Sharing the Experience:Sharing the Experience: Advance Care Planning in the State of Indiana

Program DescriptionThis continuing education program will explore the current state of advance care planning in Indiana including resources available across the state, programs that are available to facilitate advance care planning conversations, discussion of new ideas related to planning, hospice and palliative care facts, and the future of advance care planning in Indiana.

Target AudiencePhysicians, nurses, social workers, case managers, health facility administrators, clergy, chaplains and other interested health professionals

Learning ObjectivesAt the completion of this program the participant will be able to:

• Identify available resources for advance care planning

• Discuss the how advance care planning can improve practice

SpeakersDeborah Laxson, founder of the Health Care Agent Literacy Project, LLC and author of the award winning book, "The Gray Zone: When Life Support No Longer Supports Life"

Katie Hougham, HFA, MBA, BS, is the current ACP coordinator for the Northeast Indiana Coalition for ACP and Honoring Choices® Indiana. Katie is a certified Last Steps® Organization Faculty and First Steps® facilitator/instructor through Respecting Choices®, an internationally recognized model for advance care planning.

Stacy Wenk, DO, RPH, FACP, is a Hospitalist with a focus on geriatrics, end of life care and health disparities. She is President elect of the Indiana State Medical Association and works with its legislative team to help promote regulatory changes involving end of life care needs.

Kevin J. Valadares, PhD, is a professor of Health Services and chair of the Master of Health Administration program at the University of Southern Indiana. He holds a doctorate in health care ethics from Saint Louis University and is a Respecting Choices® First Steps® Last Steps® instructor/facilitator.

Nurses, Indiana social workers and health facility administrators will receive up to 5.75 contact hours. Completion of the online evaluation and attestation of attendance is required within two weeks of completion of the conference in order to receive certificate of completion. No requests for certificates after two weeks will be accepted. Certificates will be emailed immediately following completion of the online evaluation.

The University of Southern Indiana Center for Health Professions Lifelong Learning designates this live activity for a maximum of 5.75 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Attendees will receive up to 5.75 contact hours with attestation of sessions attended and completed program evaluation.
