• Call 5555–The 5555 number goes to the
receptionist, who can contact the correct people for the emergency

• Issue is placed into Closed Loop Corrective Action (CLCA) system and tracked until it is closed
• Employee who submitted the issue receives e-mail every time the CLCA issue is updated until it is closed

Injury/Illness Reporting
• Reporting is done using MedGate• The supervisor along with the employee enters the information within 24 hours of
the injury/illness• HSE reviews and adds information as needed• The information is then passed along to the HSE and Medical groups in
Rochester where the case is managed

• Click on the MedGate Link (https://aspcl2.medgate.com/ekodak55public)
• The first page of MedGate is where you enter the employees last name and Global ID number on the first line (see next slide)
• Then hit enter• Once you have entered the correct name in the top line you will notice information
about the employee has been populated into the page• Enter all the information that you can in the areas marked with an asterisk using the
blue diamonds to select from the list at each box• After completing the page, click on the Save & Submit button at he bottom• Warning – Make sure you wait for the page to update and show a case number on
the top left side of the page. After MedGate has assigned a case number you can leave MedGate. If you leave MedGate before the case number is assigned all the information will be erased.
