SUBMISSION OF END OF SCHOOL YEAR REPORTS AND SCHOOL FORMS

The Schools Division Office of Palawan hereby call the submission of End of School Year Reports

and School Forms. Schools and Districts shall follow the procedure on submission stated in this

memorandum.

2. Procedure on submission of required reports and school forms.

For Public Schools:

Report/ School Form Grade Level Instruction

School Form 5 -Kindergarten

Kindergarten (SFS-K) ► LIS Generated/ Accomplished School forms

must be in accordance to DepEd Order 58,

S. 2017.

School Form 5 Grades 1 - 10 ► SFs must be checked and signed by the

school head and all other concerned

signatories.

► SFs shall be submitted in hard copies to the Grades 11 - 12 district office filed by school. The District

School Form 5 - A (1 '1 and 2nd Head shall submit to the division office

► Submitted SFs must be clearly printed and

clean for it will be scanned for future use.
