, or tankering operations etc to mitigate any risk to both Water Quality and prevent any unnecessary Interruptions To Supply which will or may affect SW OPA Score.

All Authorised DOMS IAF are now required to be sent to the Operation Log for Situational awareness. With a Proposed or Planned start date.

All valve operations on the water network must only be completed by staff trained in DOMS both internal and external (EU Skills registered).

All Boundary Valves will only be Operated by Designated staff who are authorised by the local

Water Operation Regional Manager.

Boundary Valve Authorisation Levels have now been increased (Sept 2012) for all Scottish Water

NSO’s / LFT`s and any other designated staff authorised by the local Water Operation Regional Manager Up To DMA level ONLY..

BV Authorisation and BV Operators

A comprehensive list Of All Staff for both the BV Authorisation and BV Operators will be updated by the Regional Manager and retained by the BV control line for Boundary Valve Management with Any Un- Authorised BV operations being reportable to the Local Water Operation Regional Manager for action.

Complex shuts

Should be undertaken by Scottish Water NSO`s / NSI`s & or Leakage Delivery LFTs or Stand By NSO.

For all internal & external contractors requiring access to the SW water network which is deemed a Complex shut the Contractors staff will act under the direct instruction and or supervision of the Water Operations NSO`s & or Leakage Delivery LFTs or Stand By NSO to provide support and assistance where required for any Warning Bills / Valve Operations / Pressure tests / Flushing / Water sampling if required.

Complex shuts update

Water Operation & or Leakage Delivery may NOW deem some Complex shuts to be LOW RISK and allow either Internal and External staff to undertake these types of Complex shuts.

The Water Operation & or Leakage Delivery staff member who authorise and deem the shut to be low risk will be responsible for ensuring that all Risk Assessments, Method Statements and Contingency Plans where applicable are checked too their satisfaction and in place ahead of the Complex shut being undertaken.

The table above shows the number of properties where escalation for planned work on the network requires additional escalation to the various business functions for their information and input when and if required. The Water Operational & or the Leakage Delivery staff authorising and completing the DOMS IAF for a Complex shut are responsible for the communication and liaison between all the relevant business functions above for their information and any input required. The final decision for authorising and allowing any Complex shuts to proceed lies with the Water Operational & or the Leakage Delivery or Standby Staff.

After completion and sign off by Water Operations & or Leakage Delivery the DOMS IAF is valid for 7 days. The work on the network which requires access must occur within 7 days any network access after the agreed start / completion date will require a new IAF to be produced or the original IAF to be amended and counter signed by the appropriate regional Water Operations contact to authorise the access extension.

The person authorised for operating the valves is responsible for updating the Operations log from site on the day of the work being under taken on the water network Pre & Post valve operations with the relevant Interruption To Supply details (Water On / Off Times & Property numbers) and for updating and the completion of the DOMS IAF when the work has been completed.

Scottish Water`s DOMS Impact Assessment Forms must be completed for any valve operations or hydrant operations where there is an impact or risk to the following: • Water quality • The continuity of supply or actual Interruption to Supply • To maintain water supply hygiene All staff either internal or external who are requesting access and raising work on the water distribution network are responsible for initially enabling site and checking all the valves / hydrants on site etc are locatable and are accessible for correctly completing the DOMS Impact Assessment Form for Water Operations & or Leakage Delivery to Authorise. If any Contractor is unable to complete any enabling work on site due to lack of information they must contact the relevant business function within SW for access to a GIS drawing. A DOMS IAF must be completed for the following scenarios where there is the potential for or actually any valve operations or hydrant operations etc are required for any planned or un-planned emergency work on the water distribution network: 1. All mains repairs or any mains intervention on the water distribution network that requires or has the potential for valves to be operated. This includes any mains repaired under pressure which still need valve(s) to be operated or hydrant / scours / washouts opened to reduce the pressure to fit a repair collar. 2. Service reservoirs that require compartments or tanks to be isolated for whatever reason.

3. Isolation of treated water storage points that require compartments or tanks to be isolated for whatever reason. 4. Mains and hydrant flushing as part of a planned / regular flushing program or as an emergency flushing program as part of an incident. Note Any Mains Flushing as part of any MST does NOT require an IAF but should be updated onto the Operation Log Pre & Post Flushing. 5. De-commissioning or re –commissioning of mains 6. Tankering or bowsers being filled as part of an incident. 7. Non Intrusive Pipe sampling or New Connection above 32mm to the live distribution mains. (Under Pressure branch or Bores) 8. Under Pressure bores / connection either 25mm or 32mm will only require a DOMS IAF if the work is to be undertaken on any main deemed a Complex shut or at SW discretion. All under pressure 25mm or 32mm connection will be completed as per the generic and or any site specific Risk assessment or method statement. 9. All trial excavations must be checked, agreed and validated in advance with Water Operations staff. Any trial excavation on or near (SW discretion) any water main deemed to be a Complex shut. Requires a valid / completed DOMS Impact Assessment Form.

A DOMS IAF is required to be completed for all burst repairs including any communication pipes and service repairs. The only exception is where the individual supply can be isolated without carrying out any valve / hydrant operations on the distribution network. If there is any doubt regarding the isolation of any communication pipe or the service pipe supply a DOMS IAF must be completed. The IAF must have the necessary information supplied for the isolation of the main and the valves must be cleaned and marked out. Complex Shut Criteria is any shut off required which meets any of the following criteria

1. Any Main 200mm or above in size?

2. Is a Strategic or Critical Main going to be affected?

3. Is it a multiple valve change job which requires NSO support (generally 5 Valves or more)?

4. Is Back-feeding required?

5. Is a District Metering Area (DMA) or Boundary Valve going to be operated / changed?

6. Are any Key Customers, Hospitals or Prisons going to be affected?

• Trunk Main Any main greater than or equal to 200mm diameter. • Strategic Main Any main of importance to delivery of the water supply service into an area. This will include those previously referred to as trunk mains but are less than

300mm Dia • Critical Main Any main within the proximity of a geographic feature (e.g. Railway, River, Bridge, Buildings, etc)

Which might cause difficulty of access to effect a speedy repair and the

consequence of failure are therefore severe.

Note Raw Water Mains are defined by their function, irrespective of diameter, as being any main carrying untreated water from a source to a treatment works or other delivery point.

The Field Customer Service NSO raising a planned work request contacts the (OMC) Risk Technician to schedule for a Water Operations NSO and for a Water Response repair squad after they have checked that the valves required to carry out the work are locatable and have been cleaned out and completed a DOMS IAF to the best of their Knowledge for issue to Water Operation for further ownership and authorisation .

The (OMC) Risk Technician should assign a task to a Water Operations NSO ASAP / minimum 48hrs prior to any planned complex shut to complete any onsite valve checks and back feeds required as part of a complex shut.

All Internal and external contractors who require a NSO resource to attend any site meeting for a

DOMS IAF to be checked, authorised and sign off etc MUST be done using Customer Services Delivery scheduling resource proforma for scheduling a NSO resource through Click.

All requests must have a minimum 3 working days notice, preferably 5 days prior to any initially Planned site meeting & another 3 working days notice, preferably 5 days for any complex shut off with an Auto code / work order number provided for NSO cost & time capture. The same work order will be used in the event a Complex shut has been confirmed and requires either a Water Operations NSO or LFT to complete the complex shut on any planned & agreed date.

All contractors will ONLY contact the CSD Scheduling on 0845 601 8855 to arrange for a NSO resource in EMERGENCY SITUATIONS ONLY.

carry out the work are locatable and have been cleaned out, are accessible, operable and are marked up for Water Response Repair team’s.

If 4 valves are required to be operated then all 4 should be cleaned out, checked and accessible.

If there are reasons why any of the above criteria is not met this MUST be recorded in the comments box detailing why and what is required for the work to go ahead.

The (OMC) Risk Technician does not require sending any NSO, as the Water Response repair Squad will complete the Valve Operations as part of a Non Complex shut.

The Water Response Team Leader is responsible for communication / liaison with Water Operation & or Leakage Delivery for any Complex / Non Complex shut off required on the water network with the planned start dates and times of shut offs to the network required.

Authorisation update

Customer Focus Delivery staff and Metering Services Staff are now allowed to complete and Authorise Non Complex shuts for their work, with Water Operation and or Leakage Delivery informed and the Operation Log update accordingly.

Any Complex shuts required by CFD and Metering Services must be Authorised by Water Operation & or Leakage Delivery.

For any internal or external contractors who require onto the Water Network the DOMS IAF will be checked and authorised by Water Operation & or Leakage Delivery staff.

The EUSR Skills number must be confirmed as valid by Water Operation & or Leakage delivery staff for any external staff or contractors before being allowed to operate any valves on the water distribution network as part of any Non Complex shut or any Low Risk Complex shuts as deemed by any Water Operation & or Leakage Delivery.

The contractor’s is responsible for any valve operations as part of a Non Complex or Low risk Complex shuts as per the instruction and detail on the agreed authorised and signed off DOMS IAF.

Internal or External Contractors who require access to the Water Network to carry out work on the Water distribution network.

IE CID mains Re-Hab / Customer Connections / Solutions etc

All request for a NSO resource to attend any site meeting for DOMS IAF sign off etc MUST be done using the CSD Scheduling resource proforma

for scheduling through Click. All request must have a minimum 3 working days notice, preferably 5 days prior to any planned meeting & or shut off with an Auto code / work order number provided

for NSO cost & time capture.All contractors will ONLY contact the CSD Scheduling on

0845 601 8855 to arrange for a Water Operations NSO resource in EMERGENCY SITUATIONS ONLY .

CID work should have had a pre start meeting with Water Operations & Or Leakage Delivery confirming the scale of the work and locations

with a program of works.

This would have GIS drawings as part of the work packs issued for

the site.

Water Ops NSO confirms if the work

required is a Complex or Non Complex shut.

Contractor meets Water Operations / Leakage Delivery to review agree and authorise all DOMS IAF and any proprietary work required.

All cost for NSO are captured through auto codes or Projects IDs etc especially for Complex shuts to be under taken by Water Ops NSO or LFTs.

All request for NSO resources are completed by the contractor and e mailed to the correct Work request.

The contractor should / will check as per their GIS drawings the physical site to confirm GIS and valve locations are locatable and accessible, Prior to the meeting.

This information is critical for NSO correctly completing DOMS IAF

The contractor will carry out all the work, including warning bills and all valve ops ,

flushing & any samples etc

The contractor will act under the direct instruction & or Supervision of Water

Operations to provide support where required Warning Bills / Valve ops / pressure tests /

After completion and sign off by Water Operations & or Leakage Delivery the DOMS IAF is valid for 7 days. The work on the network which requires access must occur within 7 days. Any network access after the agreed completion date will require a

new IAF to be produced or the original IAF to be amended. Water Operations & or Leakage Delivery Update Ops log with Planned shut off date

for contractor. Contractor updates the Operation log with the agreed planned Weekly

Whereabouts.

Water Operations NSO after checking the DOMS IAF that all the relevant valves are locatable and accessible,

He then confirms if it falls into the Complex or Non Complex shut off criteria If any valves as part of the planned shut off are missing the DOMS IAF must be

amended to confirm any additional valves required to be operated.Any valves which are inaccessible must have a separate job line raised to confirm

any further follow on work ASAP.
