SAMPLE STATEMENT OF CONTRIBUTION
I will deliver patient care (role/ function) in
the continuity of care realm, so that with every
person I encounter, I can say that I have
helped to make their stay here better, and
ensured they can move back into the
community (goal). I determine to approach
my job with gratitude every single day because
I know I am called to serve (attitude). Also, I
will use my gifts of optimism, resilience and
compassion to brighten the day for others
(values/ talents).
I will remember to have fun and hope to meet
like-minded colleagues who will be my trusted
companions. I want to be appreciated and
recognised as someone who is loyal,
dependable, and puts others before self.
Carpe Diem!
