SCOPE OF INITIATIVE
• Work with hospitals, physicians, nurses, and clinical teams for at least two years
• Gap analysis/action plan– Enlist all in a program to identify, track and
report on patients with infection and severe sepsis
• Education – in person, webinars, etc.• Coaching calls• On-line tools, resources

CALL TO ACTION• Gain hospital administration support
– Sign letter of commitment– Engage willing physicians, nurses, clinical teams
• Attend Quality Summit on June 10, 2015• Complete gap analysis/identify area(s) of opportunity• Implement plan of action
– Willingness to adhere to protocols– Implementation of goal-directed sepsis therapy
• Submit baseline data– Collect compliance with 3 hour bundle and submit
numerators & denominators to OHA
