Assisted Living –This category of hous-ing provides care forthose who need somehelp, yet would like toremain as independentas possible. Assistedliving facilities offer, butdo not require, aid withthings such as: bathing,eating, dressing andhousekeeping. Anexcellent option forthose who do notrequire constant care.

Nursing Home –While this type of hous-ing is designed to carefor those unable to care for themselves,with numerous medical and health require-ments, it is always best to consider a nursinghome that allows for frequent visits fromfriends and family.

Home Care – Home Health servicesinclude skilled personnel who provide carein the patient’s home or in a living facility.This will ensure a continued independencewhile providing all of the care and assis-tance required. How to find the proper type of Senior Housing and Level of Care*

RETIREMENT HOUSING

Apartment-like facilities that include living essentials such as meals and transportation. These often include facilities for various activities.

ASSISTED LIVING

These facilities are set up to provide residents help with daily activities andalso usually offer some level of healthcare services.

RESIDENTIAL CARE

Non-apartment facilities that provide assistance for residents with medication,hygiene, and simple tasks such as getting dressed. Typically a home setting.

SKILLED NURSING/REHAB

These facilities are licensed and equipped to provide residents with healthcare that meets the needs of more involved medical problems.

ALZHEIMER’S/DEMENTIA ASSISTANCE

Alzheimer’s facilities provide special programs and assistance for those withmemory loss. Some may provide special security measures to prevent residents

from wandering off during periods of disorientation.

RESPITE CARE

Facilities that provide Respite Care offer family and other caretakers areprieve from the stress and work of everyday care of elderly persons who

cannot perform or manage care for themselves.

HOME CARE

Various types of at-home care, both medical and non-medical, that allowseniors to remain in their place of residence rather than having to travel backand forth or be admitted to a medical facility. A cost-effective alternative to

hospital stays or Skilled Nursing facilities.

SENIOR DAY CARE

Facilities that provide for the needs of seniors during daytime hours only.Services typically include meals, social interaction, arts/crafts/entertainment, andstretching/gentle exercise. Centers often accommodate seniors with Alzheimer’s

or Dementia. Day care allows family to go to work or a much needed rest.

HOSPICE CARE

End of life care for the terminally ill, usually via visits in the home or facilityof residence. Focus lies with pain management, comfort, and emotional

support for the individual and family.

CARE MANAGEMENT

Care managers examine their clients’ needs and make recommendationsregarding Home Care providers, the best type of housing for the individual,and other aspects of care that can be daunting for a senior or their family.

These recommendations are orchestrated into a comprehensive plan of care.

It’s estimated that half of all Alzheimer’s family caregivers experiencedepression. Family caregivers also experience personal health issues,complications in the workplace, fatigue, and stress with other family membersthat result in conflicts and less leisure time.

On the positive side, some family caregivers find that providing care inthe home is beneficial because it gives them a new purpose in their lives,strengthens family relationships, or fulfills commitment to their loved one.

However, providing around-the-clock care requires a great deal of timeand energy, in addition to the financial burden incurred by those providingthis care. These factors can take a significant physical and emotional toll.

As a family caregiver, we must learn to nurture our own needs. The following recommendations provide some coping strategies for these daunting challenges:

Look for support – Many family caregivers withdraw from family and friendsbecause they feel no one understands. It is very helpful and in many cases,therapeutic, for you to join a support group where you can learn from othersthrough sharing your experiences.

Let go of guilt – Let go of “the guilt trip” by realizing a need to temporarilyset aside caregiving responsibilities for a respite.

Nurture the body – Be sure to get enough sleep and eat properly.

Nurture the mind and spirit – Schedule family sessions and discuss anyconflicts that relate to the care of your loved one. Take breaks from caregivingas often as possible by asking other family members to help. Be sure toacknowledge your feeling of loss and grief.

Establish limits – Say no to requests that are beyond your capacity, andsay yes to offers of help with a love one. Remember, you have your ownneeds, too.

Texas Law dictates that both birth anddeath certificates are deemed confidential fora set period of time. Birth certificates areconfidential for 75 years, and death certificatesare confidential for 25 years after the deathhas occurred. If a birth or death certificate isrequested before this period of time elapses,certified copies of these documents can onlybe obtained by a qualified applicant.

A qualified applicant can be an immediatefamily member (by blood or marriage), a legalguardian, or the legal representative of theperson for whom the document is requested.If you are not a qualified applicant, you mayrequest a verification of a birth or death record.

Fees charged for copies of birth and deathcertificates by the different county officesmay vary slightly – contact your local officeabout cost for copies of Birth and DeathCertificates. Certified copies of the birth and death certificates are issued with a state seal andare printed on security paper. Heirloom birthcertificates have a different cost and areprinted on special paper that boasts a designcelebrating Texas. The heirloom certificate($60) is only offered through Austin, Texas.

Funeral homes will supply certified copies ofa death certificate for a small fee if requestedat the time funeral arrangements are made. It is recommended that you get at least twelvecopies of the certificate and notify anyonewith whom the deceased had an account.

VERIFICATION OF BIRTH AND DEATHThe verification of a birth or death record

is NOT a certified copy of the birth or death certificate, but a letter with the person’sname, date of occurrence, county where thebirth or death took place and a state filenumber. Letters may not be available at allbirth and death registrars. Verifications areavailable from 1903 to the present date. Feesfor birth and death verification (for regularmail or walk-in service) range from $22–$30.This fee is non-refundable for records thatare not located. Additional fees will apply forfaxing and overnight services.

Deceased Identity TheftTo avoid identity theft of a deceased person, itis recommended that you take the followingsteps:

How Does Hospice Work?• Hospice care is for any person who has a life-threatening or terminal illness.Most reimbursement sources require a prognosis of six months or less if the illnessruns its normal course. Patients with both cancer and non-cancer illnesses are eligible to receive hospice care. All hospices consider the patient and family together as the unit of care. • The majority of hospice patients are cared for in their own homes or the homes ofa loved one. “Home” may also be broadly construed to include services provided innursing homes, hospitals and prisons. • Typically, a family member serves as the primary caregiver and, when appropriate,helps make decisions for the terminally ill individual. Members of the hospice staffmake regular visits to assess the patient and provide additional care or otherservices. Hospice staff is on-call 24 hours a day, seven days a week.

Who is the Hospice Team?• Hospice care is a family-centered approach that includes, at a minimum, a teamof doctors, nurses, social workers, counselors, and trained volunteers. They worktogether focusing on the dying patient’s needs; physical, psychological, or spiritual.The goal is to help keep the patient as pain-free as possible, with loved ones nearbyuntil death. The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.

How is Hospice Paid for?• Hospice is paid for through the Medicare Hospice Benefit, Medicaid HospiceBenefit, and most private insurers. If a person does not have coverage throughMedicare, Medicaid or a private insurance company, hospices will work with theperson and their family to ensure needed services can be provided.

Who is Eligible for Medicare Hospice Benefits?Hospice care is covered under Medicare Part A (Hospital Insurance). You are eligiblefor Medicare hospice benefits when you meet all of the following conditions:

• You are eligible for Medicare Part A (Hospital Insurance), and • Your doctor and the hospice medical director certify that you are terminally ill andprobably have less than six months to live, and • You sign a statement choosing hospice care instead of routine Medicare coveredbenefits for your terminal illness,* and • You receive care from a Medicare-approved hospice program. Please note: Medicare will still pay for covered benefits for any health problemsthat aren’t related to your terminal illness.

What Does Medicare Cover?Medicare covers these hospice services and pays nearly all of their costs:

• Doctor services • Nursing care • Medical equipment (like wheelchairs or walkers) • Medical supplies (like bandages and catheters) • Drugs for symptom control and pain relief • Short-term care in the hospital, including respite and inpatient for pain and symptom management • Home health aide and homemaker services • Physical and occupational therapy • Speech therapy • Social work services • Dietary counseling • Grief support to help you and your family

Please note: You will only have to pay part of the cost for outpatient drugs and inpatient respite care.Respite Care and Medicare?Respite care is care given to a hospice patient by another caregiver so that the usualcaregiver can rest. As a hospice patient, you may have one person who takes care ofyou every day, like a family member. Sometimes your caregiver needs someone totake care of you for a short time while they do other things that need to be done.During a period of respite care, you will be cared for in a Medicare- approved facility,such as a hospice residential facility, hospital, or nursing home.The Medicare Hospice Benefit Does Not Cover the Following:Treatment intended to cure your terminal illness –As a hospice patient, you will receive comfort care to help manage symptoms relatedto your illness. Comfort care includes medications for symptom control and painrelief, physical care, counseling, and other hospice services. Medications not directlyrelated to your hospice diagnosis are not covered under the Medicare HospiceBenefit.

Hospice team members will consult with the hospice physician and will inform youand your family which drugs and/or medications are covered and which ones are notcovered under the Medicare Hospice Benefit. The Hospice uses medicine, equipment,and supplies to make you as comfortable and pain- free as possible. Under the hos-pice benefit, Medicare won’t pay for treatment to cure your illness. You should talk with your doctor if you are thinking about potential treatment to cureyour illness. As a hospice patient, you always have the right to stop getting hospicecare and receive the “traditional” Medicare coverage you had before electing hospice. Care from another provider that is the same care that you are getting from your hospice –All care that you receive for your illness must be given by your hospice team. You can’t get the same type of care from a different provider unless you change your hospice provider.Nursing Home Room and Board –Room and board aren’t covered by Medicare. You may receive hospice services wherever you live, even in a nursing home, however, the Medicare Hospice Benefitdoes not pay for nursing home room and board.How Do I Begin Receiving Hospice?Anyone can inquire about hospice services. You or your loved one may call a localhospice and request services. The hospice staff will then contact your physician todetermine if a referral to hospice is appropriate. Another way to inquire about hospiceis to talk with your physician, and he or she can make a referral to hospice.Hospice can begin as soon as a ‘referral’ is made by the person’s doctor. Questions to Ask When Choosing a Hospice ProgramHospice care is a philosophy of care that accepts dying as a natural part of life. Whendeath is inevitable, hospice seeks neither to hasten nor postpone it. Below is a list ofquestions you should consider when looking for a hospice program.

• What services are provided? • What kind of support is available to the family/caregiver? • What roles do the attending physician and hospice play? • What does the hospice volunteer do? • How does hospice work to keep the patient comfortable? • How are services provided after hours? • How and where does hospice provide short-term inpatient care? • Can hospice be brought into a nursing home or long-term care facility?
