Step 1: Entering information into the Athletics Database To
access the database, go to Inside Athletics off the VC athletics
main page: http://www.vassarathletics.com/ From here, enter your
999 id along with your Banner PIN This is where you will enter
demographic information, emergency contacts and insurance
information. This database will serve as the place where your
coaches will be informed as to the paperwork you have
completed

Step 2: Entering information into ATS/ Sports Medicine Database
To access the database, go to: vassar2.atsusers.com which will
bring up the following login screen:vassar2.atsusers.com Returning
Student Athletes should enter their 999xxID and the password 1234
New Student Athletes will enter the Athlete ID: new and Password
new

Step 3: Completing Necessary ATS information & Forms Once
you have accessed the ATS database you can now enter or update
General/Athlete Information in the screen to the bottom right If
you are a returning user, initially you will see the top right
screen and need to click on Athlete information in order to get to
the bottom right screen All yellow fields are required Enter your
999xxxxxx ID for Athlete ID You can create a unique password For
any of the required fields that do not apply to you, just enter N/A
Verify the athlete information if no changes have been made, OR
Save the athlete information if you have entered new
information

Step 3: Completing Necessary Insurance Information in ATS Next
you should proceed to the INSURANCE tab and enter the following:
Proceed to +Add an insurance if you do not have one listed or if
the one listed is incorrect. Upload a copy of your insurance card
here or under the efiles tab. Save your card as, Gallagher Ins.
Card 2015 (example: substitute your ins. Name) Student Athletes who
are on the VC Student Health Plan can access a copy of their card
from the Gallagher Koster site at: gallagherstudent.com/vassar

table to know which forms are required for you You can access all
necessary forms under the Forms tab:

Step 3: Completing Necessary ATS Forms: Helpful Hints Once you
locate the form you need to fill out, click new and fill out the
form in its entirety. Each form requires a typed name and signature
before saving. You cannot save unless all questions are answered.
You can sign using your touchpad or a mouse. If you are under 18, a
parent or guardian signature is required If you are over 18, you
may sign, N/A in the parent signature blank and type your date of
birth in the box below it. You must click on Sign for each
completed signature. You will notice it is then digitally
accepted

Step 3: Completing Necessary ATS Forms: Helpful Hints Please be
sure your parents read and understand the Secondary Insurance
Statement of Understanding form It is VERY important, as many of
you will be participating while still being covered under your
parents primary insurance. For more information on our Secondary
Insurance Coverage and Policies, you can visit the Sports Medicine
page off the Athletics main site under Inside Athletics

Step 3: Completing Sickle Cell form and NCAA compliance The
NCAA requires that all Student Athletes verify Sickle Cell Trait
status PRIOR to participation. NCAA compliance can be obtained in
one of the following: 1.Report SCT status and provide written proof
of testing 2.Request SCT testing (blood test) from a physician
Athletes arriving prior to classes must do this at home 3.Waive
testing and complete additional education and provide a certificate
of completion from ssilearn.orgssilearn.org

Step 3: Completing ADD/ADHD Medical Exceptions If a student
athlete is being treated for ADD/ADHD they must provide additional
medical documentation, as many of the medications associated with
these disorders are banned by the NCAA. With Medical Exception
documentation on file with the Sports Medicine office (must be
updated annually), the student athlete is able to apply for a
medical exception when submitting a drug test for the NCAA Student
Athletes can request further information and guidelines for this
from the Director of Sports Medicine, Susan Higgins;

Step 4: Physical Exams and Concussion Baseline Testing Once
Student Athletes arrive on campus they will be required to have a
full physical examination prior to participation Some students
athletes in high risk sports or with a previous history of
concussions will also undergo baseline ImPact Concussion
Testingbaseline ImPact Concussion Testing The sports which will
receive automatic baseline testing are: SOCCER, RUGBY, FIELD
HOCKEY, VOLLEYBALL BASKETBALL, LACROSSE, BASEBALL, DIVERS Anyone
else with a previous history of concussions will be required or can

Injury Reporting and Insurance Policies Athletics Department
Insurance is a Secondary Policy Covers injurys that occur during
organized athletic events ONLY ! Report Injuries to your ATCs so
they can be documented !! Have your Insurance card on you at all
times Reporting Report Injuries to VC Sports Medicine and/or Host
Institution medical staff Medical Staff will determine appropriate
care If referred to a provider: Its the Students Responsibility to
present both Primary and Secondary Insurance Information Dont
Ignore bills with an unpaid balance !!
