I, NARUTO UZAMAKI, Filipino, of legal age, married and resident of Brgy. Konoha, Leyte, do hereby name, constitute, and appoint my wife, HINATA UZAMAKI, of legal age, Filipino, married and a likewise a resident of Brgy. Konoha, Leyte, to be my true and lawful attorney, for me and in my name, place, and stead, to do and perform the following acts and things to wit:

1. To process, prepare, file the necessary application forms/documents, and to receive for me and in my behalf the EMERGENCY SHELTER ASSISTANCE (more commonly known as ESA) extended by the DSWD to victims of typhoon Yolanda; and

2. To prepare, accomplish and sign any document as may be required in relation to the authority granted to her.

HEREBY GIVING AND GRANTING unto my said attorney full power and authority whatsoever requisite or necessary or proper to be done in and about the premises as fully to all intents and purposes as I might or could lawfully do if personally present, with power of substitution and revocation, hereby, ratifying and confirming all that my said attorney or her substitute shall lawfully do or cause to be done under and by virtue of these presents.
