Step2:SelectPaymentType(CreditCardorOfflineDonation)

Youmayusethissitetomakeyourcontributionviacreditcardandrequestamatch.Todoso,pleaseenteryourcreditcardinformationandfollowthestepsbelow.Pleasenotethatyoucannotmakeacreditcarddonationtoapublicschool(e.g.,elementary,middle,orhighschool);youmustmakeanofflinedonationtothesetypesofpublicschools.Ifyouhavealreadymadeyourcontribution,pleaseseetheinstructionsonthenextpagetorequestamatchforyourofflinedonation.

Step3:AddaCharityChoosethe“Selectfrompre-approvedlist”toopenthecharitysearchboxtosearchallpre-approvedcharities.Pleasenotethatthispre-approvedlistismadeupofallofthecharitiesthatAbbottFundhasmatchedtooverthepast3years.

Step4:SearchandSelectaCharitySearchforyourcharityinthetopleftsearchbarandselect“Add”whenyoufindyourcharity.Youmayalsoselect“Advanced”andsearchbycity,state,employeridentificationnumber(EIN),schoolcode,orkeyword.Ifyoucannotfindyourcharity,select“Nominateacharity”andfollowtheNominationProcessdescribedbelow.

NominateaCharityYoumaychoosethe“SelectCharity”buttonandsearchfromtheentirenonprofitdatabaseof1.3millionnonprofits.Alternatively,youmayinputthecharity’sinformationintheindicatedfields(e.g.,LegalName,TaxID/EINorNCESSchoolID,orMailingAddress).Select“SaveandNext.”

ConfirmandSubmitYourNominationPleaseanswertheCertificate&Confirmationquestionsandsubmityournomination.Onceyousubmityournomination,youwillreceiveanemailconfirmingyournomination.YournominationwillbereviewedforProgrameligibilitywithin7-10businessdaysandyouwillreceivenotificationastowhetheryournominatedorganizationwasapprovedordenied.Ifyourorganizationisapproved,youareinvitedtogoonlineandsubmitamatchrequest.

Step5:EnterDonationandMatchAmountsEnteryourdonationamountandreviewthematchamountfield.Youmayalsoenterinstructionsanddedicationstotherecipientcharity,ifyouwouldlike.
