The IFZI Phantom Course:

Modules 1 + 2 – The route to becoming an Implantologist

The implantation simulator at the International Training Centre for Dental Implantology (“IFZI”) offers practical training in the administration of implant surgery and prosthetics to artificial pa-tients (phantoms). Beginners can learn and train in the basics of implantology and gain an insight into the skills and capabilities of professional implantologists. Experienced implantologists can continue to develop their skills in consultation with experienced trainers, in order to acquire the very latest techniques and to benefit from tried-and-tested tricks used in practice. The ed-ucational trainers observe the actions taken and the method used during practical exercises and offer tips and instruction that will increase effectiveness. Rudolf Moser, Head of Market-ing & Communication at bredent medical, supervised Dentist Timo Selje on the IFZI implantation simulator in November of last

year and then asked him about his experiences.

Rudolf Moser: Shortly after com-pleting your studies in dentistry you began a career in implantology and worked as an Assistant Dental Surgeon for an experienced Im-plantologist. Why did you decide to undergo further training on the IFZI implantation simulator?

Timo Selje: After completing my studies and gaining my first expe-riences in implantology during my time as an assistant dental surgeon, I had acquired the necessary theory skills but I wanted to supplement those skills as much as possible by taking part in additional practical training.

My bredent medical regional man-ager, Helmut Finger, then intro-duced me to modules 1-4 of the IFZI implantation simulator. He piqued my curiosity by proposing a full two-day practical course with professionals with many years of experience in implantology, inten-sive training and an exchange of information with other course par-ticipants.

Rudolf Moser: Are there other factors that were important to you in addition to practical orientation? What did the first day of further training consist of?

Timo Selje: It was important to me that the further training was carried out in a manageable group size. I much prefer working closely within a small group than being part of a huge anonymous organisation.

IFZI more than fulfilled this require-ment. The first day of further train-ing began with a personal welcome from Prof. Dr. Manfred Lang and a brief introduction to all participants and trainers.

Some time was devoted to the theory of the subject with the pres-entation of the SKY implant system and explanations of the thinking underlying the development of the implant. A discussion of the surgi-cal protocol then ensued. This was great, as the system is simple and self-explanatory.

Last but not least, the IFZI module and the philosophy of the fur-ther training centre was explained, which primarily involves creating a solid foundation for the activities that form part of implant sur-gery and not using dry theory, but more through practical exercises on phantom patients and through passing on many years of experi-ence in the field.After the brief introduction to the theory of the subject, all partic-ipants were able to follow the demonstration of implantation in the maxilla and mandible and the accompanying suturing process carried out by Prof. Dr. Manfred Lang in an adjacent operating the-atre.

That afternoon after a short lunch break, the group was divided into three teams of three in order to carry out the practical exercises.

Rudolf Moser: It sounds like a var-ied morning. How did the afternoon continue?

Timo Selje: It was definitely varied, but it was definitely entertaining. The morning went by in a flash.

After lunch, the teams of three underwent intensive training under the direction of an experienced IFZI trainer, so that we could put what we had been shown that morning into practice.

Every participant played the role of Surgeon, standing Assistant and seated Assistant in a very relaxed and natural environment. Three im-plants were inserted in each case here (3 + 4 quadrant and in the region of the front teeth in the mandible).

Rudolf Moser: Was there even any time left for exchanging in-formation with colleagues after all of that?

Timo Selje: Yes, of course! During the evening we sat down together to eat and found ample time to discuss what we had learnt and to recap on it.

However, I have to admit that after the meal I fell into bed with a full stomach and was filled with antici-pation for the following day.

Rudolf Moser: Did the second day manage to surpass the first day in terms of the experience you gained?

Timo Selje: The following day was just as good in every way. We fo-cussed in more depth on the practi-cal exercises in the groups that had been formed.

The next day started at 09:00 on the dot. We inserted 9 implants in the mandible and 7 implants in the maxilla, including internal sinus lift under the guidance of the IFZI trainer. With the knowledge and techniques we learnt on the previous day and the numerous tips and tricks that the IFZI trainer had up his sleeve, we were able to carry out the whole process quickly and effectively. I was genuinely surprised by how safely and quickly

I was able to insert the implant myself.

Time flew by in a flurry of edu-cational material and before we knew it, it was time for a short lunch break at the Italian restau-rant around the corner. The morn-ing programme then continued in a more intensive manner.

Once every person in the group had inserted his/her implant and assisted one another in inserting their implants, an assessment of the phantom model was carried out under the watchful eye of Prof. Dr. Manfred Lang. He offered the occasional tip, ideas for optimising the process or a little constructive criticism to everyone who was in-serting an implant. The day ended with a little celebratory distribution of certificates and we provided our feedback on the course.

Rudolf Moser: What feedback did you give and how would you sum up your experience?

Timo Selje: I personally found the two-day course extremely benefi-cial. I learnt an optimal, efficient and ergonomic technique which will enable me to improve the way in which I carry out implantation.

The exchange of ideas and knowl-edge with the IFZI trainers and with Prof. Dr. Manfred Lang was unbe-lievably beneficial to me and I took away a lot from this. The best part was that all of these people who have tremendous amounts of expe-rience in the field of implantology were happy to share this experience and knowledge with us and did so in a fun way.

This was definitely enjoyable and helps motivate people to achieve more!

I will be putting modules 3+4 in my compulsory programme at the next possible opportunity!

I would recommend this course to anybody - regardless of whether

they are a beginner or an experi-enced professional. Anyone who takes part will take something away with them for their own practice.

Innovations Based on Therapeutic Concepts

Beginning with the end in mindNeil Cooper is one of those peo-ple who enthuses you from the start. So it comes as no surprise when he tells you that he himself starts everything with the end in mind. It is an approach that has helped him build one of the most successful implant practices in the country.

Working in close harmony with dental technician, Steve Taylor, Neil and his team have gained an enviable reputation for Crossbank Dental Care in Kendal. Just how that reputation has been made not only makes interesting reading, but serves as an inspiration to any as-piring dentist who is told that their dream ‘won‘t work‘.

‚When I bought the practice in Kendal back in 2006, I was ‚reliably‘ informed that I could never have a successful implant practice based in Cumbria‘, recalls Neil,‘ we now have over 70 dentists referring work to us, which tends to suggest that one should take much of the advice one is given with a pinch of salt.‘

Where it started

After Neil graduated from Sheffield, he got a locum post in Kendal. The dentist was approaching retirement and suffering from back pain so Neil talked to the lady principal with a view to purchasing the practice. She quickly agreed it would be to everyone‘s advantage, so together with his colleague and friend from Sheffield, John Rees, the deal was done. That was in 2001 and Neil and John immediately put in place an overnight conversion to become the first private practice in Kendal. In 2006, Neil and John agreed that they wanted to pursue different directions and an amicable arran-gement was reached. Neil was now the principal.

At this stage in his career Neil was a prosthodontist with burgeoning skills as a surgeon and a keen desire to hone those skills. ‘I was greatly helped by Fazeela Khan Osborne‘ says Neil,‘ she not only taught me a great deal about placing implants, but she introduced me to a number of leading (and very successful) practitioners.‘ With unending com-mitment to learn more he travelled all over Europe and further afield to meet those who had achieved more than he ever dreamt possible.

Working within your ablities

As his skills developed, Neil con-sistently assessed his own capa-bilities. Comparing himself with the best demanded a realistic and objective view of implant cases he could comfortably undertake at that stage and those that he, in turn, should refer.

‚Basically, if you can hold a pen in a steady hand, you can drill a hole,‘ he believes,‘ what you may lack is the knowledge that comes only with experience. Some of this may be gleaned by rubbing shoulders with the finest implant surgeons, but mostly it is gained by doing only implant cases that you know you can confidently handle‘. Fun-damentally he feels that you work up to the more complex cases. He also acknowledges that this may be stating the obvious, but he takes a dim view of clinicians who punch above their weight. Like all the best practitioners, Neil likes to be confident but free from arrogance.

Neil’s knowledge grew by small steps. Realistically he assessed one of his major strengths as spacial awareness, but, equally realistically he did not see himself as an acade-mic. His approach was to develop and refine his practical skills and

then reinforce these by acquiring the appropriate and relevant scien-tific knowledge. Alongside this, his enthusiasm and passion for the true

benefits of placing implants grew apace. ‘The huge changes implants can make to people’s lives truly in-spired me’ is how Neil sees it.

More inspiration

In his studies and research, Neil became increasingly aware of the growing interest in same day im-plants. The SKY fast & fixed system developed by bredent medical par-ticularly caught his eye and visits to Michael Weiss’s practice in Ger-many confirmed his view that this was the best immediate loading protocol on the market.

To be able to offer SKY fast & fixed to the standard Neil wanted required working with a really skil-led dental technician. And this is where Neil simply got lucky. He was lined up to make a presenta-tion at a BAAD conference which, if successful would earn him full membership, when he met Steve Taylor , a technician with more than 20 years’ experience implant expe-rience and considerable expertise in immediate restorations using a reduced number of implants.

In short, it was the perfect match and Neil cites a number of specific reasons for this being the case:

• a shared appreciation of the im-portance of special awareness

• a total commitment to fulfil-ling the patient’s requirements in terms of the size, shape and orientation of the restoration

• an agreement that a problem exists only in the absence of a good conversation

• the ability to understand and process information gained th-roughout the planning process and recognise how this can translate into a final result that will look natural

• the desire to communicate rather than work in a bubble

• an absence of ego

It was clear from the start of their relationship that Neil and Steve un-derstood and appreciated the value of teamwork. Diagnostically it was important that everyone involved, from clinician through to nurse, was ‘tuned in’. ‘Neither Steve nor I are big fans of the Hollywood smile…..what we are looking to achieve is what I call “immaculate deception”, believes Neil, ‘which to us means giving patients a natural, healthy smile with pleasing im-perfections This way the patient’s edentulousness goes unnoticed and their privacy is protected ’.

I also believe that technicians can help reassure and encourage pati-ents by showing that they genui-nely love the creative “buzz” that goes with fast& fixed. Every patient is unique and that’s what’s exciting’.

Keep on learning

Both Neil and Steve have the ex-perience and wisdom to know they will always benefit from acqui-ring more experience and wisdom. Complacency is not a word that is in their individual or collective vocabularies.
