Submitting USPS Form1583Step-by-Step Instructions

MyUS must have USPS Form 1583 on file for you to receive mail on your behalf. The Application for Delivery of Mail Through Agent (Form 1583) demonstrates that MyUS is authorized to receive and forward mail for Premium +Mail members.

•You must submit a Form 1583 for every name on your Premium +Mail account.

•Each Form 1583 must be notarized by a notary public, the US Embassy or US Consulate.

•You must submit two forms of photo identification with each Form 1583.

Acceptable photo identification includes passport, drivers license, state identification card, student identification, government employee ID, corporate ID from a place of business, current lease or mortgage, voter registration card, and home or vehicle insurance policies.

Submit your USPS Form 1583 in five simple steps:

Login to your MyUS account and download USPS Form 1583 by going to My Account, then Account Documents. Click Download Forms to save the form to your computer. You can select a blank form, or one that is prefilled with contact information for the name shown.

3. Have your USPS Form 1583 notarized.Your local notary public, the US Embassy or US Consulate can notarize your completed Form 1583. Your form must be notarized before submitting it to MyUS.

5. Mail the original Form 1583 to MyUS

In addition to uploading your completed form(s), you must also mail the

original notarized Form 1583 to MyUS:
