Behandlung des metastasierten Pankreas-CA
• Überleben ohne adäquate Therapie: 3-4 Monate
• Zugelassene Medikamente:– Gemcitabin– Erlotinib
• Überleben mit Chemotherapie: 6-8 Monate
• Primäre Behandlungsziele:– Symptomkontrolle– Verlängerung des Überlebens
Spiegeln Studien die klinische Realität wider?
Patienten in klinischen Studien waren jünger, hatten einen besseren sozioökonomischen Status und waren weniger häufig “African Americans“

der Pankreas-CA Patienten? Welche Rolle haben Gemcitabin oder Gemcitabin/Erlotinib? Was ist der Standardarm in zukünftigen klinischen Studien? Ist eine Bestätigung dieser Studienergebnisse notwendig?

• Vergleichbare Effektivität von Cap + Erlotinib → Gemund Gem + Erlotinib → Cap
• Eine Gemcitabin-freie 1st-line Therapie ist effektiv
• Eine 2nd-line Chemotherapie wurde bei 51% der Patienten eingesetzt und sollte angeboten werden
• Patienten ohne Hautexanthem (34%) haben eine schlechte Prognose und bedürfen neuer Behandlungsoptionen

1st-Line Therapie des met. Pankreas-CA
• FOLFIRINOX– Alter <75 Jahre– PS 0-1– Bilirubin <1.5 UNL (keine Gallenweg-Stents?)– keine relevante Komorbidität (kardiovaskular)
• Gemcitabin + Erlotinib– alle anderen (vorausgesetzt ECOG ≤2) ?
• Keine Behandlung ?– Schlechter Performance Status (KPS<70%, ECOG>2)
2nd-Line Therapie
• Indikation– Progression der Erkrankung (Nachweis durch Bildgebung)
– kein CA19-9 Abfall innerhalb der ersten 2 Therapiezyklen ?
– kein Rash unter Erlotinib während der ersten 8 Wochen ?
– fortgesetzter Gewichtsverlust ?
– fehlende Schmerzkontrolle ?
• Einsatz (wenn möglich)– Versagen von FOLFIRINOX → Gem (+ Erlotinib?)
– Versagen von Gem (+ Erlotinib) → 5-FU/Cape (+ Oxaliplatin)
Supportive Therapie• Gewichtsstabilisierung
– Enzymsubstitution– Parenterale Ernährung sinnvoll ?
• Prophylaktische Antikoagulation– Reduktion von Thrombosen- und Lungenembolien
– Keine Verbesserung von ORR, TTF und OS
