Subjective: The client

brushes his teeth only in the morning.

The client does not brush his teeth in non-school days

The client visits the dentist only when he was two years old.

Objective: 4 decayed

teeth in upper part with 1 tooth fracture.

6 decayed teeth in lower part with 3 teeth fractures.

Impaired dentition related to deficient knowledge regarding dental health.

Analysis:

The client is not regularly brushing his teeth.

Goal:The client’s will display teeth in good repair.

Objectives:

-Demonstrate effective dental hygiene skills.

-identify adequate nutritional and proper foods.

-Encourage brushing teeth after meal and at bedtime.

-Demonstrate correct brushing and flossing procedure.

-recommend that client limit sugary/high carbohydrate foods in diet and snacks

-encourage eat

-To be free from debris and plaque on dental surface

-To guide the patient on the proper care of his teeth.

-To reduce buildup of plaque and risk of cavities caused by acids associated with breakdown of sugar and starch.

-To keep the teeth

After the nursing intervention, goal partially met.

The patient able to demonstrate correct brushing and flossing procedure teeth after meal and at bedtime.And able to identify adequate nutritional and proper foods.

But the patient’s teeth are not yet in good repair.

-Follow through on referrals for appropriate dental care.
