PARTNERS IN RECOVERY• Is a program that improves the system

response to people who experience severe and persistent mental illness with complex support needs

• Works to improve access to services

• Coordinates services so that they are working together to meet the full range of a person’s support needs

• Is a national mental health initiative funded by the Australian Government

PARTNERS IN RECOVERY ASSISTS PEOPLE WHO:

• Experience ongoing mental health challenges

• Find it difficult to access the services and support that they want

• Want support that will prioritise their personal autonomy and choice

• Want to explore improving their well being

OUR SUPPORT FACILITATORS CAN:• Support people to assess their needs

• Assist in accessing a range of supports and services to meet these needs

• Provide service coordination to support integrated, flexible and responsive provision of support

• Develop partnerships which promote collaboration and innovation

Partners in Recovery is managed by South Eastern Sydney Medicare Local, delivered by local organisations and funded by the Australian Government.
