• REGIONAL PLANNING- 3 x Trusts, 9 LHBs, Primary Care, NPHS, LAs, Voluntary sector, patients & users

• LOCAL PLANNING- Trusts, LHBs, Primary Care, NPHS, LAs,

Voluntary Sector, patients & users

REGIONAL PLANNING FUNCTIONS

• Links stakeholders in region across primary secondary and tertiary care;

• Develops clinical consensus across health care system and pathway development;

• Advise Trusts, LHBs and HCW on planning & commissioning;

• Monitor progress against national & local standards;

• Ensure streamlined care for the whole patient pathway

• Ensure adequate patient & public involvement

REGIONAL CONTD• Accountability

South East CE’s Group, linked to the South East Regional Commissioning Exec Board via LHB CE’s;

• Work Programme- Develop & implement model of care for acute stroke across SE Wales;- Agree model of care for TIA – infrastructure, workforce, training, standards;

REGIONAL CONTD• Rehabilitation & Discharge

• Public & professional awareness & education- Coordinated approach to public and health awareness in association with National Stroke Partnership and vol sector;- Identify training needs and education programmes for professionals;

LOCAL PLANNING FUNCTIONS• Collaborative of professionals working

across local organisations – Trusts, LHBs, Primary Care, NPHS, LAs, voluntary sector;

• Agree & develop local Stroke Care Improvement plans and priorities;

• Develop clinical consensus across health system and patient journey;

• Monitor progress against agreed national & local standards;

• Ensure streamlined care for whole patient journey;

• Ensure adequate patient & public involvement;

LOCAL PLANNING FUNCTIONS

• AccountabilityCorporate planning functions within Trusts, LHBs and LAs;

• Work ProgrammeAs Regional but localised though local Stroke Improvement Plans;
