B. Background Information about SecureBlue’s Network Providers

B1. Key terms

This section explains key terms you’ll see in our Provider and Pharmacy Directory.

• Providers are health care professionals and support providers such asdoctors, nurses, pharmacists, therapists, and other people who providecare and services. Services include medical care, long-term services andsupports (LTSS), supplies, prescription drugs, equipment and otherservices.

o The term providers also includes facilities such as hospitals, clinics,and other places that provide medical services, medical equipment,and long-term services and supports.

o Providers that are a part of our plan's network are called network providers.• Network providers have contracted with us to deliver services to members

of our plan. Network providers participate in our plan. That means they

accept members of our plan and provide services our plan covers. Theproviders in our network generally bill us directly for care they give you. When you see a network provider, you usually pay only your share of the cost for covered services.

• A Primary Care Clinic (PCC) is a provider who gives you routine healthcare. Your Primary Care Clinic/PCC will keep your medical records andget to know your health needs over time.

• Specialists are doctors who provide health care services for a specificdisease or part of the body. There are many kinds of specialists. Here area few examples:

o Oncologists care for patients with cancer.

o Cardiologists care for patients with heart conditions.

o Orthopedists care for patients with certain bone, joint, or muscle conditions.• You also have access to a care coordinator. A Care Coordinator

helps you manage your medical providers and services.

• Your Collaborative Care Plan is an ongoing action plan thataddresses your healthcare needs. Your care coordinator will work withyou to create a person- centered Care Plan that includes yourhealthcare goals and the resources, supports, and services you needto achieve them. Your Care Plan will be updated as your healthcareneeds change.

B2. How to choose a Primary Care Clinic/PCC

You can get services from any provider who is in our network and accepting new members.

When you become a member of our plan, you will choose a Primary Care Clinic/PCC who is part of our plan network. This provider will be your Primary Care Clinic/PCC.

If you did not choose a Primary Care Clinic/PCC or would like to make a change to your Primary Care Clinic/PCC, go to the list of providers in Section C that starts on page 3 and choose a provider:

• that you use now, or

• who has been recommended by someone you trust, or

• whose offices are easy for you to get to.

• If you want help in choosing a PCC, please call Member Services at thenumber at the bottom of this page.

• If you have questions about whether we will pay for any medical service orcare that you want or need, call Member Services and ask before you getthe service or care.

B3. How to get long-term services and supports (LTSS)

You may be able to get long-term services and supports (LTSS), such as services provided by the Elderly Waiver (EW) program as a SecureBlue member. LTSS help people who need assistance to do everyday tasks like taking a bath, getting dressed, making food, and taking medicine. Most of these services are provided at your home or in your community but could be provided in a nursing home or hospital.

You must have a Long-Term Care Consultation (LTCC) done and be found to be eligible to get additional services or support. You can ask to have this assessment in your home, apartment, or facility where you live.

Your care coordinator will meet with you and your family to talk about your care needs if you call to ask for a visit.

Your care coordinator will give you information about community services, help you find services to stay in your home or community, and help you find services to move out of a nursing home or other facility.

If you need transition planning and coordination services to help you move to the community, you may be eligible to get Moving Home Minnesota (MHM) services. MHM services are separate from EW services, but you must be eligible for EW.

B4. How to access SecureBlue’s network providers

You must get all of your covered services from providers within our network. If you go to providers who are not in SecureBlue’s network (without prior authorization or approval from us), you will have to pay the bill.

A prior authorization is an approval from SecureBlue before you can get aspecific service, drug, or see an out-of-network provider. SecureBlue may not cover the service or drug if you don’t get approval.

The exceptions to this rule are when you need urgent or emergency care or dialysis and cannot get to a provider in the plan, such as when you are away from home. You can also go outside the plan for other non-emergency services if SecureBlue gives you permission first.

• You may change providers within the network at any time during the year.If you have been going to one network provider, you do not have to keepgoing to that same provider. You do not need to call Member Services tonotify us of this change. Call the new provider and ask them to help youtransfer your health care records.

• SecureBlue works with all the providers in our network to accommodate theneeds of people with disabilities. If you need to see a provider and are notsure if they offer the accommodations you need, SecureBlue can help you.Talk to your care coordinator for assistance.

B5. How to find SecureBlue providers in your area

Providers in the SecureBlue network are listed in this directory in sections by type of provider. Examples are Primary Care Clinics/PCC, Hospitals, Nursing Homes, Chemical Dependency Providers and Dentists. To locate a provider, look in the section that contains the type of provider that you are seeking and then city (or city nearest you). If you cannot find a provider using the directory, our Member Services will help you locate one.

How to access out-of-network providers

SecureBlue covers certain health care services you get from out-of-network providers. These include: care for medical emergencies, urgently needed care and services that were originally denied but later overturned in an appeal. How to access specialty providers

SecureBlue makes it easy to see a specialist when you need to. That’s because SecureBlue features an “open-access” provider network. Open access means SecureBlue will not require you to get a referral from your Primary Care Clinic/PCC when you need to see a network specialist. If you need to see a specialist, there are several ways to find one. You can ask your Primary Care Clinic/PCC for help. Or choose any specialist in our large network. For a list of doctors and specialists, see the provider listings in this book. For the most up-to-date list and for help finding a specialist, call Member Services. You can choose any doctor, provider, hospital, pharmacy or family planning agency to receive some services. These services are: family planning, diagnosis of infertility, testing and treatment of sexually transmitted diseases and testing for AIDS and HIV-related conditions.

How to get more information about personal care provider organizations (PCPO) and home care agencies

For a listing of PCPOs and home care agencies, please refer to the Personal Care Assistance (PCA) Agencies or Home Health Care Agencies section of the directory. How to find a network pharmacy

How to determine if a facility is accessible
