The visa will be affixed to your passport before you depart the United States and it must be presented at your point of entry into the Schengen countries. Documents you must submit in order to apply for a Student Visa: 2 National Visa Application forms dully filled out and signed. Passport. 2 recent photos passport size with a white background. Staple one photo on each of the application forms.

Letter of acceptance as a full time student from Spain's University/School or US program indicating: name, address and registration number of the school with Spain's Department of Education; (Ministerio de Educación de España) full payment of tuition, duration of the program, subjects of the study and hours of study per week which must be no less than 20.

Health Insurance, International insurance coverage for health/accident with a minimum coverage equal to €30,000 and "Repatriation Coverage" during the planning period of stay in Spain (or its equivalency in dollars).

Proof of financial means during your stay: Please provide ONE of the following: 1. Letter from the University or School in Spain or in the USA assuming full financial

responsibility during your stay (this is often included into the acceptance letter) 2. Proof of financial aid or scholarship for at least $1,000 per month for room and

board. 3. Notarized letter from your parents or legal guardians assuming full financial

responsibility for at least $1,000 per month for room and board. Suggested wording: “I hereby certify that I'm the (father/mother/other) of (...), will support him/her with a monthly allowance of at least $1,000 while he/she is in Spain and that I'm financially responsible for any emergency that may arise".

4. Personal bank account statements showing at least $1,000 per month of stay. "Money-Order" to pay the non-refundable visa fees (no personal checks or cash accepted). Evidence of your migratory status in the USA (Only for non US citizens): Provide your "Alien Registration Card" or “US Visa with I-20/IAP-66” (except B1-B2). Police Records Certificate for persons 18 years old & up (for periods longer than 180 days) *** from the country of origin or place of you residence for the last five years bearing the "Apostille of the Hague Convention" (*** See special notes at the bottom of this page). If the Certificate is issued outside the USA it must be duly legalized by the corresponding Consulate of Spain plus 1 copy. Medical Certificate (for periods longer than 180 days): Doctor's statement on a doctor or medical center letterhead, indicating that the student has been examined and found in good physical and mental health to travel to study abroad and is free of contagious diseases or any other illnesses which could lead to Public Health repercussions according to the International Sanitary Regulations plus 1 copy. Notarized authorization letter to apply for a visa from the parents or custodians (only if the applicant is under 18 years of age). If you want your passport returned to you by mail you MUST bring a self-addressed and pre-

paid UPS.COM label to receive your passport back from us. Create a shipment on-line at www.ups.com and print the label. We do NOT accept any envelope that is no prepaid or any other type of courier or messenger service. Tracking of mailed items is the sole responsibility of the applicant.

•ADDITIONAL REQUIREMENTS & TIME MAY APPLY• If your documents are accepted to process the visa they will not be returned. The passport must be left in our office so we can process your visa. It takes a minimum of 4 weeks to process this type of visa. During this time no telephone consultations will be taken by this office regarding your visa status. If granted your visa will allow you to study in Spain for the requested period of time, as specified on the visa sticker, (“del….al….”). Keep in mind the date format in Europe: Day, Month and Year. If your stay in Spain is for a period longer than 180 days you will have to request, within one month of your arrival in Spain, an authorization for the completion of your studies as well as the proper student ID card from the Police Department in Spain ("Oficina de Extranjería").

**** SPECIAL NOTES FOR POLICE CERTIFICATES **** For the United States of America records issued by the F.B.I. or State Police Background Check are valid. Local Police Background Check will NOT be accepted. Also remember that an Apostille certification is required for Spain:

(*An apostille is a certification that a document that has been “legalized” or “authenticated” by the issuing agency through a process in which various seals are placed on the document.) Yes. The CJIS Division will authenticate U.S. Department of Justice Order 556-73 fingerprint search results for international requests by placing the FBI seal and the signature of a division official on the results if requested at the time of submission. Documents prepared in this way may then be sent to the U.S. Department of State by the requester to obtain an apostille if necessary. This procedure became effective on January 25, 2010 and will apply only to

documents finalized after that date. Requests to authenticate previously processed results will not be accepted. This procedure replaces the letter formerly provided by the CJIS Division that indicated the service was not provided. The apostille service is not provided to individuals

I declare that to the best of my knowledge all particulars supplied by me are correct and complete. I am aware that any false statements shall lead to my application being rejected or to the annulment of a visa already granted. I am aware that once the visa request has been submitted, a copy of the form, sealed with the indication of the date and place of reception, shall be returned to the applicant and that arrangements can be made with the applicant as to the channel to request corrections or the supply of documents or certificates, as well as to serve notice of summons and to notify the decision adopted in due course. Notices and requests shall be served by telephone or fax to the contact number provided by the interested party or his/her legal representative. If they were to prove ineffective, they shall be served in writing to the domicile set out in the application, which must be located within the consular district. Summons and requests served must be complied with within a period not exceeding ten days, unless personal attendance is required in which case the deadline is fifteen days. Once all the possibilities of serving a notice have been exhausted, the notice shall be served by means of an announcement posted at the relevant notice board of the Embassy or Consulate during ten days. If no reply is received to a request or summons, the applicant shall be deemed to have withdrawn his request and the resolution establishing this to be the case shall be notified to him. Decisions on a visa application shall be notified within one month of the date of the lodging of an application, except for residence visas without work permit. In this case, decisions shall be notified within three months, but the request for the residence authorisation that has to be issued by the competent Delegación or Subdelegación del Gobierno shall interrupt the computation of this period of time until this authorisation is notified to the Embassy or Consulate. The visa once granted must be collected within a month, unless it is a visa for family reunion or a visa for study purposes; in these cases, it must be collected before two months elapse. If a visa is not collected within the aforesaid period, the applicant shall be deemed to have renounced to the visa granted and the proceedings shall be filed.

I am aware that the visa fee is not refunded if the visa is refused.
