SECTION I - INTRODUCTION AND SUMMARY OF WORK 01 INTRODUCTION The Metropolitan Washington Airports Authority (referred to herein as “MWAA” or the “Airports Authority”) is responsible for the operation, maintenance and repair of fire extinguishers and fire suppression systems located at Ronald Reagan Washington National Airport and Washington Dulles International Airport. Collectively these jobs sites will jointly be referred to as the “Airports” and herein separately referred to as “DCA” and “IAD” respectively (Appendix A). The purpose of this statement of work (SOW) is to provide inspection, maintenance and testing services of portable fire extinguishers as well as standalone fire suppression systems located throughout various Airports Authority owned buildings located at DCA and IAD.

02 SUMMARY OF WORK The Contract that will be awarded by this solicitation shall provide for the inspection, testing and maintenance of fire extinguishers and suppression systems at both DCA and IAD. The base services section of this SOW provides for monthly inspections, annual and 6-year maintenance and hydrostatic testing services for portable fire extinguishers, as well as, required inspection, maintenance and testing of fire suppression systems during the term of this contract. The supplemental services section of this SOW provides for the recharging, refilling of expended units, supply of new units and equipment (i.e., extinguishers, cabinets, signage, etc.), and technical consulting services. The contractor shall furnish all supervision, labor, materials, tools, supplies, equipment and transportation required to accomplish this SOW. The contractor shall be responsible for performing necessary technical work to ensure all specified portable fire extinguishers and fire suppression systems comply with the requirements of the most current edition of the National Fire Protection Association standards, as well as, the State of Virginia Fire Codes. The term of this contract consists of one (1) base year with the option to extend the contract for four (4) additional one (1) year periods.

SECTION II - DEFINITIONS AIRPORT – Shall refer to either Ronald Reagan Washington National Airport, aka, “DCA” or Washington Dulles International Airport, aka, “IAD” as described in the SOW. AIRPORTS – Shall mean the DCA and IAD together. AOA - Aircraft Operation Area - The portion of the Airport used or intended to be used for landing, takeoff or surface maneuvering of aircraft. This is a security area requiring a security badge for access. Workers in this area are required to obtain and display an AOA photo ID credential. Drivers in this area are required to obtain an aerodrome vehicle operator's permit. Vehicles operated in this area are required to pass a safety inspection and meet the vehicle identification marking and equipment requirements for operating on the AOA. AIRPORTS AUTHORITY - The Metropolitan Washington Airports Authority. AIRPORTS AUTHORITY ADMINISTRATIVE WORK HOURS - The hours of 7:00 AM through 3:30 PM EST/DST, Monday through Friday, excluding weekends and holidays. CALL ORDER – All supplemental services require a COTR signed call order. The call order contains a detailed description of the services to be provided, cost estimate, and schedule required from the contractor for any work performed under supplemental services. CLEAN – The absence of dirt, litter, debris, dust, surface marks, fingerprints, spills, oils, gum, grime, film, stains, streaks, spots, blemishes, chemical residue, and/or any other foreign matter or chemical residue that can be removed without permanently damaging the underlying surface. CMMS - Computerized Maintenance Management System used to schedule and generate work orders for all maintenance and repair activities performed by the contractor. CORRECTION – Elimination of a deficiency. COTR – Contracting Officers Technical Representative (COTR). DCA - Ronald Reagan Washington National Airport. DEFICIENCY– Any condition or flaw that is non-compliant with NFPA. IAD - Washington Dulles International Airport JOB SITE - The area within the Airports Authority’s property lines or portions of such area, which are defined within the contract. LITTER - Debris, waste paper, rags, dust, dirt, beverage containers, trash etc. MA-126 - DCA, Engineering and Maintenance Department, Maintenance Engineering Division. MA-226 – IAD, Engineering and Maintenance Department, Maintenance Engineering Division METROPOLITAN WASHINGTON AIRPORTS AUTHORITY (the Airports Authority) - The public body responsible for the operation and management of both Ronald Reagan Washington National Airport (DCA) and Washington Dulles International Airport (IAD).

NFPA –10 - National Fire Protection Association 10, Portable Fire Extinguishers. OSHA - U. S. Occupational Safety and Health Administration. The Federal Government agency responsible for providing the rules and regulations on safety and health requirements in the work place. PRIMARY TERMINAL OPERATING HOURS – Both DCA and IAD terminals are operational to the public 24 hours a day.

DCA: - The hours of 5:00 AM through 11:00 PM EST/DST, daily 365 days a year (includes weekend and holidays).

IAD: - 24-hours a day, 7 days a week, 365 days a year (includes weekend and holidays).

QUALITY ASSURANCE (QA) - A means by which the Airports Authority is able to confirm that the quantity and quality of services received conform to contract requirements. These methods/procedures are not intended to aid the contractor in the performance of the contract requirements and shall not be a substitute for contractor quality control. QUALITY CONTROL PROGRAM (QC) - A method used by the contractor to assure that quality services are provided that satisfy the contract requirements. SERVICES - Includes services performed, workmanship, materials and parts furnished or utilized in the performance of services. SOW – Statement of Work. STATE - The Commonwealth of Virginia.

VANDALISM - Willful or malicious abuse and/or destruction of property. WORK ORDER DESK - Unit that is primarily responsible for receiving, dispatching and tracking service requests.

SECTION III - BASE SERVICES 01 DESCRIPTION OF SERVICES The contractor shall provide Fire Protection Systems Professionals to perform inspection, maintenance and testing on Airports Authority owned fire extinguishers and 5 types of Suppression Systems and shall implement a portable fire extinguisher and safety management audit system for fire extinguishers located within various facilities located throughout Ronald Reagan Washington National Airport and Washington Dulles International Airport. The contractor shall provide all supervision, labor, materials, supplies, parts, tools, transportation and equipment necessary to perform the requirements contained herein for all Airports Authority owned portable fire extinguishers and fire suppression systems listed in Appendix B.1 (DCA) and B.2 (IAD). Special Note, IAD Specific: The portable fire extinguisher safety management audit system shall be a Bar Code system.

02 EQUIPMENT CONDITION ASSESSMENT SURVEY REPORT Upon contract award, the contractor shall perform an initial equipment condition assessment inspection at each airport of all Airports Authority owned portable fire extinguishers. The assessment inspection surveys shall include, but not be limited to, all deficiencies noted on the units as well as a listing of all fire extinguishers locations that are not identified on the equipment information list and/or the equipment location drawings. The contractor shall establish a separate stand-alone fire extinguisher data management system, visually inspect each extinguisher, extinguisher cabinet, extinguisher mount, etc. and shall report all deficiencies found on its survey report. The contractor shall also verify and mark proper extinguisher locations on the contract drawings to ensure drawings are up to date and accurate. This report will be considered as a “starting point” for providing ongoing fire extinguisher consulting recommendations. All deficiencies shall be identified with the estimated cost and priority to correct each. A report of all deficiencies found shall be submitted to the COTR within fifteen (15) working days after the start of contract.

03 PORTABLE FIRE EXTINGUISHER INSPECTION, MAINTENANCE AND TESTING The contractor shall perform all inspections, certifications, maintenance and hydrostatic testing of existing fire extinguishers. This shall include all monthly inspections, annual maintenance/certifications, six year maintenance and hydrostatic testing required by NFPA 10. The fire extinguishers range in age from newly installed to nearly twenty-five (25) years old. It is anticipated that the number of units covered under this contract will increase as new facilities are commissioned for service. A. Inspections, certifications, maintenance and hydrostatic testing shall meet the following criteria:

1. All inspections, certifications, maintenance and hydrostatic testing shall be performed in

accordance with the guidelines and requirements of the most current NFPA 10 standard for portable fire extinguishers.

2. All inspections, certifications, maintenance and hydrostatic testing shall be performed by a

qualified inspector (QI) in accordance with NFPA 10. 3. The Contractor shall provide recommendations to the COTR regarding the adequacy of fire

extinguisher coverage as well as the appropriateness of existing fire extinguisher types and sizes based on NFPA code requirements.

4. The contractor shall prepare a monthly report in Microsoft Excel format showing all work performed by the contractor, and date of installation, serial number, equipment type and size, manufacture and manufactured date, and location for new units/equipment added.

5. When performing the monthly inspections, the Contractor shall prepare and submit an exception

report in Microsoft Excel format, detailing deficiencies noted during the inspection, along with an estimated cost to correct deficiencies.

6. The contractor shall maintain Airports Authority furnished maps of the locations of all portable

fire extinguishers. Maps provided by the COTR are to be annotated by the contractor to include the corresponding fire extinguisher numbers. The contractor shall update the maps to ensure the extinguisher numbers, and locations are accurate.

7. The contractor shall, as part of the fully loaded cost of the initial equipment assessment survey,

conduct an initial survey and inventory. 8. The contractor shall follow the Airports Authority’s Computerized Maintenance Management

System (CMMS) generated preventive maintenance schedule. The Airports Authority will provide the contractor with standing work orders for the scheduled inspections, preventive maintenance and testing services at the beginning of each month. Should the contractor have recommended changes or additions to the schedule, the contractor shall notify the COTR in writing and provide complete information and justification for the changes.

9. All deficiencies discovered during the performance of inspections and preventive maintenance

shall be documented by requesting a CMMS work order.

Note: The Authority does not utilize Non-rechargeable or Non-refillable (e.g. “Discharge and Dispose of After Any Use” or “Discharge and Return to the Manufacturer After Any Use” or with a similar equivalent marking.)

B. For Portable Fire Extinguishers the following services shall be provided:

1. Initial Survey and Inventory 2. Monthly Cyclic inspections - Through inspections, the contractor shall continuously ensure that

all portable fire extinguishers: a. are mounted, located and identified so that they are readily visible and accessible; b. have operating instructions located on the front of the extinguisher and clearly visible c. fully charged and operable and kept in their designated places at all times d. are unobstructed with cabinets clearly visible e. cabinets shall be identified in an approved manner by a permanently attached sign with

letters not less than 2 inches high in a color that contrasts with the background f. the cabinets remain unlocked - Note* Cabinets may be locked if they have visual

identification panels of glass or other approved transparent material that are easily broken and allow access.

3. Hydrostatic testing services for portable fire extinguishers shall be in conformance with NFPA

10 requirements - Any time an extinguisher is pulled out of service, a same type, size and capacity extinguisher shall immediately be put in its place as a standby or spare unit.

4. Identification, removal, and off-site disposal of obsolete apparatus

5. Supply new portable fire extinguishers and related equipment (e.g. cabinets, signage, etc.)

Special Note: IAD Specific - The contractor shall, as part of the fully loaded cost of inspecting fire extinguishers, dispose off-site, any portable fire extinguisher deemed obsolete in accordance with the 2013 Edition of NFPA 10: for portable fire extinguishers, including the following:

1. Soda acid 2. Chemical foam (excluding film-forming agents) 3. Vaporizing liquid (e.g., carbon tetrachloride) 4. Cartridge-operated water 5. Cartridge-operated loaded stream 6. Copper or brass shell (excluding pump tanks) joined by soft solder or rivets 7. Carbon dioxide extinguishers with metal horns 8. Solid charge–type AFFF extinguishers (paper cartridge) 9. Pressurized water fire extinguishers manufactured prior to 1971 10. Any extinguisher that needs to be inverted to operate 11. Any stored pressure extinguisher manufactured prior to 1955 12. Any extinguishers with 4B, 6B, 8B, 12B, and 16B fire ratings 13. Stored-pressure water extinguishers with fiberglass shells (pre-1976)

Dry chemical stored pressure extinguishers manufactured prior to October 1984 shall be removed from service at the next 6-year maintenance interval or the next hydro-test interval, whichever comes first.

Note: Any fire extinguisher that can no longer be serviced in accordance with the manufacturer's maintenance manual is considered obsolete and shall be removed from service.

Special Note: IAD Specific - The “All People Mover” (APM) train tunnels require advance notice of up to two weeks or longer to inspect based on the APM maintenance closures. All inspections accomplished specifically to the APM tunnels shall occur at night after an outage request has been submitted and approved. Inspections and maintenance may take up to two (2) weeks to inspect all 118 fire extinguishers mounted on the tunnel walls. All APM tunnel extinguishers are subject to annual inspections only by approval from the Authority Having Jurisdiction (AHJ).

04 FIRE SUPPRESSION SYSTEMS INSPECTION, MAINTENANCE The contractor, without the use of a subcontractor, shall perform the required inspections, testing and maintenance on the suppression systems listed in Appendix B.1 (DCA) and B.2 (IAD). All inspections, testing and maintenance shall be in accordance with the applicable edition of the National Fire Protection Association (NFPA) standards related to the specific suppression system type; and the Commonwealth of Virginia Fire Codes. Inspection and maintenance requirements shall, at a minimum, conform to the following criteria:

a. All work shall be performed by a qualified inspector. b. When performing inspection or maintenance, the contractor shall prepare and submit an

exception report in Microsoft Excel format, detailing deficiencies noted during the maintenance, along with an estimated cost to correct deficiencies.

c. The contractor shall follow the Airports Authority’s Computerized Maintenance Management

System (CMMS) generated preventive maintenance schedule. The Airports Authority will provide the contractor with standing work orders for the scheduled inspections, preventive maintenance and testing services at the beginning of each month. Should the contractor have

recommended changes or additions to the schedule, the contractor shall notify the COTR in writing and provide complete information and justification for the changes.

d. All deficiencies discovered during the performance of inspections and preventive maintenance

shall be documented by requesting a CMMS work order. 05 IAD ONLY - Safety Management Audit System (Bar Coding) The Contractor shall create and establish a separate stand-alone fire extinguisher data management system that can be managed by the Authority and the Contractor. This system created for the Authority shall be turned over to the Authority at the conclusion of the contract, and, shall be in such standing that a new contractor can take control and manage. In accordance with the following NFPA statement, "Records shall be kept on a tag or label attached to the fire extinguisher, on an inspection checklist maintained on file, or in an electronic system (e.g., bar coding) that provides a permanent record of the last twelve monthly inspections" the Airports Authority has elected to have the contractor provide all necessary supervision, labor, materials, parts, tools, supplies, chemicals, equipment, and transportation required to acquire and operate a commercial off the shelf computerized Safety Management Audit System program utilizing Bar-Coding to audit and manage the identification, maintenance, inspection, and testing frequencies and reporting in an inclusive database system in an effort to ensure that all facility portable fire extinguishers are accounted for and maintained in a serviceable condition. This systems shall be used by the contractor to perform:

1. updating of equipment inspection, and service dates in the database with the next date

scheduled based on the type of equipment and kind of test 2. addition of equipment on the fly based on model, serial number, and location using the barcode

reader. 3. contain model number, size, type, manufacturer, and category information derived from pre-

stored model number 4. utilization of sequentially numbered barcode labels 5. selectable reports identifying what equipment is due for inspection and/or outside service 6. consolidated reports based on size and type 7. database tracking of;

a. General Location b. Actual Location c. Equipment Manufacturer d. Size e. Type f. Serial Number g. Last Hydrostatic test Date h. Last 6 Year Maintenance Date i. Service Performed Cyclically j. Date Performed k. Historical Data

The system shall have the capability for tracking and auditing of the following types of equipment for expandability:

SECTION IV - SUPPLEMENTAL SERVICES 01 DESCRIPTION OF SERVICES A. The Airports Authority may, during the course of this contract, request that the contractor perform

supplemental services which are outside the requirements of the base services section. Supplemental services provide for the recharging/refilling of expended units, supply of new units and equipment (i.e., extinguisher, cabinets, signage, etc.), performing additional testing, as required, for suppression systems, as well as technical consulting services. Rates to provide these services shall be fully loaded with the exception of the materials used to refill units. Reimbursement for the materials used shall be provided at the per pound rate. All purchased materials not listed on the price schedule shall be invoiced at the Contractor’s cost plus applicable Contractor markup as defined in the Schedule (i.e., smoke or heat detectors, new fire extinguishers, installation of fire safety related equipment services, etc.).

B. The contractor shall be required to provide the Airports Authority with fire extinguisher consulting

services, as needed, for the proper inspection, service and upgrade as well as ensure safety code and regulatory compliance of all Airports Authority owned fire extinguishers and suppression systems at Ronald Reagan Washington National Airport and Washington Dulles International Airport.

C. The contractor shall provide all supervision, labor, administrative support, materials, tools, parts,

supplies, equipment, and transportation necessary to perform these services. Such work shall be compensated at the rates listed in the contract price schedule.

D. IAD ONLY: In conjunction with Base Services, Item 6: “The contractor shall maintain Airports Authority

furnished maps of the locations of all portable fire extinguishers”, the Contractor shall provide Computer Aided Digitized Design (CADD) services to annotate fire extinguisher locations campus-wide. File format shall be capable of creating .pdf files for distribution. CADD files shall be provided to the COTR in electronic format (CD, memory stick, or newer applicable technology formats that may be relevant).

E. The Authority shall retain the right to increase or decrease base services and supplemental services

described in this SOW and Cost Schedule. F. The Airports Authority shall incur no obligation for out of scope work that is not authorized in advance,

by a Call Order.

02 CONTRACT SERVICES CALL ORDER A. All supplemental services shall be requested and approved in advance by the COTR using the

“Contract Services Call Order” form shown in Appendix C. The call order will contain a detailed description of the services that are required from the contractor. The contractor shall provide the COTR a detailed cost estimate including an itemized breakdown for all labor, parts and materials and shipping as well as a schedule with critical milestones for completing the work to be listed on the call order. All Service Call Orders (SCO) in excess of $10,000 shall be countersigned by the Contracting Officer.

B. Labor rates included on the contract price schedule for the contract will be used in preparing these

estimates. Both the cost breakdown and schedule shall be made part of the call order. The contractor shall not proceed with any work described in such call orders until authorized in advance and in writing by the COTR.

The contractor, when and as requested by the Airports Authority, will perform various fire extinguisher technical consulting tasks at DCA and IAD to include but not be limited to the following:

A. Review and markup drawings as required for any recommended system modifications to ensure

complete, expedient, and cost effective compliance with all applicable safety code and regulatory requirements (federal, state, local, NFPA, etc.) including American with Disabilities Act (ADA), and OSHA. Contractor shall identify all deficiencies together with recommendations for appropriate remedial actions and their estimated implementation cost.

B. Identify, evaluate and/or recommend the application of current state-of-the-art technology which would improve the performance, safety, and/or effectiveness of the operation, maintenance, repair and improvement of existing and/or proposed portable fire extinguishers.

C. Monitor and/or perform acceptance inspection of new portable fire extinguishers installations as a part

of on-going capital development program (CDP) or repair and rehabilitation (R&R) projects. Perform on-site inspections to ensure compliance with construction documents, manufacturer’s specifications, industry standard and/or warranty requirements.

D. Unless otherwise directed by the Airports Authority, the contractor shall provide the Airports Authority

an accurate written estimate of the cost and time for the contractor to complete each of the tasks requested by the Airports Authority. These estimates are to be provided by the contractor to the Airports Authority within five (5) business days from the request.

E. At completion of each consulting call order, the contractor shall prepare and submit a report to the

COTR to include any applicable CADD drawings.

SECTION V - GENERAL REQUIREMENTS 01 CERTIFICATIONS The contractor shall, without additional expense to the Airports Authority, be responsible for obtaining all licenses and permits which are necessary to fulfill the requirements of the SOW. 02 RESPONSE TIME The contractor shall be required to provide 24-hour emergency services at the airport, 365 days a year throughout the period of the contract. The contractor shall take necessary action as directed by the COTR within 24 hours after receiving verbal or written notification of any portable fire extinguisher or fire suppression system problem. A work order shall be provided that the contractor shall complete and return when the prescribed job is completed. 03 CHECK-IN/CHECK-OUT REQUIREMENTS A. Contractor employees shall at all times, immediately upon arriving to the job site; check-in with the

Airports Authority’s Work Order Desk and immediately prior to their departure from the job site shall check-out with the Airports Authority’s Work Order Desk.

B. During afterhours callbacks; the contractor employees shall check-in and inform the Work Order Desk

as to the purpose of the visit, during check-out the contractor shall apprise the Work Order Desk of the work that was performed.

04 STATUS MEETINGS The contractor’s representative(s), including the project managers and the on-site technicians shall attend all meetings as required by the COTR to perform inspections, discuss, coordinate and evaluate the status and performance of services under this contract. These meetings will be held on a monthly basis or as frequently as the COTR deems it necessary.

05 QUALITY CONTROL PROGRAM The contractor shall implement an effective quality control program. This program shall ensure the contractor fulfills all the requirements of this SOW. This program shall include but not be limited to all elements of the quality control program described in the technical proposal that the contractor submitted in response to the Airports Authority's solicitation for this contract.

06 COMMUNICATION AND COORDINATION The contractor shall maintain an effective communication and coordination policy with the Airports Authority utilizing email, telephones, scanners, faxes, text messaging devices, etc. to ensure the Airports Authority is kept up to date of current equipment status, scheduled and unscheduled outages, vandalism etc. for the term of the contract. 07 LOST AND FOUND PROPERTY The contractor shall immediately turn in to the Airports Authority’s Lost and Found Department all property found on the job site. Any violations or disregard of the rules, regulations and/or policies may be cause for immediate termination.

08 SAFETY A. The contractor and each of its employees shall comply with all applicable local, state, federal and

Airports Authority rules, regulations and practices.

B. The contractor shall be held responsible for all injury to persons or damage to property that occurs as a result of the contractor’s negligence and shall take proper safety and health precautions to protect the work, the workers, the public and the property of others. The contractor shall be responsible for all materials delivered and work performed until completion and acceptance of the entire work.

C. Contractor shall perform all its activities pursuant to this contract in a safe manner. The contractor shall

assume responsibility, on the job site, for the actions of all its personnel and subcontractor(s) who are associated with performance on this contract. The contractor shall take adequate measures to prevent injury to the public or Airports Authority property on the job sites.

D. The contractor shall provide and ensure that all its personnel at the job site properly wear all applicable

safety devices and apparel. Safety devices and apparel shall be provided by the contractor at no cost to the Airports Authority.

E. The Airports Authority shall have the right to inspect all areas for safety violations at its discretion, direct

the contractor to make immediate improvements to conditions and/or procedures, and/or stop the work if other hazards are deemed to exist.

F. Notwithstanding any provision to the contrary, the Airports Authority shall not be obligated to make an

equitable adjustment for any work stoppage that results from safety hazards created by the contractor.

G. The operation of the contractor’s vehicles or private vehicles by the contractor’s employees on or about the Airport shall conform to regulations and safe driving practices. All vehicles that are to be operated on the AOA shall comply with FAA regulations and be subject to inspection. Operators of these vehicles shall be required to pass an AOA operators test administered by DCA Operations.

H. It shall be the responsibility of the contractor to immediately notify the COTR if the job site is visited by

an official authorized to enforce any regulatory requirements including but not limited to the occupational safety and health act.

I. The contractor shall provide and use adequate barricades and signs to provide sufficient notice of

potential safety hazards prior to, during, and after the performance of services.

J. The contractor shall inspect all powered equipment daily before operations for signs of wear as well as potential safety hazards. The contractor shall immediately remove from service all equipment which is potentially unsafe, damaged, inoperable and/or do not meet the manufacturers operational specification.

K. The contractor shall at no time leave power equipment unattended without disconnecting it from the

power source. 09 SECURITY REQUIREMENTS A. The contractor shall ensure all staff while on the job site visibly display at all times a valid Airports

Authority security badge. The contractor shall provide the COTR with a list of employees, a minimum of (2) two, who will be applying for security badges fifteen (15) days prior to the start date of the contract. All contractor employees working at the jobsite shall obtain a security badge, at the

contractor’s own cost, no later than thirty (30) days from the award of the contract. The contractor shall secure and safeguard all Airport security access devices (including but not limited to ID badges, security cards, keys, key cards).

B. The contractor shall not permit any employee to have keys for access to locked rooms until it has

been determined that permitting such person(s) to have such access shall not be contrary to the Airports Authority’s interest, and that the individual(s) is authorized to be admitted in accordance with applicable security orders, rules, regulations and instructions as determined by Airports Authority.

C. The contractor shall secure and safeguard all keys, key cards, and any other entry devices and codes

provided by the Airports Authority. The contractor shall maintain a record of the key numbers issued to its employees. These prohibitions and requirements shall also be applicable to all individuals with regard to access, removal, and/or possession of any information, confidential data, materials, supplies, or equipment. The contractor shall not duplicate and shall not allow any such issued items to be duplicated or removed from the job site. All keys and other entry devices used by the contractor’s employees in the performance of the work shall be returned to the Airports Authority when the contract expires.

D. The contractor shall safeguard all the identification cards, issued its employees and subcontractors by

the Airports Authority to fulfill the requirements of this SOW. The contractor shall ensure these cards are visibly displayed by the respective individual at all times while employed on site at the Airport to fulfill this requirement.

E. The contractor shall immediately report to the Airports Authority all keys issued to it by the Airports

Authority that are lost or stolen. The contractor shall be responsible for all loss, damages and expenses that the Airports Authority incurs as a result of the contractor’s loss of Airports Authority keys. These costs include the expense of changing all locks to which the lost keys provided access. The cost of changing locks or keys to the building rooms or areas accessible by the lost or stolen keys will be deducted from the contractor’s invoice to the Airports Authority for the services performed under the contract.

F. The contractor shall ensure that, under no circumstances any of its employees shall enter an area not

authorized for access by the contractor. G. The Airports Authority will not issue keys for sensitive security areas. Instead, the Airports Authority will

require the contractor’s employees to be accompanied at all times in this area by an Airports Authority employees.

H. The contractor, its subcontractors, and all its employees shall be subject to, and shall at all times,

conform with any and all rules, regulations, policies, and procedure pertaining to security at the Airport. Any violations of the rules, regulations, policies, and procedures may be cause for immediate removal from the job site.

I. The contractor shall be responsible for, at its own expense, obtaining the proper security clearance,

fingerprinting, training, badges required to access the restricted areas of the Airport including the Air Operations Area (AOA), Security Identification Display Area (SIDA). Identification badges issued by the Airports Authority must be visibly worn at all times while in the SIDA. At a minimum, the contractor shall have two individuals SIDA badged and certified to perform services at the airport, one acting as the primary service representative and the other acting as an alternate.

J. The contractor shall be responsible for, at its own expense, compliance with the requirements and

procedures to obtain approval of any motor vehicle to operate in the Air Operations Area (AOA).

K. The contractor shall not leave any tools, parts and/or supplies unattended in the public area at any time

while performing work at the Airport. 10 ACCIDENT NOTIFICATION The contractor shall immediately notify in turn the Airports Authority’s police department and the COTR both verbally and in writing of any accident on the job site which is related to the performance of this SOW that involve bodily injury or damage to property. The COTR shall provide information necessary concerning whom to contact and the specific form of the follow-up written notice.

11 SMOKE FREE ENVIRONMENT The Airports Authority’s facilities are smoke free. The contractor and its employees shall adhere to all applicable rules and regulations regarding maintenance of a smoke free environment on the job sites.

SECTION VI - REGULATORY REQUIREMENTS 01 PERMITS AND RESPONSIBILITIES A. The contractor shall, without additional expense to the Airports Authority, be responsible for obtaining

and renewing all necessary licenses and permits. The contractor shall also be responsible for all damages to persons or property that occur as a result of the contractor’s negligence and shall take proper safety and health precautions to protect the work, the workers, the public and the property of others. In addition, the contractor shall be responsible for all materials delivered and work performed until completion and acceptance of the entire work.

B. The contractor shall comply with all applicable revisions, additions, changes and/or upgrades to any

Federal, state, and municipal laws, codes, and regulations which are in effect on the date of contract and which affect the performance of the work. The contractor shall also obtain and pay the costs of any royalties and licenses for any patented or copyrighted items used in the performance of the work.

C. It shall be the responsibility of the contractor to promptly notify the COTR if an official in charge of

compliance with the Occupational Safety and Health Administration visits the work site.

SECTION VII - DOCUMENTATION AND REPORTING 01 COMPUTERIZED MAINTENANCE MANAGEMENT SYSTEM (CMMS) A. The contractor shall document all work performed using the Airports Authority’s Computerized

Maintenance Management System (CMMS) which will schedule and generate work orders for all preventive maintenance and repair activities performed.

B. The CMMS will generate preventive maintenance work orders each month and repair work orders when

requested. The contractor shall complete work orders for all work performed (i.e. inspections, service call backs, vandalism/abuse and corrective maintenance repairs). Upon completion of work the contractor shall be responsible for returning the completed work order to the work order desk. The information to be entered by the contractor shall include, but not be limited to; name of contractor’s employee(s), date of service, duration of work performance, specific work accomplished, date the work was completed and comments explaining the corrective action or other work performed.

02 REPORTING A. The contractor shall provide an initial equipment survey report as required in this SOW, updating the

existing data that will be provided by the COTR. This initial equipment survey report shall be in a format acceptable to the COTR and shall include; notating drawings provided by the COTR; depicting equipment locations, serial numbers, date of manufacture, size and type of equipment, and manufacturer. The contractor shall include the next due dates for required maintenance and testing of each unit. This report shall be provided to the COTR in Microsoft Excel format and shall be submitted to the COTR no later than the fifteen working days after the start of the contract.

B. Contractor shall provide a monthly Microsoft Excel report identifying equipment inspected or maintenance performed. This report shall also identify all deficiencies discovered during the past month along with recommended corrections and estimated cost to perform corrections. The contractor shall provide serial numbers, dates and locations for all new units installed at the Airports Authority.

SECTION VIII - PERSONNEL 01 KEY CONTRACTOR PERSONNEL The contractor shall provide the Contracting Officer and COTR with a list of names and telephone numbers of the key personnel who will fulfill any part of the requirements of this SOW including responding to emergency callbacks. This list shall include the job title, duties, responsibilities and authority of the contractor’s personnel. This list shall be provided to the Contracting Officer and COTR fifteen (15) days prior to the start of the contract and shall be updated when approved changes are made. 02 PERSONNEL The contractor shall utilize responsible, capable employees in the performance of all services of this contract. The contractor shall provide all the management, supervision, labor, and administrative support necessary to fulfill the requirements of the contract. 03 PROJECT MANAGER The name of the Project Manager, and name of the alternates who shall assume the Project Manager duties when the primary Project Manager is absent, shall be provided in writing to the COTR fifteen (15) business days prior to the contract start date. The Project Manager shall be available for calls 24 hours a day, seven (7) days a week. The Project Manager shall meet as promptly as possible with the COTR at the COTR’s request to discuss the performance of the work or other provisions of the contract. 04 CONDUCT The contractor’s employees at all times while on the job site, whether on or off duty, shall conduct themselves in a professional, orderly and safe manner. Rudeness, fighting, being under the influence of alcohol and/or drugs or bringing and/or consuming alcohol and/or drugs, gambling, soliciting, stealing, and any immoral or otherwise undesirable conduct shall not be permitted on the job site and shall result in immediate and permanent removal from the job site of any employee engaging in such conduct.

SECTION IX - DELIVERABLES The contractor shall submit the following items to the COTR: FIFTEEN (15) BUSINESS DAYS PRIOR TO CONTRACT START DATE

01 CONTRACTOR INFORMATION After notification of the contract award but not less than fifteen (15) business days prior to the start of the contract the contractor shall submit to the COTR:

A. A list of the names of all the contractor’s employees as well as subcontractors and their employees who

will fulfill any part of the requirements of this specification. This list shall include the job title, their duties, responsibilities and authority.

B. A list of the cell phone numbers for the contractor’s employees who will fulfill the requirements of this

contract. C. Contractor’s email address. D. Documentation that the contractor has received all required airport security badges and permits

necessary to fulfill the requirement of the SOW. This documentation shall include legible photocopies of all AOA security badges and AOA operator’s permits issued by the Airport.

E. Alphabetical lists and copies of materials safety data sheets (MSDS/SDS) for all chemical products the

contractor will use on the job site. This information shall be contained in a loose leaf binder in alphabetical order according to the common name of the chemical. The alphabetical lists shall be by (1) the common chemical and (2) trades names for each MSDS/SDS.

FIFTEEN (15) BUSINESS DAYS AFTER AWARD

02 EQUIPMENT CONDITION ASSESSMENT REPORT Upon contract award, the contractor shall perform an assessment inspection of all Airports Authority owned portable fire extinguishers. The contractor shall visually inspect each extinguisher, extinguisher cabinet, extinguisher mount, etc. and shall report all deficiencies found on its survey report. The contractor shall also verify proper extinguisher locations on the contract drawings to ensure drawings are up to date and accurate. This report will be considered as a “starting point” for providing ongoing fire extinguisher consulting recommendations. All deficiencies shall be identified with the estimated cost and priority to correct each. Report shall be submitted to the contracting officer’s technical representative (COTR) within fifteen (15) working days after the start of contract.

MONTHLY

03 REPORTING Contractor shall provide the COTR a Microsoft Excel formatted report which indicates equipment inspected, maintenance performed and deficiencies found, no later than the 7th day of each month.

SECTION X - CONTRACTOR FURNISHED RESOURCES 01 GENERAL The contractor shall furnish all supervision, labor, administrative support, materials, tools, parts, supplies, equipment, and transportation necessary to fulfill all the requirements and satisfactorily perform all services described in this SOW in a safe, orderly, timely, efficient and workmanlike manner. The contractor shall provide any additional resources necessary to fulfill the contract requirements at no additional cost to the Airports Authority. 02 SAFETY EQUIPMENT The contractor shall provide all safety equipment/devices, personal protective equipment and clothing as required for its employees. 03 COMMUNICATION EQUIPMENT A. The contractor shall, at its own expense, provide fully operational cellular telephones to key personnel

at all times that they are on the job site to fulfill the requirements of this SOW. The contractor shall provide the assigned telephone numbers for this equipment to the COTR at least fourteen (15) business days prior to the start of the contract. The contractor shall notify the COTR of any changes in these assignments.

B. The contractor shall have email capability. The contractor shall provide its email address to the COTR

no later than fifteen business days prior to the start of this Contract. C. The contractor shall provide and utilize on the job site a digital camera with date and time functions. 04 SERVICE VEHICLE The contractor shall provide at a minimum one (1) service vehicle, which is equipped, licensed, insured and AOA inspected. The service vehicle shall be on site anytime the contractor’s employees are working at the Airport. The service vehicle is required to transport materials and supplies, contractor’s employees and tools to various locations at the Airport.

SECTION XI - AIRPORTS AUTHORITY FURNISHED RESOURCES 01 PARKING A. The Airports Authority will provide at no cost to the contractor one (1) parking space for the on-site

service vehicle. B. Visitor parking spaces will be provided at no charge for the contractor to attend meetings, conferences

and conduct contract related business at the East Building and Airports Authority Corporate Office Building. Visitor passes for this space shall be obtained from the receptionist desk.

SECTION XII - METHOD OF PAYMENT The contractor shall submit one invoice on a monthly basis for services that have been completed to the satisfaction of the COTR. The contractor’s invoice shall be submitted to Accounts Payable MA-22B no later than the 10th day of the following month. Monthly invoices shall be itemized to provide a breakdown of cost for all contract services according to the following: 01 BASE SERVICES The contractor shall invoice the Airports Authority for actual number of units serviced per month at the end of the calendar month during which the work was performed in accordance with the provisions contained in the contractor’s price proposal (schedule). The invoice shall state the period when the services were provided and the equipment numbers for all items inspected or serviced. The line item rates contained in the contractor’s price proposal (schedule) shall be fully loaded rates inclusive of all labor, materials, parts and supplies necessary to ensure equipment is compliant with all applicable regulations. 02 SUPPLEMENTAL SERVICES A. Supplemental services shall be invoiced to the Airports Authority, in full, at the end of the calendar

month in which the respective contract services call order is completed by the contractor. Unless approved in writing by the COTR, the contractor shall not invoice the Airports Authority for supplemental services for an amount greater than the estimated cost given in the COTR approved contract services call order.

B. Original invoices for supplies and materials not identified on the contract schedule that are requested

by the Airports Authority, and purchased by the contractor, shall be submitted to the Airports Authority with contractor’s invoice. The contractor shall make a reasonable attempt to pay only the lowest prices that can be obtained by the contractor for reimbursable items. The Contracting Officer shall have the option to require the contractor to obtain competitive bids from a minimum of three (3) sources on any single item or group of items that may exceed $300.00 in total cost. The contractor shall not charge more than the percentage markup stated in the contractor’s price proposal (schedule) for Airports Authority requested supplies or materials. Contractor shall not invoice for any item that has not been requested in writing by the CO and/or the COTR. There shall be no markup allowed for shipping and handling costs.
