10 A regular meeting of the Section on Maternity Care and Rural Physicians was held on Friday, April 7, 11 2017 at the Renaissance Austin Hotel in Austin, Texas. The Chair called the meeting to order at 2:30 p.m. 12

13 The Section took action at its meeting that resulted in the following recommendations for 14 consideration by the Board of Directors: 15

16 1. That the Academy offer three scholarships in the amount of $2,000 for a student and a resident,17

and $1,000 for a practicing physician member to attend AAFP’s Family-Centered Maternity Care18 Live Course each year. Awardees will be chosen by a selection committee appointed by the19 Section on Maternity Care and Rural Physicians at its meeting during Annual Session. This20 scholarship program will sunset after five years.21

22 BACKGROUND 23

24 AAFP hosts a four-day conference annually providing CME and clinical procedural workshop 25 training in maternity care. This year’s conference will be held in Denver, CO, July 19 - July 22. 26 Information can be found at http://www.aafp.org/cme/cme-topic/all/maternity-live.html. 27

28 The goal for these scholarships is to increase our state’s workforce of rural family physicians who 29 perform obstetrics. The scholarships will help defray costs for registration, housing, and 30 transportation for the educational experience. 31

32 The process proposed by the Section for selecting awardees and the requirements expected of 33 awardees are contained in the attached memo. 34

35 The following items were discussed at the meeting and are presented for information and further 36 distribution to members. 37

38 1. The Section discussed increasing difficulties with rural hospitals shutting down, which puts39

pregnant women at risk since the nearest hospital can be hours away. Mentioned that OB nurses40 are more expensive than floor nurses and with deliveries you have to have them. If things get41 worse – those departments will be closed first and then ultimately the whole hospital. The Section42 discussed that ambulances in rural areas are also being urged not to take pregnant women, which43 is giving expecting mothers in rural areas very little options for care. Members recommended that44 TAFP advocate for policies to keep those hospitals open.45

2. The Section Board liaison, Dr. Briggs, reported that the Section’s action item from its last46 meeting to create a resident exchange program for obstetrical training was not approved.47

3. Dr. Young presented his recent research on physicians providing obstetrics in rural hospitals. He48 explained there is no official list of rural hospitals.49

4. The Section suggested the Academy could have a resource list of rural hospitals that provide OB50 and then make that list available to residency program directors for residents interested in rural51

rotations that include OB. Staff will investigate the feasibility of creating and maintaining such a 1 list. 2

5. The Section discussed ways to get more students interested in OB and rural care. They mentioned3 the benefit of lobbyists going to residency programs, Skype lectures from rural practices, and4 connecting FMIGs with family docs in rural areas. They also mentioned scribe services that often5 hire and train pre-med students and that having them in family medicine may help to get them6 interested in family medicine.7

The Section on Maternity Care and Rural Physicians passed the following action item:

That the Academy offer three scholarships in the amount of $2,000 for a student and a resident, and $1,000 for a practicing physician member to attend AAFP’s Family-Centered Maternity Care Live Course each year. Awardees will be chosen by a selection committee appointed by the Section on Maternity Care and Rural Physicians at its meeting during Annual Session. This scholarship program will sunset after five years.

The TAFP Board of Directors approved these scholarships with a small correction. Several of the members of the Board felt that the practicing physician should also get $2,000 in travel funding, since $1,000 would only cover the registration and a little of the hotel.

A member of the Board made a substitute motion to provide $2,000 for each of the scholarships and the motion was approved.

Scholarship details

Selection process

Applicants will be chosen by a selection committee comprised of at least three attendees to the Annual Session meeting of the Section on Maternity Care and Rural Physicians. The selection committee will review applications and elect the award winners either by a conference call, over email, or by some other method. Awardees will be notified and invited to attend the Section meeting during Interim Session.

Expectations of awardees

Awardees will be expected to share what they have learned. They can do this in a number of ways but they will be expected to report their activity to the Section at the following Annual Session. Some ideas for presenting what they learned include:

• Give a presentation to the Section on Residents; • Give a presentation to the Section on Medical Students; • Give a presentation to a Family Medicine Interest Group; • Give a presentation to a Family Medicine Residency Program; • Write a blog post or a column for Texas Family Physician, something like

“Top 5 things I learned at AAFP’s Family-Centered Maternity Care Conference.”

Practicing physicians will also be expected to enroll as a preceptor in the Texas Family Medicine Preceptorship Program.

Note: The Section reserves the right not to award one, two, or any of the scholarships if it is deemed there are not suitable candidates for any of them. Because of the 5-year sunset provision, 2022 would be the final year for this scholarship program barring its reauthorization.

Rural Health Member Interest Group Grow connections among American Academy of Family Physicians members with a dist inct interest in rural health, inclusive

of rural practice topics and rural medical education issues related to producing family physic ians prepared for rural practice when you join the Rural Health Member Interest Group (MIG).

Established: July 2014

Objectives

• Grow connections among American Academy of Family Physicians (AAFP) members with a distinct interest in rural health, inclusive of rural practice topics and rural medical education issues related to producing family physicians prepared for rural practice

• Work collaboratively with the cross-commission rural group to provide grassroots AAFP member information for the use of the cross-commission rural group and the sponsoring AAFP commission

• Increase the participation of rural practicing physicians in the AAFP

• Articulate ongoing objectives of AAFP members with a distinct interest in rural practice and rural health by providing an annual update of the Rural Health MIG to the sponsoring AAFP commission

• Assist the AAFP in membership recruitment and retention of rural practicing family physicians

Long-term Goals

• Provide an effective grassroots communication structure for AAFP members with a distinct interest in rural health, inclusive of rural practice topics and rural medical education issues related to providing continuing medical education and producing a workforce of family physicians prepared for rural practice

• Engage and coordinate the members of the Rural Health MIG at AAFP meetings such as AAFP Assembly, as appropriate

• Integrate the Rural Health MIG with existing AAFP structures to work collaboratively. This includes cooperating with the cross-commission rural group to provide grassroots AAFP member information and working closely with the sponsoring AAFP commission to be a resource to the work of the commission.

• Increase AAFP membership among rural practicing physicians and other family physicians with interest in rural health, and demonstrate a recognized value of AAFP membership through the Rural Health MIG

• Provide a resource and sounding board to the AAFP on rural health and rural practice issues through the articulation of ongoing objectives in the annual update of the Rural Health MIG to the sponsoring AAFP commission

The 2017 fiscal year of the Texas Family Medicine Preceptorship program saw significant improvement in student participation, due in part to continued state funding, more aggressive marketing efforts and a greater understanding of the program experience.

In total, we received 189 applications from students across the lone star state. While the marketing and awareness challenges we faced in 2016 were largely avoided, 2017 brought a new set of challenges to overcome.

Preceptor employer challenges

Many of our volunteer preceptors faced internal challenges with their employer when it came to accept medical students for rotations. Some employer locations were willing to accept medical students into their facilities, but required several prequalification – drug testing, criminal background checks, vaccine records, etc. – to be completed prior to signing off on the experience. Often these requests delayed the specifically requested start times for the medical student and caused the rotation to be canceled.

Affiliation agreements

Affiliation agreements were another hurdle facing TFMPP. Several medical facilities required an affiliation agreement between themselves and the medical schools of students to facilitate the rotation. This also caused several matches to be cancelled.

Plenty of students for certain locations

Because we had an influx in medical student participants, we faced the obstacle of having a surplus of students requesting specific locations. Those popular locations did not always have enough available family physicians and again, often had number of restrictions for medical student rotations.

With these new challenges in mind we have begun dialogues with medical schools, individual practice administrators and physicians, to avoid these issues during the 2018 preceptorship program. TAFP remains committed to the opportunity to expose as many medical students to the family medicine specialty as possible.

ADDITIONAL FUNDING EFFORTS Texas Academy of Family Physicians continues to support the Texas Family Medicine Preceptorship Program in various ways. Multiple TAFP staff members provided their expertise to support and promote the program without billing the preceptorship program for their services. Reproductive Health Care Member Interest Group Promote evidence-based reproductive health care in family medicine when you join the Reproductive Health Care Member

Interest Group (MIG). Help AAFP members integrate comprehensive reproductive health care into their pract ices.

Established: October 2014

Objectives

• Promote evidence-based reproductive health care in family medicine.

• Create and offer CME activities on topics related to reproductive health care.

• Help AAFP members integrate comprehensive reproductive health care into their practices.

• Assist the AAFP in membership recruitment and retention of family physicians practicing reproductive health care.

Long-term Goals

• Allow women and men to obtain full spectrum reproductive health care in the privacy of their own family physician’s office.

• Influence policies of the AAFP to support advocacy, education and public health measures that expand access for women to high quality, evidence-based family planning care and which encourage appropriately planned pregnancies and good birth outcomes.

• Embrace miscarriage care as part of the scope of practice of family medicine in order to provide more holistic care to families.

• Develop and propose a Maintenance of Certification Part IV module to the American Board of Family Medicine focused on evidence-based comprehensive reproductive health.

• Develop leadership of MIG participants by creating opportunities through advocacy for the above goals.
