WarrantyInformation

FrameWarranty: DixonCorrectionalIndustriesOpticalLabwarrantsallframessuppliedbyourlabtobefreeofmanufacturingdefectsinmaterialsorworkmanshipforaperiodof180days(approximatelysixmonths)fromtheoriginalshipdatefromourlab,totheorderingfacility.ShippingchargestoreturnwarrantyeligibleframeswillbecoveredbyICI,andaprepaidshippinglabelwillbeissuedforthereturnofthebroken/defectiveframe(s).Facilitystaffmustreturnthebroken/defectiveframe(s),theexistinglenses,andacopyoftheoriginalpaperworksentwiththeorder.Framedefectsmustbebroughttoourlab'sattentionwithin�ive(5)businessdaysfromthedateofsignedreceiptbyfacilitystafforuntildispensedtothepatient,whichevercomes�irst.

LensWarranty: DixonCorrectionalIndustriesOpticalLabwarrantsalllensesfabricatedbyourlabtobefreeofmanufacturingdefects and to have been fabricatedwithinANSI standards, according to the optical order submitted. Lenses are coveredunderwarrantyforaperiodof�ive(5)businessdaysfromthedateofsignedreceiptbyfacilitystafforuntildispensedtothepatient,whichevercomes�irst.Intheeventofalaberrorinthefabricationoflenses,returnshippingchargeswillbecoveredbyICI,andaprepaidshippinglabelwillbeissued.Facilitystaffmustreturnthelensesandacopyoftheoriginalpaperworksentwiththeorder.Facilityerrorswillberemadeatthefacility,resident,orWexford’sexpense.DixonCorrectionalIndustrieswillnotpayreturnshippingchargesforfacilityerrors.
